Key facts The number of people with diabetes has risen from 108 million in 1980 to 422 million in 2014 (1). The global prevalence of diabetes* among adults over 18 years of age has risen from 4.7% in 1980 to 8.5% in 2014 (1). Diabetes prevalence has been rising more rapidly in middle- and low-income countries. Diabetes is a major cause of blindness, kidney failure, heart attacks, stroke and lower limb amputation. In 2015, an estimated 1.6 million deaths were directly caused by diabetes. Another 2.2 million deaths were attributable to high blood glucose in 2012**. Almost half of all deaths attributable to high blood glucose occur before the age of 70 years. WHO projects that diabetes will be the seventh leading cause of death in 2030 (1). Healthy diet, regular physical activity, maintaining a normal body weight and avoiding tobacco use are ways to prevent or delay the onset of type 2 diabetes. Diabetes can be treated and its consequences avoided or delayed with diet, physical activity, medication and regular screening and treatment for complications. What is diabetes? Diabetes is a chronic disease that occurs either when the pancreas does not produce enough insulin or when the body cannot effectively use the insulin it produces. Insulin is a hormone that regulates blood sugar. Hyperglycaemia, or raised blood sugar, is a common effect of uncontrolled diabetes and over time leads to serious damage to many of the body's systems, especially the nerves and blood vessels. For years, health experts have bemoaned the rise of childhood obesity in the United States. About 17 percent of kids and teens in the U.S. are now considered obese, a figure that has more than tripled since the 1970s, according to data from the Centers for Disease Control and Prevention. A report in the New England Journal of Medicine lays out one of the consequences of all this excess weight: a corresponding increase in childhood cases of Type 2 diabetes. Type 2 diabetes occurs when extra body fat makes it hard for cells to use insulin, a hormone that turns sugar into energy. Over time, blood sugar levels rise and cause blood vessels to become stiff, increasing the risk of life-threatening conditions like heart attacks, strokes and kidney failure, among others. More than 75,000 Americans die of diabetes each year, the CDC says. Type 2 diabetes used to be called adult-onset diabetes, because it would take years to develop. (That's in contrast to Type 1 diabetes, formerly known as juvenile diabetes, which occurs when the immune system destroys the cells that make insulin.) Diabetes is not a disease of blood sugar. It is a disease of insulin and, perhaps more importantly, leptin signaling. Until that truth is accepted, we will continue to see epidemic growth in type 2 diabetes and obesity, growth that underscores the inadequacy of current conventional medical treatment and the falsity of prevailing nutritional advice. Elevated blood sugar is merely the symptom of diabetes, not its cause. The disease itself is one of metabolic miscommunication. Symptoms are manifestations of the way our body deals with a disease. Tweet Autoimmune disease refers to illness or disorder that occurs when healthy tissue (cells) get destroyed by the body's own immune system. The term autoimmune disease is one that many people with diabetes - in particular, those with type 1 diabetes - will have come across or be familiar with. In the case of type 1 diabetes, the disease-fighting system mistakes healthy cells in the pancreas for foreign, harmful invaders and attacks them, leaving the body unable to produce its own insulin and keep levels of blood glucose under control. There are more than 80 different types of autoimmune disease, from multiple sclerosis and type 1 diabetes to coeliac disease and rheumatoid arthritis. The immune system is the body's protection against harmful substances such as bacteria, viruses and toxins, all of which contain harmful antigens. To counter this, the immune system produces and sends antibodies (special proteins) to identify destroy these antigens. However, in some cases the immune system cannot distinguish between healthy, harmless tissue and antigens and, as a result, it attacks and destroys normal tissue (in people with diabetes, the cells mistakenly targeted are the insulin-producing beta cells in the pancreas). Diabetic patients are more likely to develop periodontal disease, which in turn can increase blood sugar and diabetic complications. People with diabetes are more likely to have periodontal disease than people without diabetes, probably because people with diabetes are more susceptible to contracting infections. In fact, periodontal disease is often considered a complication of diabetes. Those people who don't have their diabetes under control are especially at risk. No Such Thing As Type 2 Diabetes? Why Old Notions Of 'disease' Need Revamping
As medicine advances, thanks to expanding research and sophisticating technologies, it’s always got the task of shedding its past and redefining itself. Part of this is to rid itself of conventions that are determined to be useless, or worse, erroneous. In a smart new editorial in The Lancet, researcher Edwin Gale of the University of Bristol argues that there’s a major problem with the way we think about disease today, in particular the ones that are not so clear cut, like type 2 diabetes, which itself affects millions across the globe, and now includes a growing new subset: children. But slapping the “type 2 diabetes” label on a wide constellation of symptoms (and, perhaps, causes) that has no simple treatment is a major “category error,” argues Gale. And this error can lead us – researchers and patients alike – down the wrong path when it comes to solutions. Here’s Gale’s central argument. He says that medicine, unfortunately, still has the tendency to look for a unique cause and/or symptom to define a “disease.” As it happened in the case of diabetes, over a century ago, a researcher decided that blood sugar was the way to define the disease, and this convention has, very sadly, stuck. Even today, committees of experts sitting around a table deciding what defines normal and abnormal blood sugar is considered an acceptable method. History A History Of Diabetes Mellitus Or How A Disease Of The Kidneys Evolved Into A Kidney Disease
Diabetes mellitus has a long history during which it was considered to be a disease of the kidneys well into the middle of the 19th century. Recognized in antiquity from its excessive urine output and described as a disease of the urinary tract, its clinical features and fatal outcome were quite accurately recorded by the 1st century ad. Galen (129–200) described it as a disease specific to the kidneys because of a weakness in their retentive faculties. The sweet taste of diabetic urine, which is described in ancient Indian texts and noted by Avicenna (980–1037) and Morgagni (1635–1683), was attributed to the passage of absorbed water and nutrients unchanged into the urine. In 1674, Thomas Willis (1621–1675) first differentiated diabetes from other causes of polyuria by the sweet taste (quasi melle) of diabetic urine and suggested that the sweetness first appears in the blood. A century later, Matthew Dobson (1732–1784) showed that the urine sweetness was because of sugar and was preceded and accompanied by sugar in the blood. Although diabetes then came to be ascribed to increased sugar in the blood, the presence of sugar in the urine continued to be attributed to the decreased retentive properties of the kidneys. Mario Alberto Maciel Tinajero looks like a fairly healthy 68-year-old. He has a few extra pounds on his chest but he's relatively fit. Yet he's suffered for the last 20 years from what he calls a "terrible" condition: diabetes. "I've never gotten used to this disease," he says. Maciel runs a stall in the Lagunilla market in downtown Mexico City. This market is famous for its custom-made quinceañera dresses and hand-tailored suits. Diabetes has come to dominate Maciel's life. It claimed the life of his mother. He has to take pills and injections every day to keep it under control. And because of the disease he's supposed to eat a diet heavy in vegetables that he views as inconvenient and bland. "Imagine not being able to eat a carnitas taco!" he says with indignation. His doctors have told him to stop eating the steaming hot street food that's for sale all around the market — tacos, tamales, quesadillas, fat sandwiches called tortas. His eyes light up when talks about the roast pork taquitos and simmering beef barbacoa that he's supposed to stay away from. "A person who has to work 8 or 10 hours has to eat what's at hand, what's available," he says. "It's difficult to follow a diabetic diet. The truth is it's very difficult."
