Monitoring blood glucose at home is second nature to many people with diabetes. In fact, it’s often so automatic that you may not stop to think about all the steps in the process that may interfere with achieving an accurate blood glucose reading. First, there’s the near-automatic loss of accuracy that comes from taking a blood sample from capillaries, which is what happens with both fingertip and alternate-site testing. The blood glucose level in your capillaries is delayed compared to the blood glucose level in your veins, which is considered your “true” blood glucose level and is what lab tests measure. The two levels may differ by as much as 10% to 15%, depending on how quickly your blood glucose level is changing. If you’ve been fasting and your blood glucose level is stable, the two probably won’t differ by very much. Second, your blood may become tainted when it reaches the surface of your skin, especially if you’ve been handling food prior to lancing your finger. And third, the test strip or meter you use may deliver inconsistent or inaccurate results. This last possibility is one that people with diabetes often forget about. After all, meters give the impression that they can pinpoint the exact glucose level of blood samples; they don’t display a range of possibilities. But most new meters are only guaranteed to be accurate within a 20% range. This means that if your actual blood glucose level is 100 mg/dL, the meter might display a number anywhere between 80 and 120. And malfunctioning meters can, of course, be even less accurate. Glucometers: Our Testing Process Initially, we intended to test the accuracy of each glucometer because an accurate blood-glucose reading is the most important part of managing your diabetes. However, we quickly learned that testing on real blood was unrealistic because of lab costs and small sample sizes. We'd need to send blood to a lab to compare actual glucose levels with the readings of the same blood sample by a glucometer. And we'd need hundreds, if not thousands, of readings to provide reliable data. To get around this hurdle, we tested the accuracy on a control solution— a liquid with a known and stated glucose level— provided by the meter manufacturer to calibrate your glucometer when it seems like it's not giving a correct reading. When you perform a test with a control solution, you can determine if your glucometer is providing accurate results if they match that known glucose level. However, while testing, we learned that many of the control solutions use a proprietary mixture. This means that some glucometers work only with their assigned control solution. In other words, this method also failed to provide adequate data because we could not test and compare the results head-to-head. Although we were unable to test the accuracy of the meters, glucometers must meet FDA regulation standards before they hit the market. The regulations state that each device must be accurate within 15 percent of the actual blood sugar levels, 95 percent of the time. While these numbers seem fairly accurate, the margin of error means it's important to have a backup glucometer. One question we hear often from people is, "Why do I need control solution? That is outdated technology." On the contrary, most meters on the market today do either come with or recommend control solution, including 11 other meters we purchased to compare against our meter for accuracy. And, there is a really good reason why it is still around. Control solution is a glucose solution that manufacturers produce with a set amount of glucose. They calibrate the control solution with the meter so you can only use control solution with its own brand of meter, just like test strips. To be clear, the control solution does NOT calibrate the meter. It checks the accuracy of the test strips for the meter. There are usually 2-3 different levels of control solution. Each has a different concentration of glucose, allowing the user to check for test accuracy at low, normal and high glucose levels for any given batch of test strips. All test strips have a plastic or paper base with very thin metal or carbon overlay and an enzyme. The blood or solution runs up the strip and mixes with the enzyme. The meter passes an electric charge through it and the resistance is what determines your blood sugar. The enzyme on the strip is very sensitive to heat, cold, moisture and direct sunlight.Performing a control test and comparing your result to the range on the side of a newly opened container of test strips tells you whether the batch will work correctly.
