Device: 523/723 (Revel), 522/722, 515/715, 512/712 Important: Using cold insulin can cause air bubbles in the reservoir and tubing. If your insulin is stored in the refrigerator, allow it to reach room temperature before filling your reservoir. When filling the reservoir, take care to remove air bubbles. Remove reservoir from package. Make sure plunger rod is fully extended. Swab the top of the insulin vial with alcohol. Press the transfer guard onto the top of the insulin vial until it snaps into place (make sure you do not push down on the plunger during this step) Push and hold the plunger rod down to pressurize the vial. Continue to hold the plunger rod as you turn the vial over (the vial should now be on top and the reservoir on the bottom). Slowly pull the plunger rod down to fill the reservoir with insulin. Gently tap the side of the reservoir to make any air bubbles rise to the top of the reservoir. Slowly push up on the plunger rod just enough to remove any air bubbles from the reservoir. When all air bubbles are removed, slowly pull the plunger rod down to fill the reservoir with the number of units needed for the next 2 or 3 days. Now turn the vial and reservoir over again (the insulin vial should now be on the bottom and the reservoir on top). Hold the transfer guard with one hand as you turn the reservoir counter-clockwise, and pull up on the reservoir to remove it from the transfer guard. Hold the infusion set tubing connector in one hand and the reservoir in the other. Place the connector onto the top of the reservoir and turn the connector clockwise as you press it gently against the reservoir, until it slides into place. Are you looking for a model that works with a BGM monitor and allows you to administer insulin via a remote? Maybe you need a pump that offers integration with a CGM system. What about size? Color? To use tubes or not to use tubes? Presenting the options so you can find out what pump best fits your lifestyle is our goal. So, with several models on the market, let us help you sort through the choices by taking a look at what’s available and breaking down the various features of each. We’ve separated the pumps into four categories: pumps that incorporate or work with a Blood Glucose Monitor (BGM) and offer Continuous Glucose Monitoring (CGM), pumps with just CGM capability, pumps with just a BGM, and standalone pumps that don’t work with a BGM or offer CGM. Hybrid Closed-Loop System The 670 G system is the newest member to the Medtronic pump family and the first hybrid closed-loop system. In other ways, it is a like a “basal modulator” where you have to still bolus but it predicts your basal rate. Medtronic Facing New Wrongful Death Lawsuit Over Failed Insulin Pump In Wake Of 2009 Recall

The executor of the will of a New Jersey man who died of a night-time diabetes attack is suing Medtronic ($MDT) for the alleged failure of its MiniMed insulin pump and infusion set. Joann Hassan said the company was relying on underqualified high school graduates to test the pump, leading to the device's eventual failure on Charles Slack Jr. Slack's estate says the April 2013 death occurred because he did not receive the correct dosage of insulin. "The pump, set and reservoir failed to take the necessary, expected and warranted steps to alert [Slack] of the failure," according to Courthouse News Service. The specific devices used by Slack were the MiniMed Paradigm Real-Time Revel 523 System and MiniMed Paradigm Quick-Set Infusion Sets, the estate said. Medtronic says the Revel provides continuous glucose monitoring and notification of oncoming high and low blood sugar levels, while the infusion sets consist of thin plastic tubes used to deliver insulin from the pump to the patient, the FDA says. The estate says that the diabetic attack left him "lying in his urine drenched bed, in a state of unconsciousness and unresponsiveness, yet alive." He died after spending the next several weeks hospitalized and in a coma, Courthouse News Service says. Hassan's suit seeks punitive damages for a variety of claims including breach of warranty, negligence and wrongful death. Medtronic's MiniMed infusion sets were hit by a Class I recall notice in June 2009. "These infusion sets may not allow the insulin pump to vent air pressure properly. This could potentially result in the device delivering too much or too little insulin and may cause serious injury or death," the FDA said. After months of waiting, I finally received the world’s first hybrid closed loop system, the Minimed 670G system. Medtronic’s Minimed 670G system is an insulin pump and continuous glucose monitor that also has technology to put you in “auto” mode where it will automatically