Because type 2 diabetes is linked to high levels of sugar in the blood, it may seem logical to assume that eating too much sugar is the cause of the disease. But of course, it’s not that simple. “This has been around for years, this idea that eating too much sugar causes diabetes — but the truth is, type 2 diabetes is a multifactorial disease with many different types of causes,” says Lynn Grieger, RDN, CDE, a nutrition coach in Prescott, Arizona, and a medical reviewer for Everyday Health. “Type 2 diabetes is really complex.” That said, some research does suggest that eating too many sweetened foods can affect type 2 diabetes risk, and with the Centers for Disease Control and Prevention (CDC) estimating that 30.3 million Americans have the disease — and that millions of more individuals are projected to develop it, too — understanding all the risk factors for the disease, including sugar consumption, is essential to help reverse the diabetes epidemic. Olivia Yang was stunned when she learned she had type 2 diabetes six years ago, when she was 19. Her doctor was shocked, too. In fact, her physician tested her twice to be sure there wasn’t some mistake. Yang was young, had a normal weight for her 5-foot-2-inch frame, and didn’t consider herself a particularly bad eater. She certainly didn’t seem like someone at risk. Now a new study may hint at why some patients end up with type 2 diabetes or prediabetes even when they don’t appear to have all of the typical risk factors such as age, obesity, and an unhealthy diet. Yang learned of her condition sophomore year of college. She’d gone for a physical — a requirement in order to begin working out with a fitness trainer — but her A1C blood test came back abnormally high, indicating diabetes. An A1C test tells a person’s average blood sugar level over the past few months. More specifically, an A1C test measures what percentage of your hemoglobin — a protein in red blood cells that carries oxygen — is coated with sugar. It’s used to diagnose type 1 and type 2 diabetes and to keep tabs on how a person is managing their condition over time. Normal readings land below 5.7 percent. The range for someone with prediabetes falls between 5.7 and 6.4 percent and indicates a high risk of developing diabetes. Anything higher is considered diabetes. The make up about 45% to 65% of calories in a healthy diet (the exact percentage is hotly debated); the rest come from fat and protein. You'll find carbohydrates in the healthiest foods you eat, and in the least healthy. Check the food label to find out exactly how much is in your favorite foods. How you eat can affect blood sugar Choosing the right kind of carbohydrates and spacing them out evenly throughout the day can keep blood sugar from rising too high, too fast (90% of the carbohydrate calories you digest end up as glucose, so they have a much bigger impact on blood sugar than fat or protein). Research into the connection between sugar consumption and diabetes is ongoing. Most doctors argue that sugar alone does not trigger diabetes. But some emerging research suggests a closer link between sugar consumption and diabetes than was previously thought. Can people get diabetes from eating too much sugar? Both type 1 and type 2 diabetes affect the body's ability to regulate blood glucose levels. But eating sugar will not cause type 1 diabetes. If you are living with diabetes, you've probably been told to minimize or eliminate your intake of fruit because "fruit is high in sugar." And if this is the case, maybe you refrain from eating fruits because it causes your blood glucose to spike. Attracted by the smell, color and taste, you may find yourself asking a simple question: "Should I avoid fruit in the long-term? And if so, will I ever be able to eat fruit again?” It turns out that this ant-fruit message is a perfect example of pseudoscience at its best. A recent study published in PLOS medicine tracked the health of 512,891 Chinese men and women between the ages of 30 and 79 for an average of 7 years, in order to understand the effect that their diet had on their overall health (1). We like these types of studies because they are: For those who did not have diabetes at the beginning of the study, those who had a higher fruit consumption were 12% less likely to develop diabetes, compared with those who ate zero pieces of fruit per day. The researchers found a dose-response relationship, which means that the more frequently these nondiabetic individuals ate fruit, the lower the risk for developing diabetes. Amongst those living with diabetes at the beginning of the study, those who ate fruit 3 times per week reduced their risk of all-cause mortality (death from any cause) by 17%, compared with diabetic individuals who ate zero pieces of fruit per day. In addition, researchers uncovered that those who ate fresh fruit 3 days per week were 13-28% less likely to experience macrovascular complications (heart disease and stroke) and microvascular damage (kidney disease, retinopathy and neuropathy). Q: How often should a person with type 2 diabetes eat? A: Everyone needs to eat about every four to six hours during the day to keep energy levels up. People with type 2 diabetes usually have better blood glucose control if their meals and carbohydrates are spaced evenly throughout the day. Too many carbohydrates at any one time can raise blood glucose too high, even if you take diabetes medicine. Many people tend to skip breakfast, eat a light lunch, and then eat too much in the evening. A person with diabetes should attempt to eat about the same amount of carbohydrates at each meal. Breakfast is especially important if you need to control your weight. It helps to jump-start your metabolism and makes you less likely to overeat later. Surveys find that nearly 100% of young women and almost 70% of young men sometimes have food cravings. For most people, cravings at worst add a little weight. With diabetes, they can be a serious problem. Food cravings might cause you to eat way too much of things that spike your sugars. What are food cravings, though? Where do they come from, and how can we deal with them in a healthy way? Let’s divide cravings into two types: physical hunger and emotional distress. It’s normal to feel strong hunger if blood sugar is low, or if your stomach is too empty. Then you really need to eat. If your sugar is low, you might need some carbs; if you just feel empty, some high-fiber vegetables or water might be preferable. Diabetes can cause hunger if glucose is not getting into the cells where it’s needed. Other medical causes of excess hunger include thyroid problems (such as Graves’ disease,) pregnancy, cannabis smoking, and depression. To avoid the cravings of low sugar or empty stomach, remember to eat regularly, especially breakfast. Breakfast with protein should keep cravings away at least until the afternoon. The symptoms of type 2 diabetes (also called type 2 diabetes mellitus) develop gradually—so gradually, in fact, that it’s possible to miss them or to not connect them as related symptoms. Some people are actually surprised when they are diagnosed with type 2 diabetes because they’ve gone to the doctor for something else (eg, fatigue or increased urination). The symptoms develop gradually because, if you have the insulin resistant form of type 2, it takes time for the effects of insulin resistance to show up. If you’re not insulin resistant—and instead your body doesn’t produce enough insulin to process glucose well—the symptoms also develop gradually.
