Metformin is key in the management of type 2 diabetes mellitus but comes with an additional financial burden, particularly with the use of brand-name products. The availability of generics allows for generics substitution reducing the cost of treatment. However, only generics that offer similar bioavailability with the originator should be considered. The authors therefore carried out a study to assess the bioequivalence of generic metformin tablets within Nigeria. Metformin tablets selected from the Nigerian market were appraised for quality following British and United States Pharmacopoeia guidelines. An in vivo bioequivalence study in healthy volunteers was carried out for generics and the originator brand-name metformin in an open-label, 2-arm, 2-treatment crossover fashion with a 1-week washout period. Blood samples were collected 0, 0.5, 1, 1.5, 2, 3, 4, 6, 8, 10, and 24 hours post-dose. Plasma concentrations of metformin were analyzed using a validated high-performance liquid chromatography(HPLC) method and pharmacokinetic parameters were obtained using a non-compartmental approach. The bioequivalence of the formulations was assessed based on guidelines from the US Food and Drug Administration, Centre for Drug Evaluation. Despite being made by dozens of manufacturers around the world, to date there have been only 15 recalls of the popular drug, with most being minor. I have been taking metformin (Glucophage) for about three years. My doctor recommended it since my A1c reading had been climbing from about 5.5 to 6.3% over a two-year period. There is also a history of Type 2 diabetes on the paternal side of my family. Metformin caused me some stomach upset so I switched to Glumetza, a coated form of the same drug, Glucophage, a standard treatment for diabetes that has been available in the United States since 1995. I have excellent insurance coverage and receive all my medications in the most economical form, three month prescriptions delivered by mail. On February 6th, Caremark mail service called to tell me there was a prescription coming for which I would have to sign. I told the caller that it was fine to leave it at the door. The Caremark representative said that was not possible since it was a high-value prescription. The prescription came the next day, I signed for it, and I checked the invoice only because the person on the phone had told me it was a “high-value prescription” and I knew it was only metformin. When I looked at the bottom of the invoice I noticed that the listed price was $14,020.87. My co-pay was $40.00 I called Caremark to tell them about the computer error. I didn’t think it was possible that metformin cost $14,020.87. I was even more certain that this was a computer error because the prescription was not Glumetza, but a generic, metformin ER. Dr. Bernstein has been preaching about this on his web telecasts for years, but it bears repeating: If you are having problems with generic metformin or not seeing it make much impact on your blood sugar, change brands before you assume it isn't working or that you can't tolerate it. I just had this message brought home to me when my pharmacy (Walgreens) filled my prescription for metformin ER with tablets from SunPharma instead of the ones from Teva they'd given me for years. The pills were about half the size of the ones I'd been getting, which suggested they contained less of a matrix substance to slow the release of the metformin. And sure enough, when I took the same dose I had been taking with no problems with the Teva brand metformin, I felt exhausted and semi-poisoned. It felt just like when I had taken an overdose of metformin some years ago, when my family doctor prescribed an overdose after confusing the dosage instructions for regular metformin--which can be taken in larger doses--with those of metformin ER. Not only that, but my fasting blood sugars went up. Clearly the SunPharma metformin ER was not behaving like a true extended release should and releasing slowly through a 24 hour period but was hitting my blood stream all at once and then was done. A quick visit to Google revealed that Sun Pharmaceuticals is an Indian company and that in the past the FDA has forced them to recall batches for quality issues. Would like to make a decision regarding which generic brand of Metformin ER to buy. You have years of experience and knowledge. Have been pre-diabetic since my 20's. Started watching carb count in my early 50's and have kept myself from going full on diabetic. My age is now tracked on the geologic time scale. My GP and naturopath both want to see my fasting blood sugar lower, currently 115-120 upon waking, and both doctors keep pestering me about it. Keep my carbs around 40-50 gm./day. Front load my carbs and meat/veggies dinner. Tried eating protein snack before bed. Can't bring the fasting blood sugar any lower and both doctors have given me prescriptions for Metformin ER. Still sitting on them. Wish to avoid as much stomach upset as possible. Glucophage XL highly recommended but not covered by my insurance and way to spendy out of pocket. Any other way to purchase? Been doing research on generic brands and the Amneal from Walmart looks like a no-go. Teva no-go. Sun no-go. What brands work well and spare one from cramping and so forth. My online research predominates with the Extended Release as it makes Metformin easier on the stomach. That true? Was thinking of starting with 500 mg.
