I’ve recently received a number of queries from patients and practitioners who are curious about a handful of studies and anecdotal reports that indicate a ketogenic diet may help to curtail cancer growth. For those not familiar with the ketogenic diet, it’s a very low carbohydrate diet that contains moderate amounts of protein and a high percentage of fats. I prefer to think of foods in their whole, natural forms (for example, almonds, apples, asparagus, blueberries, oatmeal, olives, potatoes, rye, and salmon) instead of in reductionist terms of carbohydrates, fats, and proteins. Keeping this in mind, the primary purpose of dietary carbohydrates is for fuel—the body converts carbohydrates via the liver into glucose, which is used for everything from powering muscles to brain function. When confronted with a lack of carbohydrates, the body switches to burning fats for energy by converting fats (again via the liver) into ketone bodies. Here’s why cutting back on carbohydrates appears to curtail cancer growth: Cancer cells primarily use glucose for fuel, but healthy cells primarily use oxygen and secondary glucose, fatty acids, and ketone bodies. The idea behind the ketogenic diet is to starve cancer cells by depriving them of glucose, while continuing to provide healthy cells with ketone bodies for energy. On the surface, the ketogenic diet makes sense. But I don’t recommend it, because although a ketogenic diet may be an improvement over a nutritionally poor diet, it’s not what I consider to be an ideal diet. Some children with seizures saw great success on the diet, often where drugs had failed. Today, the ketogenic diet principles — particularly using coconut oil as the main fat — are being used to treat neurological diseases such as Alzheimer’s, as well as successfully treating cancer. The primary motivation to develop drugs that mimic the effects of the diet is because it is considered “difficult” to follow the diet. The future of cancer prevention and treatment may not be in sexy, high-tech, gene-targeting therapies, but in our own hands…or rather, on our own plates. Which diets work best for cancer treatment and prevention? [This is the 4th in a 4-part article inspired by Dr. Thomas Seyfried’s book Cancer as a Metabolic Disease. To begin with the first article, click HERE.] Cancer Treatments: Standard Therapies vs. Dietary Therapies Standard Therapies Conventional treatments can help in the short-term but can cause problems in the long-term. Chemotherapy is toxic to healthy cells and can breed resistance among cancer cells, increasing the risk of more aggressive cancers if relapse occurs. Radiation turns up the activity of the tumor growth pathway (PI3K/Akt/HIF), which promotes not only tumor growth, but also recruitment of new blood vessels (angiogenesis) and drug resistance. Radiation increases fusion activity between cells, which means that normal and healthy cells can merge into hybrid cells and become more aggressive. Radiation directly damages mitochondria, which increases risk for cancer in the future. Both radiation and immunosuppression therapy (drugs that suppress the immune system) can increase the incidence of metastatic cancers (cancers that spread). Steroids such as dexamethasone (Decadron), often used to reduce inflammation, raise blood sugar levels, feeding tumor cells and enhancing their survival. Dietary Therapies DER (dietary energy restriction) triggers cancer cell death via apoptosis (programmed cell suicide), which is a natural, noninflammatory process that happens from within the cell, causing no collateral damage. The only therapy that targets cancer cells exclusively while not only sparing but also protecting our body cells is dietary therapy. Surgical therapy – this is an excellent therapy in the early stages of the disease when there is no distant spread and the tumour can be totally removed. However, normal tissue is excised with the abnormal cancer tissue which in itself leads to morbidity. For example, in breast cancer the entire breast is removed. In kidney cancer the kidney is removed with the normal parts of the kidney. Luckily we can live without a breast and with only one kidney. However, we cannot remove the entire liver of an entire half of the brain. The remainder harbours some cancer cells which not only grow but grow more aggressively after the surgery. This is because surgical incisions and wounds result in inflammation, oedema (swelling) and release of substances that promote the growth of cancer cells. Once the tumour has spread to distant sites (metastases) surgery is mostly palliative Radiation therapy – For all the advances radiation cannot differentiate between cancer and normal cells and will destroy all cells in their path. Of course the side-effects of radiation are many and include radiation burns, damage to mucosal cells. Cancer research has shifted its direction in the last 10 years towards understanding tumor metabolism and therapeutic approaches for cancer treatment. Emerging evidence indicates that cancer is primarily a metabolic disease and not based on mutations as previously thought. In accordance with this theory, Boston College’s Dr. Thomas Seyfried, author of the book, Cancer is a Metabolic Disease, believes cancer can be controlled with metabolic-based approaches. These therapies include cancer signaling, a therapeutic ketogenic diet, and drugs that target cancerous metabolism. The metabolic theory of cancer is not a new idea; it was discovered in the early 1920s by German scientist Otto Warburg. He won a Nobel Prize for this discovery in 1931. His theory states that, “The prime cause of cancer is the replacement of the respiration of oxygen in normal cells by a fermentation of sugar.” This fermentation can be seen in PET scans which use radioactive glucose to light up the cancerous areas in patients. It shows what sugar hogs cancer cells are. The metabolic theory further states that cancer originates from damage to the cells’ machinery that make ATP (adenosine triphosphate). ATP is energy that is made in the mitochondria of the cell and is used to power virtually every activity that is necessary for life. Healthy cells have a flexible metabolism — cancer cells do not. This broken energy metabolism is seen in all cancers across the board and the greater the degree of damage, the more aggressive the cancer. The story of Elaine Cantin is an example of this, who began to research and modify the ketogenic diet to suit her own needs after seeing too many loved ones die after chemo and radiation. Eventually, her cancer was gone, but this diet is still hidden from most of the general public in obscurity. So what other evidence is there to support the ketogenic diet as a cancer treatment? Let’s face it, cancer can be scary. When you hear the ‘C’ word, fear is often the first feeling to well up from deep inside. You wonder if you’ll ever be okay again. This was my experience when at 28, I was diagnosed with a malignant melanoma of the eye in 2008. I was almost overwhelmed by thoughts of the future, my future. I say ‘almost’ because after I spent three days researching my cancer online, my husband wisely locked away my laptop and gently suggested I take a deep breath and think about what I really needed. And what I needed was perspective; it felt as if nothing was in my control any more, like my cancer had a life of its own, one that was now bigger than mine. Listen to yourself But I did regain control. And here’s how I did it. I listened to my intuition. Returning to my usual lifestyle made no sense. I realised, too, that while there was no way of knowing whether my consultant was making the right decisions, I could have some control over what I put in my mouth – the fuel that feeds my body, my brain and yes, even (back then) my cancer.
