Joslin Diabetes Center DO-IT program: a review from a participant's point of view I began my life as a type 1 diabetic as a patient at the Joslin Clinic in Boston in 1966, and according to the program then followed, all patients were required to return every five years for a one- or two-week stay at the clinic. When I was first there, the technology for convenient blood sugar tests did not yet exist, so we all had to line up in the tiny kitchen several times a day for our urine sugars to be determined by being boiled in a test tube mounted on a rack over the stove burners. These measurements were totally inaccurate, since they only showed the total urine sugar excreted over the previous several hours and varied according to the renal threshold of each patient, but the clinic was very strict in insisting that they were absolutely necessary, though today this is known to be ridiculous. I remember one old woman standing next to another at our early morning urine test at 6 AM, and when one obtained a test result of 4+, the highest possible, the other went into a fit, shaking her head rapidly and mechanically, shouting "I don't like it! I don't like it," over and over again, like a robot gone berzerk. One day, when a new patient, a Black man, came into the urine sugar testing kitchen, we all fell silent, thinking the same thought, of what a tragedy it was to be Black in that primitive era of racial discrimination and also to be diabetic. The walls of the clinic were lined with bottles containing a blue fluid used for the urine sugar tests, which were labeled 'Benedict's Solution: Poison.' [email protected] To describe the use of telemedicine for teaching group diabetes education classes to individuals with diabetes mellitus in a rural medically underserved area. Adults with diabetes from a rural area served by Oswego Hospital in upstate New York were asked to participate in this study. Volunteers received diabetes education through real-time teleconferencing (n=27) by joining age- and sex-matched patients from the Joslin Diabetes Center, Syracuse, NY, in our "Living with Diabetes Class" (n=39). The two 3-h sessions offered comprehensive diabetes education by a diabetes nurse educator, dietitian, and exercise physiologist. These sessions were followed in 3 months by a 3-h follow-up class. Each group receiving tele-education consisted of two or three patients with diabetes. The hemoglobin A1c test (a blood test that estimates the overall average glucose levels over the past 3 months) improved in the face-to-face and the telemedicine groups. There was no significant change in weight between groups. Each group had significant improvements in scores on the Problem Areas In Diabetes survey, which is a measure of emotional functioning in diabetes. Diabetes treatment satisfaction as measured in the Diabetes Treatment Satisfaction Questionnaire improved in the face-to-face group but not in the telemedicine group. The award ceremony, set for tomorrow, Thursday, Sept. 23, at 2 p.m. at The Nottingham Senior Living facility in Jamesville, NY, will honor Robert Cleveland, who has lived for more than 78 years with type 1 diabetes, and his brother Gerald Cleveland, who has lived for 72 years with the disease. "Robert Cleveland and Gerald Cleveland have led remarkable lives despite the challenges that diabetes poses," said George L. King, M.D., director of research at Joslin Diabetes Center in Boston. "To put their accomplishments into perspective, the Cleveland brothers developed diabetes shortly after the discovery of insulin, and more than 70 years later neither has developed any serious complications.
