The classic 1983 study by Dr Phinney measured the athletes blood BOHB as 1.28 mmol/L at rest[6]. This makes me wonder if there is a "Goldilocks effect" to ketone levels. Too high may indicate too little protein, and poor glycogen stores, that may impair endurance performance. The level required to be ketogenic (hyperketonemia) has been suggested as 0.2 mmol/L measured as the combination of AcAc and BOHB in whole blood as this is slightly above the levels seen in "normal" individuals[7]. Personally, I'd argue this is too low to be considered ketogenic, as this level is seen in people on a high carbohydrate diet after a night's sleep. The book "The Art and Science of Low Carbohydrate Living" calls the range 0.5 to 5.0 mmol/L of blood ketones "nutritional ketosis"[8] The follow on book "The Art and Science of Low Carbohydrate Performance" suggests that BOHB levels of 0.5 mmol/L to 3.0 mmol/L is "optimal"[9], with benefits starting at 0.5 mmol/L and improving to 3.0 mmol/L, but levels above 3.0 mmol/L not producing additional benefits[10]. (It is unclear what research these levels are based on.) For epilepsy, the recommendation is for AcAc to be 80-160 mmol/L as measured by urine dipstick[11], though this level is not necessarily sufficient[12]. The range of 2 to 7 mmol/L[13] or 2 to 5 mmol/L[14] has been suggested in some literature as a "therapeutic" range. A study of 74 children on the ketogenic diet for epilepsy found that blood BOHB levels of greater than 4 mmol/L were correlated with better seizure control than those with lower levels[12]. Abstract LEARNING OUTCOME: Determine the effect of the level of ketosis on seizure control The ketogenic diet is one of the oldest forms of treatment for seizure control. The diet is designed to maintain ketosis which helps to reduce seizure activity and has improved seizure control in 50–70% of children in past research studies. Limited research has been conducted to determine if the level of ketosis has any effect on seizure control. A retrospective study involving 51 patients (29 male, 22 female) was conducted to determine what effect urine and serum ketone levels have on seizure control and if there is a correlation between urine and serum ketone levels. Age of participants ranged from 2–18 years (median age: 6 years). In this study, 82% (42) of the individuals maintained ketone levels between 80–160 mg/dl; 43% (18) were seizure free, 50% (21) had reduced seizures and 7% (3) showed no change in seizure control. For the 16% (8) of the study participants who maintained ketone levels of 40–80 mg/dL; one was seizure free, 5 had reduced seizures and 2 showed no change in seizure control. Two percent (1) maintained only trace amounts of ketones, however the seizures were decreased.B-hydroxybutyrate (BHB) was available for 31% (16) of the individuals in this study. BHB is one of three serum ketone bodies. Because of its greater concentration and stability relative to acetoacetate and acetone, BHB may become a more reliable guide in monitoring the effect of the diet. BHB > 1.0 resulted in a urine ketone level > 40 mg/dL. A rise in BHB values did not reflect a direct correlation with a rise in urine ketone levels. Go to: Abstract Ketonuria (on standard urine testing) is a frequent finding in children presenting to emergency departments. With the advent of hand‐held ketone meters, blood ketone levels can now be rapidly quantified. Point of care testing (POCT) of blood ketone levels could provide clinically useful information on severity of illness in children and risk of hospital admission. A prospective study using POCT of blood ketone levels in a convenience sample of children <13 years old, with a typical case mix of medical problems. Findings 186 children were studied. The range of ketone levels varied widely among this study population depending on the presenting complaint. Higher levels were noted in those presenting with anorexia or vomiting and fever. The median ketone level of the total study population was 0.2 (range 0–6.0, interquartile range 0.1–0.9) mmol/l. Ketone levels correlated poorly with discharge destination and duration of admission. However, receiver–operator characteristics for ketones as a predictor of admission were comparable to Pediatric Risk of Admission scores (area under the curve 0.64 and 0.72, respectively) and may represent an independent risk factor for admission. A ketone level >1.2 mmol/l has a positive predictive value of 66.7% for admission. Ketone levels correlated well with decreased oral intake (R2 = 0.25; p<0.001). A strong association was found between ketone levels, decreased oral intake and fever. Although ketone levels do not correlate well with more traditional markers of illness severity, they can help to predict the requirement for admission to hospital when interpreted in the context of the presenting illness.
