Humulin R is the brand name for Eli Lilly's recombinant human regular insulin. The usual version of Humulin R is U-100 and contains 100 units of insulin activity per 1 milliliter of fluid. But there's also a rarely-used version called U-500. The U-500 insulin is 5 times more potent than U-100 insulin. U-500 insulin contains 500 units of insulin activity per 1 milliliter of fluid. People may need to use U-500 if their diabetes is not well-controlled with U-100. The U-500 version should be used only for patients requiring doses above 200 units a day. Important information about U-500 insulin Since the use of U-500 insulin is not as common as the use of U-100 insulin, some health professionals may not be aware of it. This by itself increases the chance of dispensing errors. The main problem with U-500 insulin is the risk of a mix-up between the insulin U-100 and insulin U-500 versions. If this occurs, it can result in very dangerous low blood sugar or very high blood sugar. A mix-up between the two insulin concentrations may occur if doctors or pharmacists select the wrong concentration (for example, selecting the U-500 regular insulin from computer screen instead of U-100). Sometimes the two concentrations appear one line apart on the computer screen, which sets up the possibility of selection errors. I use humulin r u 500 vial 20ml . When I went to a restaurant with my family i went to the restroom so I can take my insulin shot the vial broke and I dont have a extra one. So I try to get a refill the pharmacy said it was too soon. If I wanted one I will have to paid $1,471.00 for one vial. I have the money for this insulin and I have medicare part A and Breakfast and part Chicago and part D and I have extra help to paid for my medications. So what I did I called the company it made sense humulin r u 500 on Friday. I will find out tomorrow on what they are doing to do? @Marie17 : we broke a vial once and called our insurance right away - they allowed the purchase of an extra vial because of accidental damage. So my first suggestion is to call the insurance company first thing tomorrow morning to see if they will allow a refill due to accidental damage. I am not familiar with Medicare etc. so I am not sure who to call. Please dont spend all that money without first trying to talk to the insurance. I hope someone can help with more specific information. I would start calling first thing in the morning. I would also call my doctors office to ask what they can do. Possibly they can give you a prescription for a slightly different insulin that would still work for you but that your insurance will accept to pay for. Another path, but much more expensive: I believe that Humulin R may be the same formula as ReliOn Regular, sold at Walmart for much cheaper, and without the need for a prescription, although NOT in concentrated form. U-500 insulin is five times more potent than standard U-100 and is utilized in patients requiring high doses of insulin. Prescribing of U-500 is on the rise due to the obesity epidemic, adoption of tighter glucose control protocols, increased insulin resistance, and utilization of insulin pumps.1 Potential for adverse drug events exists due to unfamiliarity with U-500 and its higher potency. This review is intended to enable safe provision of U-500 insulin by increasing awareness of the drug and its correct administration. As insulin resistance worsens, greater doses of insulin are required to meet glycemic goals. This is especially true for persons with insulin resistance (IR) syndromes. These are patients with an insulin requirement of 200 U/day and greater. FIGURE 1 illustrates the body’s response to insulin as the dose increases. Note that for patients with IR syndromes, the dose response to insulin is significantly diminished. This is especially true when insulin doses exceed 100 U. However, this phenomenon does not mean greater doses are without effect.2 In fact, higher doses of insulin eventually achieve therapeutic targets. For this reason, use of high-dose insulin should not be avoided in insulin-resistant patients. Otherwise, these patients will fail to achieve glycemic goals by underdosing insulin. What is the most important information I should know about Humulin R U-500? Do not share your Humulin R U-500 KwikPen or U-500 insulin syringes with other people, even if the needle has been changed. You may give other people a serious infection or get a serious infection from them. Do not take Humulin R U-500 if your blood sugar is too low (hypoglycemia) or you are allergic to any of the ingredients in Humulin R U-500. Tell your healthcare provider: If you have liver, kidney, or heart problems or any other medical conditions. If you are pregnant, plan to become pregnant, or are breast-feeding. About all the medicines you take, including prescription (especially ones commonly called TZDs [thiazolidinediones]) and over the counter medicines, vitamins, and herbal supplements. Calculating Concentrated Insulins

