Background Type 2 diabetes (formerly known as non?insulin-dependent diabetes or adult-onset diabetes) commonly begins as insulin resistance, with cells using insulin incorrectly. Gradually, the pancreas loses its ability to produce insulin. According to the National Institutes of Health, 18.2 million Americans (6.3% of the US population) have diabetes.1 Type 2 diabetes accounts for ~90% to 95% of all diabetes. This disease is associated with obesity, a family history of diabetes, older age, and physical inactivity. Type 2 diabetes also is more prevalent among African Americans, Hispanic/Latino Americans, American Indians, and some Pacific Islander and Asian Americans. Type 2 diabetes has been treated with several antihyperglycemic drugs. Metformin, used for almost 50 years, has been shown to reduce all-cause and diabetes-related mortality and macrovascular disease risk.2 Fortamet (metformin hydrochloride) Extended-Release (ER) Tablets, marketed by Andrx Laboratories, are a new once-daily formulation. Pharmacology Metformin, a biguanide, acts by decreasing hepatic glucose production, decreasing intestinal absorption of glucose, and improving insulin sensitivity by increasing peripheral glucose uptake and utilization. Fortamet ER uses a single-composition osmotic technology (SCOT), consisting of an osmotically active core formulation surrounded by a semipermeable membrane. Two laser-drilled holes on either side of the tablet enable the drug to exit the tablet at a constant rate.3 Absorption is increased by ~60% when the drug is taken with food. The half-life is approximately 5.4 hours, and ~90% of the drug is renally eliminated within the first 24 hours. Clinical Trials A double-blind, randomized, active?controlled study of a total of 680 patients compared Fortamet ER wit Continue reading >>

[effect Of Metformin On The Clinical Course Of Gout And Insulin Resistance].

Abstract The aim of this prospective study was to evaluate results of metformin (MF) therapy during 1 year of uric acid (UA) metabolism and the clinical course of gout with insulin resistance (IR). The study included 30 patients (28 men and 2 women) of mean age 51 yr and duration of he disease 4-11 yr. IR was diagnosed based on the HOMA index. INCLUSION CRITERIA: the absence of anti-gout therapy, normal renal and hepatic function, abstinence. The patients were given 1500 mg MF/day. The measured parameters included anthropometric and clinical characteristics, 24 hour AP, plasma UA, glucose, insulin, urea, creatinine, ALT, AST, lipid spectrum at the first and subsequent visits. UA clearance and excreted UA fraction were calculated. UA level decreased from 569 +/- 109.5 to 442.8 +/-107.4 mcmol/l (p < 0.01) after 12 months of MF therapy. Normouricemia ( < 360 mcmol/l) was achieved in 11 patients. Fasting insulin level dropped by 35% (from 23.9 to 15.9 mcU/ml, p < 0.01), HOMA index from 6.5 to 3. 7(p < 0.01). Serum glucose, cholesterol, triglycerides, and LDL cholesterol decreased while HDL cholesterol increased. Parameters of renal UA regulation and anthropometry remained unaltered. MF therapy resulted in a decrease of serum UA, insulin, and the degree of IR. The hypouricemic effect of MF was unrelated to renal UA excretion, reduced AP and body weight. It is hypothesized that MF reduces production of UA in patients with gout due to inhibition of synthesis of free fatty acids. Continue reading >>

New Takeda Type 2 Diabetes Treatments Nesina® (alogliptin) And Kazano™ (alogliptin And Metformin Hydrochloride) Now Available For Quebec Patients

New Takeda Type 2 Diabetes Treatments Nesina® (alogliptin) And Kazano™ (alogliptin And Metformin Hydrochloride) Now Available For Quebec Patients

