Welcome To The African Journal Of Diabetes Medicine. Many Of You Will Have Known The Title Under Its Former Name Of Diabetes International.

We took the decision to change the name at the beginning of 2010 in order to better reflect our area of focus, and in tandem with the move to put the journal on the web on an open access basis. The incidence of diabetes in Africa continues to increase along with the other core non communicable diseases such as cardiovascular disease and hypertension. WHO and the IDF estimate that there are currently some nine million diabetes sufferers in Africa and that this figure will double in the next 30 years. As such diabetes represents a significant percentage of Africa's disease burden. Type 2 diabetes, the most common form, and is becoming more prevalent owing to rising rates of obesity, physical inactivity and urbanisation. Type 1 diabetes exists in two major forms in the region: type 1A or autoimmune and type 1B or ketosis-prone type 2 diabetes. At present there are scanty epidemiological data on either. The current morbidity of diabetes is primarily due to the high rates of microvascular complications, while macrovascular complications, once rare, are becoming more common, particularly in the urban setting. The disease burden needs to be addressed. Research in Africa needs to be encouraged, especially by Africans. AJDM can hopefully act as a platform for publication of this. While our primary aim is to assist the practitioner in his/her care of such patients, and to afford access to information that may allow the prevention of such complications, it is the Editors' wish to function as a forum for that information which, while still experimental, may shed light upon current thinking of those active in the fields appropriate to the aims of Journal of Diabetes and its Complications. In addition to general articles on clinical aspects of diabetes mellitus, Journal of Diabetes and its Complications also presents articles on basic research in all areas of diabetes and its related syndromes. Topics covered relevant to the diabetic patient will include diagnosis, pathogenesis, and clinical management of the following: diabetic retinopathy, neuropathy and nephropathy; peripheral vascular disease and coronary heart disease; gastrointestinal disorders, renal failure and impotence; and hypertension and hyperlipidemia. Journal of Diabetes and its Complications will also publish papers on the general pathogenesis and prevention of diabetes. Unfortunately, the risk of chemotherapy toxicities, such as mucositis, myelosuppression, neuropathy, and cardiomyopathy, increases with advancing age. It has been estimated that frailty in elderly cancer patients can be as high as 42%.This has led to the practice of empiric chemotherapy dose reductions (ECDR) in elderly patients to prevent morbidity and mortality.A multisite cohort study involving elderly cancer patients demonstrates that 29% of those with advanced cancer have experienced a dose reduction in their first chemotherapy cycle. There are existing guidelines on chemotherapy dosage for patients with NSCLC. The present study was conducted to ascertain how cigarette smoke affects the exocrineendocrine interactions of the human pancreas with diabetes mellitus secondary to pancreatic diseases (type 3c). Blood has been collected from smoking and non-smoking healthy individuals as well as from patients with diagnosed chronic pancreatitis and diabetes type 3c. The concentrations of interleukin-6, endothelin-1 and insulin in the plasma were determined by enzyme-linked immunosorbent assay (ELISA) tests. The activities of amylase and lipase in the serum, as well as the lipid profile, creatinine, uric acid and urea concentrations, were measured using colorimetric methods. Samples of normal pancreatic tissue and chronic pancreatitis were verified histopathologically and then interleukin-6, endothelin-1, insulin and glucagon were localized by immunohistochemical staining using a monoclonal anti-human antibody. The highest levels of interleukin-6 and endothelin-1 and the lowest levels of insulin and glucagon intensity from the immunostaining were observed in smoking patients with diabetes. In all smoking patients with pancreatitis and diabetes, there was a significant elevation in interleukin-6 and endothelin-1 concentration and amylase and lipase activities, hyperlipidaemia and a lower value of estimated glomerular filtration rate and blood urea nitrogen when compared to non-smokers. Our study confirmed that smoking exerts a pro-inflammatory effect and disturbs the exocrineendocrine interactions of the pancreas.
