KANSAS CITY, KANSAS. Metformin (glucophage) is a widely used drug for the treatment of type 2 diabetes. It is relatively benign but is associated with a serious, but rare side effect called lactic acidosis, a condition in which lactic acid builds up in the blood stream faster than it can be eliminated and thereby creates an acidic milieu in the body. Lactic acidosis can also result from intense exercise and impaired kidney function. A group of cardiologists from the University of Kansas Hospital and Medical Center now reports that case of a 68-year-old man who developed atrial fibrillation (AF) as a result of treatment with metformin. The patient had normal liver and kidney function, but did suffer from hypertension, type 2 diabetes, anxiety disorder, and coronary heart disease, and was on 6 different medications. At a follow-up visit with his primary physician his prescription for sitagliptin (Januvia), an oral anti-diabetic drug, was replaced with one for metformin. Seventy-two hours after taking the first dose of the new drug, the patient reported to the emergency department with AF. He reported a similar episode in the past when he was on metformin, which resolved when he discontinued the drug. After arriving in the ER the metformin was discontinued and the patient converted to normal sinus rhythm (NSR) on his own after 48 hours. Once in NSR he was rechallenged with metformin and once again 48 hours later went into AF. NSR was restored 48 hours after discontinuing the metformin. The Kansas cardiologists confirmed a build-up of lactate during metformin administration and conclude that the patient developed lactic acidosis subsequent to metformin administration and that this, in turn, caused a bout of AF. I have had Type 2 for several years. Seem to be doing fine until I was diagnosed with atrial fibrillation. Was put on Metropolol twice day. I take Metformin 2x day/500mg each. I have been taking the Metropolol for 4 months now. I was also told to take Lovestatin and a regular aspirin a day, 325mg. I was diagnosed at the ER as I had an "occurrance" at 11pm. My doc declined to put me on Coumadin as I was not in A-Fib continually but only had an occurrance when I drank any caffeine or had high stress. I hate this medication. I am so weak, I can't even open jars anymore. I can pick up my grandbaby without fear of dropping him. I just read that statins can decrease effectiveness of exercise. I want to go off all this medicine. I have searched the internet for info on A-Fib but am not happy with what I have found. I don't know what happens, what causes it, how to prevent it, if possible. I got no info at the ER or from my doc. (I realized that later after my initial reaction to being diagnosed with "heart problems"). Has anyone else had this problem? They did not link the A-Fib to my diabetes, and I am not doing so either but I am wondering if there could be a link. Does anyone know of a really good website with lots of info, technical, that will help? Or any other ideas? D.D. Association of metformin with lower atrial fibrillation risk among patients with type 2 diabetes mellitus: a population-based dynamic cohort and in vitro studies Chang et al.; licensee BioMed Central Ltd.2014 Atrial fibrillation (AF), an inflammatory process involving arrhythmia, is associated with severe morbidity and mortality and commonly seen in patients with diabetes mellitus (DM). The effect of metformin, the most commonly used medication for patients with DM, on AF has not been investigated. The primary aim of this study was to examine whether metformin prevented the occurrence of AF in type 2 DM patients by analyzing a nationwide, population-based dynamic cohort. Additionally, we investigated the effect of metformin on tachycardia-induced myolysis and oxidative stress in atrial cells. The study population included 645,710 patients with type 2 diabetes and not using other anti-diabetic medication from a subset of the Taiwan National Health Insurance Research Database. Of these patients, those who used metformin were categorized as the user group, and the remaining were classified as the non-user group. The time-dependent Coxs proportional hazard model was used to examine the effect of metformin on AF and the status of metformin use was treated as a time-dependent covariate. HL-1 atrial cells were paced with or without metformin, and then troponin and heavy-chain-myosin were measured as markers of myolysis. After 13years of follow-up, 9,983 patients developed AF with an incidence rate of 1.5% (287 per 100,000 person-years). After adjusting for co-morbidities and medications, metformin independently protected the diabetic patients from new-onset AF with a hazard ratio of .81 (95% confidence interval 0.76-0.86, p < 0.0001). The doctor started me on 2 a day to ease me in for a week. Initially I did have very smelly farts (frankly, was happy to pay my husband back for all those years!!!! and sometimes loose bowels but it settled quite quickly. Again, not sure how much it actually helps my blood sugars but I had a very high reading at first so doctor didn't really give me an option! I have been on Metformin for 8 years now & my sugar stays 300 +. Not 1 dr. has been able to bring my sugar down in 8 years. What is wrong with that picture ? I have been taking Metformin for about 17 years - since diagnosis, and have had no problems with side effects. When I got my bloods into normal figures, I suggested to my GP that I might stop taking it. She wanted me to remain on it because of its heart benefits. It is also under investigation for dealing with other health problems. I didn't have any problems starting on Metformin, although I take the SR form, as it costs our NHS only a little more than the standard tablets. There is evidence that Metformin improves cardiovascular outcomes in Type-2. There is also evidence I've seen that its effectiveness in reducing BG is genetically linked. One could assume likewise for the adverse effects. My side effects initially, in 2011, came from the Ramipril and later the Statin. I've seen some evidence that Statins negate the benefits of Metformin! Type 2 since 2004 and on metformin for most of that time with no side affects at all. The global prevalence of type 2 diabetes mellitus (DM) is 9% among adults, causing millions of deaths. [1] The primary goal of DM treatment is to reduce the progression of cardiovascular complications through optimal glycemic control. Virtually all patients with early DM are advised to modify their lifestyle such as with appropriate diet control and exercise as initial glycemic control with or without oral hypoglycemic agents (OHAs). OHAs may not be administered for the patients in whom glycemic control is optimal after lifestyle modifications. Metformin is recommended as first-line therapy in patients without contraindications based on reports demonstrating the reduction of major adverse cardiovascular events (MACEs) in various diabetic populations, including patients with atherothrombosis, myocardial infarction, and heart failure, and patients who have undergone coronary interventions. [212] Large trials conducted by the UK Prospective Diabetes Study have validated the safety profile of metformin for its early use in diabetic patients, and demonstrated the overall cost effectiveness. [2,15,16] A recent trial also demonstrated that early metformin treatment improved the distribution of lipoprotein. [17] However, in addition to lifestyle modifications, it is unclear whether metformin montherapy is beneficial for patients with a new diagnosis of DM compared to those who do not need OHAs for glycemic control.
