A urine test strip or dipstick test is a basic diagnostic tool used to determine pathological changes in a patient’s urine in standard urinalysis.[1] A standard urine test strip may comprise up to 10 different chemical pads or reagents which react (change color) when immersed in, and then removed from, a urine sample. The test can often be read in as little as 60 to 120 seconds after dipping, although certain tests require longer. Routine testing of the urine with multiparameter strips is the first step in the diagnosis of a wide range of diseases. Point-of-care Blood Ketone Testing: Screening For Diabetic Ketoacidosis At The Emergency Department.

Abstract INTRODUCTION: We piloted the use of a blood beta-hydroxybutyrate meter as a point-of-care testing in an emergency department (ED) to establish its role in distinguishing ketosis/ketoacidosis from simple hyperglycaemia, and develop guidelines for its use in the ED. METHODS: 111 consecutive patients presenting with capillary glucose levels exceeding 14 mmol/L had a simultaneous blood ketone measurement at triage. This was correlated with clinical diagnosis, venous bicarbonate levels and urine ketone testing. RESULTS: The median beta-hydroxybutyrate levels was 5.7 (range 4.3-6.0) mmol/L for patients with diabetic ketoacidosis (DKA) and 0.1 (0.0-3.2) mmol/L for the remaining patients. Only 47.7 percent could provide urine samples in the ED. A blood ketone result of 3.5 mmol/L yielded 100 percent specificity and sensitivity for the diagnosis of DKA. CONCLUSION: This is a useful tool that allows clinicians to immediately distinguish between simple hyperglycaemia and potentially life-threatening ketotic states. When you detect ketones in your body, it is a warning sign that your blood glucose level is too high and you need to bring it down immediately. Ketones are produced when the body burns fat for fuel. Normally these ketones will be completely broken down (metabolised) so that there are very few ketones in the urine. If for any reason the body cannot get enough glucose for energy it will switch to using body fats, causing an increase in ketones in the body. This results in more ketones present in the urine. What are ketones? Ketones are produced when the body burns fat for energy. Normally, your body gets the energy it needs from carbohydrate in your diet. But stored fat is broken down and ketones are made if your diet does not contain enough carbohydrate to supply the body with sugar (glucose) for energy or if your body can't use blood sugar (glucose) properly. Ketones are usually formed in the liver and are broken down so that very small amounts of ketones appear in the urine. However, when carbohydrates are unavailable (for example, in starvation) or can't to be used as an energy source (for example, in diabetes), fat becomes the main source of energy and large amounts of ketones are made. Therefore, higher levels of ketones in the urine indicate that the body is using fat as the major source of energy. High levels of ketones in your body can cause tummy (abdominal) pain, feeling sick (nausea), being sick (vomiting) and diarrhoea. Great results for dAfne singapore – next stop, south-east Asia su-Yen Goh and daphne Gardner Upon returning to Singapore, the newly christened SgDAFNE team adapted the materials shared by OzDAFNE and de- veloped a culturally relevant SgDAFNE programme, including a modified car- bohydrate portion booklet for use in Singapore and the rest of South-East Asia. To the best of our knowledge, ours is the first centre in Asia to of- fer DAFNE, and we are growing and developing. With two courses given in 2011, and at least three more scheduled for 2012, we also provide healthcare professional DAFNE awareness events/ sessions, such as on World Diabetes Day this year. The inaugural course for SgDAFNE in April 2011 was conducted under the watchful eye of an auditor from Diabetes Australia, and OzDAFNE have included us in the OzDAFNE collaborative. We are committed to contributing to the OzDAFNE data- base until such time as the SgDAFNE programme is able to establish its own independent collaborative. Many challenges have surfaced during the development and implementation of SgDAFNE. Prior to DAFNE, there were no structured or standardized education and self-management programmes for people with type 1 diabetes in Singapore. HOW DO THE KETONE TEST STRIPS WORK, AND WHERE CAN I GET THEM? Ketone urine-testing strips, also called Ketostix or just ketone sticks … are small plastic strips that have a little absorptive pad on the end. This contains a special chemical that will change color in the presence of ketones in the urine. The strips may change varying shades of pink to purple, or may not change color at all. The container will have a scale on the label, with blocks of color for you to compare the strip after a certain time lapse, usually 15 seconds. Most folks simply hold a strip in the flow of urine. However, the force of the flow can “wash” some of the chemical away, experts advise that a sample of urine be obtained in a cup or other container, then the strip dipped into it. The chemical reagent is very sensitive to moisture, including what’s in the air. It’s important to keep the lid of the container tightly closed at all times, except for when you’re getting a strip to take a reading. Make sure your fingers are dry before you go digging in! They also have an expiry date, so make note of this when you purchase the strips … that’s for the UNopened package. Once opened, they have a shelf-life of about 6 months — you may wish to write the date you opened on the label for future reference. Ketone test strips can be purchased at any pharmacy, and are usually kept with the diabetic supplies.
