Diabetes mellitus is a disease that prevents your body from properly using the energy from the food you eat. Diabetes occurs in one of the following situations: The pancreas (an organ behind your stomach) produces little insulin or no insulin at all. (Insulin is a naturally occurring hormone, produced by the beta cells of the pancreas, which helps the body use sugar for energy.) -Or- The pancreas makes insulin, but the insulin made does not work as it should. This condition is called insulin resistance. To better understand diabetes, it helps to know more about how the body uses food for energy (a process called metabolism). Your body is made up of millions of cells. To make energy, the cells need food in a very simple form. When you eat or drink, much of your food is broken down into a simple sugar called glucose. Glucose provides the energy your body needs for daily activities. The blood vessels and blood are the highways that transport sugar from where it is either taken in (the stomach) or manufactured (in the liver) to the cells where it is used (muscles) or where it is stored (fat). Sugar cannot go into the cells by itself. The pancreas releases insulin into the blood, which serves as the helper, or the "key," that lets sugar into the cells for use as energy. When sugar leaves the bloodstream and enters the cells, the blood sugar level is lowered. Without insulin, or the "key," sugar cannot get into the body's cells for use as energy. Diabetes mellitus is a chronic disease caused by inherited and/or acquired deficiency in production of insulin by the pancreas, or by the ineffectiveness of the insulin produced. Such a deficiency results in increased concentrations of glucose in the blood, which in turn damage many of the body's systems, in particular the blood vessels and nerves. There are two principle forms of diabetes: Type 1 diabetes (formerly known as insulin-dependent) in which the pancreas fails to produce the insulin which is essential for survival. This form develops most frequently in children and adolescents, but is being increasingly noted later in life. Type 2 diabetes (formerly named non-insulin-dependent) which results from the body's inability to respond properly to the action of insulin produced by the pancreas. Type 2 diabetes is much more common and accounts for around 90% of all diabetes cases worldwide. It occurs most frequently in adults, but is being noted increasingly in adolescents as well. Certain genetic markers have been shown to increase the risk of developing Type 1 diabetes. Type 2 diabetes is strongly familial, but it is only recently that some genes have been consistently associated with increased risk for Type 2 diabetes in certain populations. Fatigue is one of the most common disabling diabetes symptoms. Diabetes fatigue can disrupt and interfere with all aspects of daily living. What causes diabetes fatigue, and why is it so common? We’ve written about fatigue before and received tons of great comments on those posts. But this time let’s go deeper and find the whole range of causes and solutions, even if it takes a few weeks. Hopefully, everyone will find something that might help them, because this is a serious problem. For example, Melanie wrote, “[Fatigue] really takes a toll on my family and things we can do. I just want to have the energy to play with my son and to do things around the house or with friends…I can’t drive more than 30 minutes because my husband is afraid I will fall asleep…and wreck [the car]. (I have dozed while driving before.)” Maria commented, “Fatigue is a constant and I have had to learn to do only what I can. I don’t push myself anymore as I pay for it dearly. It is usually (although not exclusively) seen in young people. Type 2 diabetes or non insulin-dependent diabetes. It tends to affect adults over 40 and overweight people, although it is now becoming commoner amongst younger people. Type 2 diabetes occurs more frequently in people of South Asian and African-Caribbean descent. There are also other types of diabetes. Women can develop diabetes during pregnancy. Gestational diabetes, as it is sometimes referred to, usually disappears after the birth of the baby. However, having gestational diabetes increases the risk of type 2 diabetes later in life. Existing type 1 diabetes may be exacerbated during pregnancy. This is when diabetes is caused as the result of another condition, eg inflammation of the pancreas, or by the use of certain medication such as diuretics or steroids (the most common cause). Most people believe that people with type 1 diabetes are not insulin resistant simply because they are not overweight. This could not be farther from the truth. While insulin resistance affects many overweight individuals, many people with type 1 diabetes remain skinny their entire lives despite a large degree of insulin resistance (1–3). Over the past decade, I have helped many people with type 1 diabetes measure, track and reverse insulin resistance. In practice, 100% of all my clients with type 1 diabetes suffer from insulin resistance despite the assumption that they were insulin sensitive. By measuring their baseline insulin resistance, we were able to identify an impaired ability to utilize glucose as a fuel, and through dedicated diet modification and frequent exercise, some of my clients have reduced their insulin usage by as much as 60%. If you have type 1 diabetes, do not be fooled into thinking that you are insulin sensitive simply because you are skinny. Diabetes is not an inmmune compromising (IC) disease per se, although some reports demonstrate immune cell activity dysfunction the findings are not clinically relevant. In my experience well controlled diabetics do not suffer more infectious diseases or cancers or opportunistic diseases than the general pupolation. Acute or chronic diabetes decompensation can lead to several medical and surgical complications but they are not related to IC. They are related to hyperglycemia, low tissue oxigen tension secondary to vascular damage (macroangyopathic/microangyopathic damage), acute changes in hydroelectrolytic balance, blood pH, ketoacidosis and several other issues. Infections of the lower limbs (Diabetic foot) are more frequent than in the general pop but they are caused by vascular damage, hypoperfusion and tissue necrosis in the presence of high glucose levels.