adjust your basal insulin every 5 minutes based on your blood sugar levels. I’ve had the system for about two weeks now so I thought I’d share my thoughts on it. I’m going to break this down into two posts because I feel the auto mode review needs it’s on page. I’ve been with Medtronic and on an insulin pump since 1997, 20 years! For most of the 20 years, the insulin pump has looked the exact same. It has “mainly” had the same features and not a lot of technology advancements. This new pump, however, is completely different than anything Medtronic has released in the last 20 years, with one caveat that they did release the Minimed 630G a few months prior. I received the Minimed 630G as part of the Minimed 670G Priority Access Group. In case you weren’t aware, Medtronic released a new pump, the 630G, last year and a few weeks (or months, not positive on the timing), the FDA approved their 670G pump faster than they realized. To not waste all the millions of dollars they probably spent on the 630G, they started a Priority Access Group for the 670G where you had to get the 630G first and then once the 670G was released, you could be the first to get it. The stars aligned for me where my pump went out of warranty last year and I had reached my out of pocket max because of the birth of my baby, so I was able to get the 630G for free! MiniMed 630G System Support > Reading Your Device Screen The Home screen appears by default after you change the battery, when you wake the pump from sleep mode, and when you are not actively using another screen. You can navigate to the Home screen from any other screen by pressing and holding the back button for about one second. The following items appear on your Home screen: Displays icons that show a quick status of your pump system. By selecting the status bar you can access more detailed status screens. If you have taken a BG meter reading using your compatible Bayer meter or manually entered a BG meter reading within the last 12 minutes, the BG meter reading appears on the Home screen. You can enter your BG meter reading manually using the Event Markers feature, or when you use the Bolus Wizard to deliver a bolus. Displays the amount of bolus insulin the pump estimates is still working to lower your blood glucose levels. Allows you to access your bolus delivery options, and provides access to all of your insulin settings. If you have not set up the Bolus Wizard and Preset Bolus features, you only have access to Manual Bolus from this screen. When you are on any screen other than the Home screen, press and hold the Allows you to access your basal delivery options, and provides access to all of your insulin settings. To access all of your basal options from this screen, you must have set up a Preset Temp basal rate. This post is something that I knew I had to write, but was not looking forward to it. I am stepping down as a MiniMed Ambassador. There is simply one reason why I am doing this; I am no longer using an insulin pump. Medtronic has been there for me since I first went on a pump 12 years ago. I have been using a MiniMed pump ever since then. Their customer service has always been amazing. Their marketing team has always been great to work with. They have always treated me with the utmost respect and have gone above and beyond for me on any request that I have ever asked of them. As a company, they are one of my favorites. Unfortunately, I have decided to stop using an insulin pump. Why would I go off some of the best technology available in the 670G? Because my diabetes is different than every one elses and its what works for me. I will be going into more depth in tomorrows post as to why I have decided to stop using an insulin pump, but today is a sad post for me. I have enjoyed my time as an Ambassador. I first wanted to become an Ambassador because when I was first going on a pump, I had a million questions. My first question was, is it surgically implanted? I knew NOTHING about an insulin pump. And thats why I wanted to be an Ambassador, because I wanted to help people who had questions. Earlier this month, Medtronic unexpectedly announced FDA approval of the MiniMed 630G. This pump offers the same threshold (low glucose) suspend algorithm as the MiniMed 530G with Enlite, but on the company’s next-gen pump platform – adding a color screen, waterproofing, remote bolusing from the Contour Next Link glucose meter, more alarm customization, insulin on board displayed on the home screen, and other key improvements listed in the table below. The pump is approved for people 16 years and up and will begin shipping in September. Medtronic has more information posted on its website. There was a lot of confusion in the