Clements, PharmD, BCPS, CDE, BCACP | Sep 06, 2016 ecently, representatives from Eli Lilly came to the school of pharmacy to educate faculty and fourth-year professional students on concentrated insulin, such as Humalog (insulin lispro U-200) and Humulin R (insulin regular U-500). While I was listening to the speaker talk about the devices, I reflected on how much diabetes has changed over the years. When I was in pharmacy school studying diabetes in the fall of 2004, the faculty member taught us about metformin, sulfonylureas, acarbose, rosiglitazone, and pioglitazone. She also mentioned insulins that were available such as NPH, regular insulin and others. At this time, I never had to calculate a U-500 dose. As a faculty member for the past 10 years, I would speak to fourth-year professional students on my ambulatory care learning experience about concentrated insulins in order to make them aware about the potential opportunity of dispensing and/or counseling on these medications when they become an independent pharmacist. With the new concentrated insulin pens, it is essential to educate students about these options because as it may be that the use of concentrated insulin increases over the next couple of years. It is important to remember that there are other concentrated insulins on the market, such as Toujeo (insulin glargine U-300) and Tresiba (insulin degludec U-200). Keep in mind that everyone may have different approaches depending on the experience and comfort level with U-500 insulin. Concentrated insulin is a man-made form of a hormone that is produced in the body. It works by lowering levels of glucose (sugar) in the blood. Concentrated insulin (U-500) is a long-acting form of insulin that is different from other forms that are made from animal insulin. Concentrated insulin is used to treat type 1 (insulin-dependent) diabetes in people with significant daily insulin needs (more than 200 units per day). Concentrated insulin may also be used for purposes other than those listed in this medication guide. Humulin U-500: Counseling Your Patients
Pharmacists looking to counsel patients on Humulin R U-500 can use counseling points that are very similar to U-100, which patients often find comforting. To begin with the product, which is a concentrated insulin that is typically considered in patients injecting large volumes of U-100, should be clear and colorless at all times. They should be administering the insulin 30 minutes before their meal(s). The most common adverse effect reported with Humulin R U-500 is hypoglycemia. Studies show that patients are at risk for severe hypoglycemia as long as 18-24 hours post injection and should always be considered a risk factor if the patient is found unresponsive. For this reason patients should also be cautious while driving/operating heavy machinery at all times, but especially when first starting this medication as well as any dose changes. To minimize the risk of hypoglycemia, Humulin R U-500 should not be administered IV, IM, or via insulin pump in a hospital setting or outpatient. Other common adverse effects would include lipodystrophy, weight gain, edema, allergic reactions, and/or immunogenicity (antibody formulation). Considered a Pregnancy Category B, this product, like other insulin products should be used in pregnant women with both caution and care but it is noteworthy that Humulin R U-500 does not pose any greater risk. The insulin does pass into the breast milk and gets ingested by the infant, however, the insulin is broken down into the gastrointestinal tract and does not raise any concern, therefore, it is safe to administer while breastfeeding. Using U-500 Insulin
What is it? Humulin R U-500 is a kind of insulin that is much stronger than the more common U-100 insulin. It is used by people with diabetes who need large doses of insulin, usually because they are “insulin resistant.” That means their bodies can't use insulin well, so they need large doses to keep their blood glucose (sugar) under control. How is it used? U-500 is 5 times more concentrated than U-100 insulin. This means that every 1 unit of U-500 is the same as 5 units of your usual insulin. This makes it a more powerful medicine. It also means that you need to be careful about giving yourself the right amount of U-500. If you are using a regular insulin syringe like the kind used for U-100 insulin, the markings each correspond to one-fifth of a U-500 unit. So, if you are taking 100 units of U-500 in an insulin syringe, you would draw insulin from the vial up to the 20 mark (100 ÷ 5 = 20). Your health care provider might want you to use a different kind of syringe, called a “tuberculin syringe.” This kind of syringe is marked in milliliters (ml). If you are taking 100 units of U-500 in a tuberculin syringe, you would draw insulin from the vial up to the 0.2-ml mark. (See chart at left.)
