My Experience With Exogenous Ketones

Last year I wrote a couple of posts on the nuances and complexities of ketosis, with an emphasis on nutritional ketosis (but some discussion of other states of ketosis—starvation ketosis and diabetic ketoacidosis, or DKA). To understand this post, you’ll want to at least be familiar with the ideas in those posts, which can be found here and here. In the second of these posts I discuss the Delta G implications of the body using ketones (specifically, beta-hydroxybutyrate, or BHB, and acetoacetate, or AcAc) for ATP generation, instead of glucose and free fatty acid (FFA). At the time I wrote that post I was particularly (read: personally) interested in the Delta G arbitrage. Stated simply, per unit of carbon, utilization of BHB offers more ATP for the same amount of oxygen consumption (as corollary, generation of the same amount of ATP requires less oxygen consumption, when compared to glucose or FFA). I also concluded that post by discussing the possibility of testing this (theoretical) idea in a real person, with the help of exogenous (i.e., synthetic) ketones. I have seen this effect in (unpublished) data in world class athletes not on a ketogenic diet who have supplemented with exogenous ketones (more on that, below). Case after case showed a small, but significant increase in sub-threshold performance (as an example, efforts longer than about 4 minutes all-out). So I decided to find out for myself if ketones could, indeed, offer up the same amount of usable energy with less oxygen consumption. Some housekeeping issues before getting into it. I get a ton of emails a few months after these consults telling me that they are ecstatic and are now pregnant but are wondering on what to eat now. As if this diet of REAL food would be harmful to a fetus. There are many reasons why to not add in certain foods like gluten and dairy. Many times when cravings get the best of pregnant clients and they consume these foods, the auto-immune response can result in a miscarriage. But even if the clients are committed about staying away from gluten and dairy, they often worry that too low of carbs is bad for the fetus. You will never find evidence of this, but you will read it all over the web. The information that clients read have a few flaws: 1. A huge mistake is when people and doctors compare benign dietary ketosis to diabetic ketoacidosis. You can produce ketones in a starvation state. So instead of using a well-formulated low carb diet, they starved pregnant rats to get them into ketosis. The flaw in that evidence should be obvious. 2. The last form of this “evidence” is when they sliced up the brains of rat fetuses and saturated them in ketones. What happened was that the brain cells lived but it stopped producing new brain cells. This is thought to be evidence that ketosis causes retardation. Now let’s dive into the facts. The lean human body is 74% fat and 26% protein by calories. Of course it does! The problem is how long can you keep it up? A new mother developed a life-threatening condition due to eating a low carbohydrate diet while breastfeeding, doctors claim. The 32-year-old Swedish woman was rushed to hospital with nausea and vomiting, heart palpitations, trembling and spasms in her limbs. When questioned, she said she had been following a a strict low carbohydrate high fat diet (LCHF) in order to lose her baby weight, doctors describing her case in the Journal of Medical Case Reports said. The regime saw the woman, who is unidentified, eating less than 20g of carbohydrate day, the equivalent of a medium-sized potato, or a thick slice of toast, while breastfeeding her 10-month-old son. In the UK, adults are advised to get half of their daily energy intake from carbohydrates, according to a report by the Scientific Advisory Committee on Nutrition, who advise the Government. This is the equivalent of 200 - 240g of carbohydrates in a person eating 2,000 calories a day. The woman said she had lost 4kg on the diet but had begun to feel very ill. In hospital, medics carried out tests and discovered she was suffering from ketoacidosis, a rare but potentially life-threatening condition normally seen in people with type 1 diabetes. ‘The primary diagnosis was thought to be ketoacidosis due to starvation induced by the LCHF diet,’ doctors writing in the journal. When a person has raised blood glucose levels, or are eating a low carbohydrate diet, their body may go into a state of ketosis. If you follow the ebb and flow of the dietary industry, then you have probably heard of the term “ketosis,” “ketones,” or “the ketogenic diet.” You generally hear reference to them when we talk about low-carb, higher fat diets. In a society where low-fat/no-fat is pretty much the accepted standard, this whole low-carb/high-fat concept can seem a little… out there. Especially when there is so much confusion about what ketones are and what they do. THEN, I start talking about the possibility of supplementing with ketones – and people get even more confused. So let’s unpack all of this, learn a little more about ketones, and why taking a supplement might actually be a good thing. Let’s start with the basics. What ARE ketones and what is ketosis? When we think of the body’s natural fuel source, we think of glucose, or sugar. We know that our body needs to use sugar as a fuel, but what many people don’t realize is that fats can be an equally, and even superior, fuel source to the body. The process of ketosis refers to the body’s ability to use fats as its primary source of energy, over glucose. When the body burns fat, it produces three bi-products of fat breakdown, β-hydroxybutyrate (BHB), acetoacetate (ACA) and acetone. These bi-products are called Ketones. This is a naturally occurring process and it’s what allows our bodies to survive during times of food restriction.