Decision to reimburse provides new treatment options to help individualize the management of type 2 diabetes OAKVILLE, Ontario – April 15, 2015 – Takeda Canada Inc. announced today that NESINA® (alogliptin) and KAZANO™ (alogliptin and metformin hydrochloride), two new treatments for type 2 diabetes, are now available for Quebec patients. The availability of these new treatments in Quebec is the result of a decision by the Quebec government to reimburse them following Health Canada approval of the treatments. NESINA and KAZANO are included on the province’s public drug plan with codes to provide facilitated access to the product under the medicament d’exception program. Alogliptin is in the class of treatments known as DPP-4 (dipeptidyl peptidase-4) inhibitors. Across a number of trials and patient populations, alogliptin 25 mg tablets taken once daily demonstrated clinically and statistically significant reductions in HbA1c (blood glucose levels), with low incidence of hypoglycemia. "As treating physicians, an important objective in the management of type 2 diabetes is to help patients reach their target HbA1c levels,” said Dr. Jean-Marie Ekoé, M.D., an endocrinologist at the Centre hospitalier de l’Université de Montréal (CHUM) and professor at the Université de Montréal. "We start with the obvious which is diet and exercise and move onto medications, starting with metformin. It is essential for us to have multiple proven treatments available for patients who need to move on from metformin. NESINA and KAZANO are two new welcome additions.” There are more than 830 000 Quebecers who live with diabetes, which represents more than 10 per cent of the provinces population. “With more treatment options, more Quebecers with diabetes will be able to cont Continue reading >>

Metformin (glucophage) Side Effects & Complications

Metformin (glucophage) Side Effects & Complications

The fascinating compound called metformin was discovered nearly a century ago. Scientists realized that it could lower blood sugar in an animal model (rabbits) as early as 1929, but it wasn’t until the late 1950s that a French researcher came up with the name Glucophage (roughly translated as glucose eater). The FDA gave metformin (Glucophage) the green light for the treatment of type 2 diabetes in 1994, 36 years after it had been approved for this use in Britain. Uses of Generic Metformin: Glucophage lost its patent protection in the U.S. in 2002 and now most prescriptions are filled with generic metformin. This drug is recognized as a first line treatment to control blood sugar by improving the cells’ response to insulin and reducing the amount of sugar that the liver makes. Unlike some other oral diabetes drugs, it doesn’t lead to weight gain and may even help people get their weight under control. Starting early in 2000, sales of metformin (Glucophage) were challenged by a new class of diabetes drugs. First Avandia and then Actos challenged metformin for leadership in diabetes treatment. Avandia later lost its luster because it was linked to heart attacks and strokes. Sales of this drug are now miniscule because of tight FDA regulations. Actos is coming under increasing scrutiny as well. The drug has been banned in France and Germany because of a link to bladder cancer. The FDA has also required Actos to carry its strictest black box warning about an increased risk of congestive heart failure brought on by the drug. Newer diabetes drugs like liraglutide (Victoza), saxagliptin (Onglyza) and sitagliptin (Januvia) have become very successful. But metformin remains a mainstay of diabetes treatment. It is prescribed on its own or sometimes combined with the newer d Continue reading >>

What Is It?

What Is It?

What Is It? Gout is a disorder characterized by too much uric acid in the blood and tissues. In gout, crystals of uric acid are deposited in the joints, where they cause a type of arthritis called gouty arthritis. These same crystals can also deposit in the kidneys, where they can cause kidney stones. There are three main causes of the high levels of uric acid that lead to gout: A diet rich in chemicals called purines, because purines are broken down by the body into uric acid. Foods that contain high levels of purines include anchovies; nuts; and organ foods such as liver, kidney and sweetbreads. High production of uric acid by the body. This can happen for unknown reasons. It can also occur in certain inherited genetic metabolic disorders, leukemia and during chemotherapy for cancer. The kidneys do not excrete enough uric acid. This can be caused by kidney disease, starvation and alcohol use, especially binge drinking. This also can occur in people taking medications called diuretics (such as hydrochlorothiazide or furosemide). Obesity or sudden weight gain can cause high uric acid levels because the body's tissues break down more purines. In some people, gout is caused by a combination of these factors. People with a family history of gout are more likely to develop the condition. About 90% of patients with gout are men older than 40. Gout is quite rare in younger women; when women develop gout, they are usually post-menopausal. Symptoms The first attack of gouty arthritis usually involves only one joint, most commonly the big toe. However, it can affect other joints, such as a knee, ankle, wrist, foot or finger. In gouty arthritis, the joint can become red, swollen and extremely tender to the touch. Typically, even a bed sheet brushing against the joint will trigger Continue reading >>

