A study on metformin, also sold as Glucophage and Novo-Metformin, says nearly one in four patients could experience dangerous side effects. The study is published in the most current issue of the Journal of the American Medical Association. Metformin helps the body use insulin and is one of the most common drugs used to treat Type II diabetes, sometimes linked to obesity and called "adult-onset" or "non-insulin dependent" diabetes. There are at least 1.2 million Canadians with diabetes according to Health Canada. More than 90 per cent are Type II. Metformin can cause a side-effect called lactic acidosis, a buildup of lactic acid in the blood that is fatal in half of all cases. The label says it shouldn't be used by patients with kidney disease or by those taking heart medication. A study of metformin patients by the University of North Carolina in Chapel Hill found a quarter met that criteria. Fortunately, none of the patients developed lactic acidosis. "It is difficult to determine whether clinicians are aware they are prescribing metformin against a black-box warning," wrote the researchers. "Black-box" refers to the highlighted cautionary information on labels of drugs that have serious side effects. This medication has been around for more than half a century and is often touted as a wonder drug for individuals with type 2 diabetes as well as for those living with other health challenges. My reasons for taking metformin were highly personal: I have a genetic predisposition for both prostate cancer and type 2 diabetes (Im not diabetic but I swing in and out of pre-diabetes based on my daily blood sugar readings), and Im getting older (who isnt!). I did copious amounts of research and it seemed on all three counts ( prostate cancer prevention , managing type 2 diabetes (increasing insulin sensitivity), and anti-aging), it truly was a wonder drug. Oh, and I had also read that pretty much every billionaire in Silicon Valley was on it mainly for its purported life extension benefits (as it can mimic the effect of calorie restriction see more below). Before I went on it I wanted to get some key blood indicators taken, specifically: Testosterone (total, free, and bioavailable). A level of 160 is equivalent to an A1c level (a more commonly used measure of blood sugar control) of about 7.2 percent This is a reasonable level for most people in their 70s, where lower levels ("tighter" control) increase the risk for dangerously low blood sugars. Most medicines are more effective at higher doses. For metformin, 500 mg twice daily is the most common dose I see. Although it can be dosed up to 1,000 mg twice daily, it's not much more effective in most people, and is more likely to cause side effects, especially nausea. Should I already take metformin as prediabetic? I had gestational diabetes and gave birth to my son in October last year. Last week I took the follow up OGTT. The fasting numbers were ok (97), but after two hours i had 197. My GP wanted to look at my A1C so I had that test done yesterday. Fasting numbers were good again (92) and I had an A1C of 5% even after not really paying extra attention to my food the last couple of months. I didn't follow any kind of diet, but I don't eat outrageous meals either. I was slightly overweight at the start of my pregnancy, but because I was very strict about my diet with GD, I only gained 4kgs. I now weigh about 5kg less than before I got pregnant and am back to a normal BMI. Anyway. I'm puzzled. My GP said I'm prediabetic, but shouldn't the A1C be elevated as well? Especially after three months of eating basically everything that I wanted including many desserts during the holiday season. I was put on metformin, but I was wondering if that is really necessary. Abstract Insulin resistance (IR) plays a pivotal role in PCOS. Insulin-sensitizer agents such as metformin and inositols have been shown to improve the endocrine and metabolic aspects of PCOS. The purpose of this study is to compare their effects on the clinical and metabolic features of the women with PCOS. Fifty PCOS women with IR and/or hyperinsulinemia were randomized to treatment with metformin (1500 mg/day) or myo-inositol (4 g/day). IR was defined as HOMA-IR >2.5, while hyperinsulinemia was defined as a value of AUC for insulin after a glucose load over the cutoff of our laboratory obtained in normal women. The Matsusa Index has been calculated. The women have been evaluated for insulin secretion, BMI, menstrual cycle length, acne and hirsutism, at baseline and after 6 months of therapy. The results obtained in both groups were similar. The insulin sensitivity improved in both treatment groups. The BMI significantly decreased and the menstrual cycle was normalized in about 50% of the women. No significant changes in acne and hirsutism were observed. The two insulin-sensitizers, metformin and myo-inositol, show to be useful in PCOS women in lowering BMI and ameliorating insulin sensitivity, and improving menstrual cycle without significant differences between the two treatments.
