This article has been cited by other articles in PMC. The global prevalence of diabetes mellitus is increasing rapidly. Measurement of glycated hemoglobin, predominantly HbA1c, is fundamental to the management of patients with diabetes. HbA1c is used to monitor long-term glycemic control, adjust therapy, assess the quality of diabetes care and predict the risk for the development of complications. While HbA1c is the standard method for long-term glycemic control in diabetic patients, there are different methods for measurement of HbA1c and all laboratories do not use the reference method (high-performance liquid chromatography [HPLC]). The objective of this study is comparison of three different methods with HPLC to find out which method has an acceptable concordance and correlation with the reference method. Fifty-eight diabetic patients were assessed in this study. The blood sample of each patient was checked with Diazyme (enzymatic assay), Nycocard (boronate-affinity binding) and Biosystem (micro column chromatography). Results of HbA1c measuring is used in management diabetic patients. So agreement between results of different HbA1c measuring methods and kits is critical in medical decision making.Results of measring HbA1c of 55 patients blood samples with five commonly used kits in Iran, including Pars Azmon, Pishtaz Teb, Biosystem, Roche, and NycoCard, compared with total mean results. All kit results showed good correlation (r>0.96) with total mean results. In paired t-test analysis, results of Pars Azmon, Biosystem, and NycoCard had no statistically significant difference with total mean results, but results of Pishtaz Teb and Roche kits were significantly lower and higher than total mean results, respectively. However, this differences were clinically insignificant.With respect to results of external quality assessment program (EQAP) in Iran and results of this study, analytical performance of studied kits are acceptable, except for Roche kit which had a positive bias. Afinion, in which AxisShield has invested about 20m in research and development and capital expenditure costs, is designed for use in doctors surgeries and hospital point-of-care sites. The firm introduced the HbA1c test kit that accompanies it in the US and Nordic countries last year, but said the UKmarket is still almost nonexistent, despite having two machines placed with the army. However, concerns have persisted over the past year because the roll-out has been delayed by regulatory approval times and manufacturing issues. Objective: To assess four commercially available point-of-care HbA1c analytical systems with respect to (1) inaccuracy against the central laboratory HbA1c method and (2) imprecision against the HbA1c analytical goal of coefficient Methods: Analytical inaccuracy was assessed by analysis of 110 patient samples on all five analytical platforms (Biorad Diastat, Drew and Roche Tinaquant (used in central lab)). Analytical imprecision was assessed by analysis of two levels of patient sample four times daily for six days, as well as analysis of two levels of Results: Deming linear regression for agreement: Diastat=0.98 * Tinaquant + 0.36; DS5=1.23* Tinaquant-0.65; DCA2000=0.95 * Tinaquant + 0.63; Nycocard=0.94 * Tinaquant + 0.92. Analytical coefficients of variation (CVs) at Tinaquant HbA1c levels of 6.2-10.8% were: Tinaquant 0.8-1.1%, Diastat 1.6-6.6%, DCA2000 2.6-7.2%, DS5 5.1-11.7%, Nycocard 8.5-15.3%. Two HbE samples gave elevated HbA1c results with the Conclusions: The Diastat and DCA2000 systems gave the best performance with acceptable the central lab and each other. The DS5 was less precise with a significant positive bias compared to the other methods and interference from HbE, while the Nycocard system showed the poorest precision in the evaluation. Current practice and advantages over existing technology: The clinical process to assess patients with suspected diabetes typically involves at least two appointments with a GP/practice nurse; blood samples being taken during the first visit and 1-2 weeks later results being discussed with the patient, after laboratory analysis. If an elevated HbA1c is found and there are no other symptoms then a repeat blood test would normally be undertaken, adding to the length of time taken to reach a diagnosis. Furthermore, once diagnosed with diabetes, HbA1c concentration is monitored on a regular basis (every 3-6 months) to assess control of blood glucose concentration ( 5 ). As an alternative, point-of-care testing (POCT) aims to provide immediate results at the time of patient consultation to enable therapeutic decisions to be made at the earliest possible opportunity, resulting in fewer patient visits and improved glycaemic control. POCT is defined as clinical testing close to the site of patient care, typically with small/portable instruments. There is some evidence supporting the use of POCT for HbA1c analysis: studies report on an overall improvement of clinical outcomes in the primary as well as secondary care settings after usage of POCT in the management of diabetes ( 6 ).
