Maybe it’s the friend’s child next door or a member in the family who has Type 1 diabetes. Perhaps you’ll be in charge of care at some point or are simply interested in learning more about T1. Consider this guide to help you navigate Type 1 as a friend or family member. What is Type 1? Type 1 diabetes is an autoimmune disease that affects a person’s pancreas. The pancreas is responsible for producing insulin, a hormone people need to get energy from food. Our pancreas, for reasons that have not been identified, does not produce any insulin. As a result, we need to inject or continually infuse insulin through a pump and carefully balance our insulin doses with eating and daily activities. We must also regularly monitor our blood-sugar levels. Type 1 is a non-stop and 24/7 balancing act that we must maneuver every day. There is no way to prevent Type 1 and there is no cure (currently!). How do you manage it? We get by with a little help from our friends! These include our glucose meter, insulin, needles, and monitors. The glucose meter is a device that measures blood sugar. We use a device that pricks our finger and we put the blood sample onto a test strip. From there, the test strip is read by the meter and gives us a number on the meter screen. We can get insulin into our bodies through multiple daily injections or an insulin pump. Injections are delivered to our bodies through insulin pens and needles. There are two types of insulin that we use. Fast-acting insulin gives our bodies insulin right away and is taken with meals or to correct a high blood sugar. Fast-acting insulin is used multiple times a day, depending on when you eat. The other is long-acting insulin, which is given once a day. I wrote this poem last night at 3 a.m. Seems like almost every night I am up at that time. Truth be told though I am not alone. Multitudes of others share a similar experience when caring for someone with this disease. If you know someone with Type 1 Diabetes and they are managing it so well know that there is a whole lot of hard work behind it. The first alarm goes off at 12 Time to make sure all is still well A blood sugar reading of 65. The World Health Organization recently shared a shocking statistic: 347 million people worldwide have diabetes. In the UK almost 5% of the population has diabetes and in the US almost 10% of the population has diabetes. These figures do not include the percent of the population that are prediabetic or don’t realize they have diabetes. Diabetes can have serious health complications including nerve, eye and kidney damage. The good news is that 93 percent of diabetes is caused by lifestyle choices – what we eat and how much we move. Which means that most diabetics can manage and even prevent diabetic damage through diet and exercise. People with prediabetes can stop it dead in it’s tracks and never develop the disease at all. Nothing about type 2 diabetes is inevitable if you can make positive changes. For over 10 years, I’ve been working with diabetes patients in the UK. I’ve seen first-hand what happens when diabetics lose weight and adopt a healthy diet. Many of my patients reduce their medications or are able to stop taking it all together. A lot of people ask me, “Tyler, how can I control diabetes naturally?” So I thought I’d share what I say to them here. During all these years that I have dedicated to helping people use food as medicine, one of the questions that I hear quite commonly comes from people with diabetes and their concerns about fasting. I have to admit that this is a tough one because there are people who have too much sugar (hyperglycemia), and there are also those who don’t have enough sugar (hypoglycemia). When it comes to fasting, I’ve actually facilitated Type I and many Type II diabetics through juice and water fasting. I somehow feel that this is one of those issues when people might want to have help from someone who is experienced in these conditions. Either way, the point is that if you’re a Type I or Type II diabetic, and you want to do a fast, it’s just a matter of monitoring where your blood sugars are at, and making sure to keep them within a comfortable level. Watch For Blood Sugar Spikes Typically, when Type I or Type II diabetics go on a fast, they’re drinking a lot of fresh-pressed juices, especially ones made from fruits, which might cause them to have a spike in their blood sugar immediately. What you’ll find, is after two or three days it doesn’t spike as much because there’s not as much fat in the circulatory system that’s causing that blood sugar to stay there. So, it comes down to drinking small amounts of juice through the entire day. Another crucial thing is to monitor and maintain balanced blood sugar levels! The main thing I noticed about juicing with Type 1 is that your body requires a lot less insulin... for example, prior to juicing my basal rate was 14 units of Lantus/day. Now I am taking 9 units. Be careful and make sure you scale back your basal insulin! In terms of carb to insulin ratios for your bolus dosing, I would not take the same amount of insulin that you do for whole foods. For example, if you usually take 1 unit of fast acting insulin to eat one whole apple, I would take 1 unit of insulin to juice about 5 apples. You are not ingesting all of the starch and solid carbs, only a small amount of glucose carbs, so you need a lot less insulin. The first doctors instructions to eat more fruit and drink more fruit juice were, in hindsight, both a blessing and a curse. The curse part is obvious. That doctor sent my blood sugar into the stratosphere, accelerating my downward spiral into diabetic ketoacidosis, a very dangerous condition where the body poisons itself. But I see it as a blessing for the exact same reason. Without his terrible medical advice, I probably wouldve kept going for a few more weeks without visiting another doctor or hospital. I wouldve been in bad shape, but not horrible shape, hovering somewhere above the danger zone without plunging straight into it. For the past few months, I have been REALLY struggling with my health. My bg numbers have been bouncing all over the place... between the 30's and upper 300's regularly, my digestion has been poor, my body has been sore, my weight has gone up, skin needs much improvement. You get the picture. I was needing a drastic change of pace and redirection. Against all forms of common sense, being that I am a type 1 diabetic, I ventured on a juice fast...that's right a JUICE fast. I am now on day 4 and I wanted to share with you my results because they are shocking to me and I thought it may be helpful to some of you as well if this is something you had been considering. Since I have started, I have had to REDUCE my basal rates daily. Today, my pump is set to deliver just 60% of my "regular" basal rate and my numbers are still on the low side. My bolus ratios have gone from approx. 1:8 (prior to juice fast) to 1:16. My blood sugars have been ranging between 50 and 130. I am still working on reducing the hypo's. The highest spike I have seen post "meal" has been 150, and that was on the first day. I am juicing mostly vegetables, and a little fruit (mainly apples, lemons, and blueberries to supplement the vegetables). I have lost 7 lbs so far. I am not sure how long I will do this; maybe 10-15 days. So far, it has been great to break the emotional and habitual attachment I have with food and has made my body feel great...no soreness, no stomach problems. My body is thankful.
