With this type of diabetes, insulin produced by the pancreas is not able to get sugar into the cells of the body where it can work properly. Using metformin alone, with a type of oral antidiabetic medicine called a sulfonylurea, or with insulin will help to lower blood sugar when it is too high and help restore the way you use food to make energy. Many people can control type 2 diabetes with diet alone or diet and exercise. Following a specially planned diet and exercising will always be important when you have diabetes, even when you are taking medicines. To work properly, the amount of metformin you take must be balanced against the amount and type of food you eat and the amount of exercise you do. If you change your diet, your exercise, or both, you will want to test your blood sugar to find out if it is too low. Your health care professional will teach you what to do if this happens. At some point, metformin may stop working as well and your blood glucose will increase. You will need to know if this happens and what to do. Instead of taking more metformin, your doctor may want you to change to another antidiabetic medicine. When I reviewed the research that went into my book, Diet 101: The Truth About Low Carb Diets I was surprised to learn just how poorly low carb dieters fared in the research studies that I had always seen cited in the low carb community as proving the diet superior to others. Partly this is because few diets of any kind work well for more than a few months, and looked at in that time frame, the low carb diet often surpasses others in it's effectiveness. But once most people get past the first six months of of their low carb diets, peer reviewed research and the posts of hundreds of people who visit online low carb discussion boards make it very clear that many if not most people see their weight loss stall out completely. This doesn't have to be the case. There are a few, very interesting studies, that show that weight loss can continue on low carb diets, though at a much slower pace than dieters grew accustomed to during the first heady months of their diets. I’m sure I was “pre-diabetic” for several years before an appendectomy and subsequent post-operative infection threw me over the edge into full blown Type 2 diabetes. I was sent to diabetes education classes and put on metformin. That was around 12 years ago. Metformin did a fine job taking care of my diabetes for me with very little effort on my part. My A1C was always in the 6.0 range, fasting sugars were in the 120-130 range, blood pressure, cholesterol, triglycerides all near normal (with applicable medications doing their job). A few months ago, my blood pressure began creeping up, and my blood sugars, fasting or not, began to creep higher and higher, and I began to gain weight. My A1C jumped to 7.1 in less than 6 months. Exercise didn’t help. I felt so sad, knowing that I have not done everything I could do to control my diabetes (let’s be honest – not much at all), instead, depending on the metformin to do it all for me. I met with my doctor, and he advised that not only is diabetes progressive, but that metformin eventually loses its effectiveness. He said most people get 2-3 years before they have to add another medication to their metformin regimen. He started me on Victoza along with the metformin. Concern about the cardiovascular safety of metformin and sulphonylurea combination is still under debate, but several large outcome studies didn’t show increased risk of this therapy. In conclusion, metformin and sulphonylureas are unable to prevent the long-term progression of hyperglycemia in most of type 2 diabetes patients. The tailoring of treatment to the individual patient remains the most important key to successful diabetes therapy. People with type 2 Diabetes (T2D) develop severe chronic complication early in the course of the disease if not treated optimally. These work by causing your pancreas to release insulin. But the medicines can lose their effectiveness after several years. In the past, adding or switching to insulin were your only options. Now there are several options, including adding a medication such as metformin (Glucophage), troglitazone (Rezulin), or acarbose (Precose) to the medicine you’re already taking. Because these new medicines work differently from sulfonylureas, they often result in additional blood sugar lowering, says the American Diabetes Association (ADA). All of these drugs reduce glycosylated hemoglobin by 1%-2% when added to a sulfonylurea. Talk to your diabetes care team about the pros and cons of adding another diabetes medicine to your schedule.
