Abstract There are several claims about the beneficial effects of supplementing L-glutamine to both type 1 and type 2 diabetes. The purpose of the present study was to provide detailed knowledge about the fate of this amino acid in the liver, the first organ that receives the compound when ingested orally. The study was done using the isolated perfused rat liver, an experimental system that preserves the microcirculation of the organ and that allows to measured several parameters during steady-state and pre steady-state conditions. L-Glutamine was infused in the portal vein (5 mM) and several parameters were monitored. Livers from type 1 diabetic rats showed an accelerated response to L-glutamine infusion. In consequence of this accelerated response livers from type 1 diabetic rats presented higher rates of ammonia, urea, glucose and lactate output during the first 25–30 minutes following L-glutamine infusion. As steady-state conditions approached, however, the difference between type 1 diabetes and control livers tended to disappear. Measurement of the glycogen content over a period of 100 minutes revealed that, excepting the initial phase of the L-glutamine infusion, the increased glucose output in livers from type 1 diabetic rats was mainly due to accelerated glycogenolysis. Livers from type 2 diabetic rats behaved similarly to control livers with no accelerated glucose output but with increased L-alanine production. Ten adolescents (15.2 ± 1.4 years [SD], A1C 6.9 ± 0.9%) on insulin pumps were studied. The subjects were randomized to receive a glutamine or placebo drink pre-exercise and at bedtime (0.25 g/kg/dose). A 3:00 p.m. exercise session consisted of four 15-min treadmill/5-min rest cycles. Pre-exercise blood glucose was 140–150 mg/dl and was monitored throughout the night. Studies were randomized crossover over 3 weeks. Blood glucose levels dropped comparably (52%) during exercise on both days. However, the overnight number of hypoglycemic events was higher on glutamine than placebo (≤70 mg/dl, P = 0.03 and ≤60, P = 0.05). In adolescents with type 1 diabetes with no residual insulin secretion, oral glutamine supplementation increased the incidence of postexercise, nocturnal hypoglycemia compared with a calorie- and nitrogen-free placebo. Insulin sensitivity, as measured using a two-dose hyperinsulinemic-euglycemic clamp clamp the following morning, did not differ between oral glutamine and placebo. Whether long-term oral glutamine would have a role as an oral supplement to affect glucose control remains to be explored. The decline in insulin sensitivity (SI) associated with puberty increases the difficulty of achieving glycemic control in adolescents with type 1 diabetes (T1D). The aim of this study was to determine whether glutamine supplementation affects blood glucose by enhancing SI in adolescents with T1D. Thirteen adolescents with T1D (HbA1C 8.20.1%) were admitted to perform afternoon exercise (four 15-min treadmill/5-min rest cycles of exercise) on two occasions within a 4-wk period. They were randomized to receive a drink containing either glutamine (0.25 g/kg) or placebo before exercise, at bedtime, and early morning in a double-blind, crossover design. I get a bulk container with powdered creatine monohydrate. I wanted to take glutamine to help with recovery. I am starting to train for a triathlon. I have shied away from creatine because there is a chance of damaging your kidneys and liver. I know there is no real hard proof of that, it was just a precaution. A six week double-blinded, placebo controlled trial was conducted in 66 type 2 diabetic subjects randomized to receive oral glutamine supplementations or a placebo. There were 53 patients who completed the trial with 27 in the glutamine group and 26 in the placebo. Each patient took 30g/d of the amino acid three times a day with each main meal. At the end of the study, there were significant differences between body mass, waist circumference, blood glucose levels, and average HbA1c levels between the two groups. A difference in body fat mass (+0.05 kg with glutamine vs +1.05 kg with placebo group, p=0.01) and % body fat (+0.01% vs +1.36%, p=0.008) was observed between the two groups. Moreover, a significant reduction was seen in waist circumference (-1.33 vs +2.34 cm, p<0.001). Fasting blood glucose levels significantly decreased in the glutamine group compared to the placebo (-0.79 vs -0.06 mmol/L, p=0.04) as well as the mean HbA1c (p=0.04). In addition, there were significant differences in systolic blood pressure favoring glutamine over placebo (p=0.005). Glutamine had no effects on cholesterol, HDL, LDL, triglycerides, and C-reactive proteins.