Metformin Hydrochloride Side Effects

Metformin Hydrochloride Side Effects

Gout is a common, painful form of arthritis. It causes swollen, red, hot and stiff joints. Gout happens when uric acid builds up in your body. Uric acid comes from the breakdown of substances called purines. Purines are in your body's tissues and in foods, such as liver, dried beans and peas, and anchovies. Normally, uric acid dissolves in the blood. It passes through the kidneys and out of the body in urine. But sometimes uric acid can build up and form needle-like crystals. When they form in your joints, it is very painful. The crystals can also cause kidney stones. Often, Gout first attacks your big toe. It can also attack ankles, heels, knees, wrists, fingers, and elbows. At first, Gout attacks usually get better in days. Eventually, attacks last longer and happen more often. You are more likely to get Gout if you Gout can be hard to diagnose. Your doctor may take a sample of fluid from an inflamed joint to look for crystals. You can treat Gout with medicines. PseudoGout has similar symptoms and is sometimes confused with Gout. However, it is caused by calcium phosphate, not uric acid. Metformin Makes Headline News

Metformin Makes Headline News

Metformin is the first-line drug of choice in the treatment of type II diabetes. It was first approved in Europe in 1958.1 Americans had to wait until 1994 to legally obtain metformin.1 The holdup in approving metformin goes beyond the FDA. It is an indictment of a political/legal system that will forever cause needless suffering and death unless substantively changed. When Life Extension® informed Americans about drugs like metformin in the 1980s, the FDA did everything in its power to incarcerate me and shut down our Foundation.2 FDA propaganda at the time was that consumers needed to be "protected" against "unproven" therapies. As history has since proven, the result of the FDA's embargo has been unparalleled human carnage. So called "consumer protection" translated into ailing Americans being denied access to therapies that the FDA now claims are essential to saving lives. Today's major problem is not drugs available in other countries that Americans can't access. Instead, it is a political/legal system that suffocates medical innovation. Headline news stories earlier this year touted the anti-cancer effects of metformin, data that Foundation members were alerted to long ago.3 The problem is that it is illegal for metformin manufacturers to promote this drug to cancer patients or oncologists. It's also illegal to promote metformin to healthy people who want to reduce their risk of cancer, diabetes, vascular occlusion, and obesity. This fatal departure from reality continues unabated, as our dysfunctional political/legal system denies information about metformin that could spare countless numbers of lives. Type II diabetics suffer sharply higher rates of cancer4-7and vascular disease.8-11 The anti-diabetic drug metformin has been shown in numerous scientific studies Continue reading >>

Answers (1) Like the answers? Chat privately with the doctor of your choice Normal Uric acid levels are 2.4-6.0 mg/dL (female) and 3.4-7.0 mg/dL (male) Some diets that are rich in purine end up causing an increased production of uric acid. In other cases, it may be due to a completely sedentary lifestyle. Less intake of water also may be to blame in some cases as water plays an important role in washing out the excess of the uric acid. Regular usage of some medicines also cause the uric acid levels to rise. Another important factor is being overweight as obese people are much more likely to get gout as compared to those who have normal weight. It is a tragedy of the modern world that the superior advertising and marketing power of the allopathic system of medicine has eclipsed the good and genuine work of Homeopathy. This is largely because Homeopathic system of medicine cannot compete with the money power of Allopathy. Just because allopathy does not have any cure for gout, patients tend to think that there is no cure for gout at all. People are not aware of the great work that Homeopathy can do in the cure of gout. I cannot stress enough here that homeopathic medicines are pretty effective at curing the acute as well as the chronic gout You can easily take an online consultation for further treatment guidance and permanent cure without any side effects Disclaimer : The content is not intended to be a substitute for professional medical advice, diagnosis, or treatment. This type of diabetes is also known as non-insulin dependent diabetes mellitus (NIDDM) or maturity onset diabetes. when diet, exercise and treatment with either metformin or a sulfonylurea medicine are not enough to control your blood glucose in people whose blood sugar levels are already well controlled by the combination of metformin and a sulfonylurea medicine. Glucovance is to replace these two medicines. Glucovance contains two active ingredients, metformin hydrochloride and glibenclamide. Metformin belongs to a group of medicines called biguanides. Metformin lowers blood glucose by helping your body make better use of insulin. Glibenclamide belongs to a group of medicines called sulfonylureas.