diabetes community about this product and why it is launching, so some common questions are answered below. Can current Medtronic pumpers upgrade to the 630G? Yes, but it is pricey and not covered by insurance if the pump being upgraded is less than four years old. The out of pocket cost to upgrade to the 630G is $599 for Medtronic pumps purchased after May 1, 2016 and $3,100 for pumps purchased before May 1, 2016 (assuming the old pump is sent in to Medtronic). Current MiniMed 530G/Enlite users who want the 630G’s Enlite CGM integration will also need to buy a new transmitter for $511. How does this compare to the 640G (predictive low glucose suspend) available in Europe and the MiniMed 670G (hybrid closed loop) under FDA review? The FDA approved the Medtronic MiniMed 530G insulin pump with its display of the Enlite continuous glucose monitor in September, 2013. The 530G is the first system in the United States that can automatically stop insulin delivery if the CGM's glucose value falls to a preset level (60-90 mg/dl) AND the wearer doesn't respond to the Threshold Suspend Alarm. This functionality puts the device in the newly created OZO: Artificial Pancreas Device Classification created by the U.S. Food and Drug Administration. Although the 530G is an early step toward an artificial pancreas, some consumers have criticized Medtronic's marketing of the 530G using this classification, as discussed below. The 530G will be sold as an integrated system, paired with their CGM and its Enlite sensor. Those who want to use the Medtronic pump with a different CGM like the Dexcom G4 or Navigator would order the Revel 523 or 723 pumps that are identical to the 530G other than its display of the Enlite CGM. These pumps do not have threshold suspend. The Enlite sensor is said to be more comfortable, have slightly better accuracy than the older Guardian sensor, and can be worn up to 6 days rather than 3. Medtronic suggests that the sensor be worn on the abdomen only. It comes with a new Enlite Serter that puts the sensor in at a 90-degree angle while hiding the needle from the wearer. Like the Revel 523 and 723 pumps, the 530G uses the same 180-unit and 300-unit reservoirs and infusion sets. Threshold Suspend (TS) is the suspension of basal insulin delivery when a selected low blood glucose threshold is reached. The goal is to minimize time spent in hypoglycemia. I give up - I'm going to try a pump afterall! In my experience with this condition I learned to Never say Never and Never say Always. The pump can only do what you know is the best thing for whatever is going on now imo. That is of course just my opinion but I love the ability to quickly respond to whatever the day brings. It's not a magic wand, it is just a better way to deliver what and how much insulin you need for any given day, hour, minute, second. And it sometimes never and sometimes often changes on that same schedule. You really don't need luck, you need patience and the ability to adjust without approval of others. Good luck anyway! D.D. Newsflash: Medtronic Launches New Minimed 630g System

A new Medtronic insulin pump is now available in the United States, introducing a new des ign and color-screen as part of a complete makeover to these diabetes devices that have fundamentally looked the same for 30 years! Say hello to the Minimed 630G, which Medtronic announced on Thursday after receiving the FDA's Pre-Market Approval on Aug. 10. This news caught many by surprise, as the California pump-CGM company hadn't previewed this product in advance as they typically do before launches. Instead, they've focused on their forthcoming 670G hybrid closed loop system, expected to be the first-gen commercial Artficial Pancreas system to hit market in 2017. A number of users are actually upset that they weren't warned of this interim product launch, because they've recently purchased new products or upgrades. Others were holding out for the future closed loop system. So we're all wondering: Just what is this 630G that we've heard nothing about before? Remember, Medtronic got FDA approval in September 2013 for its 530G that automatically stops insulin delivery once a user crosses a certain low glucose level. That was step one in moving toward closed loop technology. Next up is their Predictive Low Glucose Suspend (PLGS) features that can anticipate oncoming hypos in advance and shut off insulin to prevent them from happening. That's built into the Minimed 640G system that hit the market outside the U.S. in early 2015. Medtronic ultimately decided it would not pursue bringing that device to the U.S., opting instead to leapfrog that model and focus on getting the first hybrid closed loop 670G to market in 2017.
