Amber Taylor, MD, director of The Diabetes Center at Mercy Medical Center in Baltimore, wrote Smith a prescription for Humulin U-500 insulin, a much more concentrated form of the drug. It could deliver far more active ingredient at far less overall volume, which was important since Smith's insulin doses were getting high. But when Smith showed up at the pharmacy, she was told she'd have to shell out $900 for a vial. "There was no way I could afford that," Smith told MedPage Today as she recalled the pharmacy visit, which happened about a year ago. And the price has gone up since then: Now a single bottle of highly concentrated Humulin U-500 insulin that lasts a diabetes patient about one month costs $1,200 wholesale -- more than five times the $220 it cost in 2007. Those increases have far surpassed those for the less concentrated Humulin U-100, which only rose three times during that period, at a rate comparable to the rest of the insulin market, a MedPage Today analysis found. Although less than 5% of all diabetes patients are taking U-500, Taylor and other clinicians are wondering why an old insulin that had been on the market for many years and had remained relatively cheap suddenly became a valued commodity. While discounts and rebates paid by manufacturers make insulin affordable for most people, they don't directly help the uninsured or people in the deductible phase of their high-deductible plans. And while some high-deductible plans exempt insulin from the deductible phase, others require people to pay most or all of the retail price until the deductible is met - meaning these people don't fully benefit from rebates when they visit the pharmacy. By using the Blink Health platforms, people who pay full price for most Lilly insulins may save 40 percent. This discount program will be the first time branded medicines will be discounted via the Blink Health platform, which has previously only been used for generic prescription drugs. "We understand the burden people face when paying full price for insulin," said Mike Mason, vice president, Lilly Diabetes. Gee, when I took NPH (for over 20 freakin' years), the curve was anything but flat. If it were flat, I wouldn't have had lows at about 1:30pm if I didn't eat lunch by then. If it were flat, I wouldn't have needed bedtime snacks to prevent overnite lows. Perhaps in your case it's working, but my experience, and the experience of many members here who lived through the days when NPH was the best available insulin, tells a different story. I'd just suggest you keep an open mind regarding NPH, as it may help you tame some of your other issues. Did you take the same dosage of NPH every 8 hours on the nose? I take 20 units every 8 hours right on the clock. As you can see in the photo in my previous post, I have the NPH curves copy and pasted for every 8 hours. I then summed the two overlapping curves together with my calipers and made a new curve above which I highlighted. As you can see it is relatively flat. If you look up NPH 3 times per day, you'll see it is often used a substitute to basal insulin. In fact I fear changing to levemir or lantus because those basal insulins aren't flat for 24 hours.
