How glimepiride + metformin + voglibose works This is a combination of three antidiabetic medicines: Glimepiride, metformin and voglibose. They work by different mechanisms to provide better control of blood sugar when single or dual therapy is not effective. Glimepiride is a sulfonylurea which works by increasing the amount of insulin released by the pancreas to lower the blood glucose. Metformin is a biguanide which lowers glucose production in the liver, delays glucose absorption from intestines and increases the body's sensitivity to insulin. Addition Of Voglibose To Glimepiride And Metformin Have Betterglucose Control In Diabetics: A Prospective, Parallel-groupand Open-label Comparative Study

Addition of Voglibose to Glimepiride and Metformin have BetterGlucose Control in Diabetics: A Prospective, Parallel-groupand Open-label Comparative Study This study compares the change in BMI, fasting and postprandial blood glucose , glycosylated haemoglobin (HbA1c), Glomerular Filtration Rate (GFR), serum creatinine, blood urea and lipid profile levels when two different therapies (group A = metformin+glimepiride) and group B = metformin+glimepiride+voglibose) are given to diabetic patients to compare efficacy of voglibose as an add-on therapy. This study was a 10 months prospective, open-label comparative study. Type II diabetic subjects, aged >18 years were selected and were divided into group A and B. All the parameters were evaluated before the treatment and reassessed after 3 months of treatment. Group A and B was having significant decrease in BMI with p<0.017 and p<0.049, respectively. In both of the groups glucose triad levels decreased significantly as p value found less than 0.05. There was no effect was found over blood urea level. Protective function of kidney observed when voglibose added. Evidence of controlling lipid profile also observed in triple drug therapy as p-value was <0.05. The add-on therapy using voglibose in dual therapy including glimepiride and metformin showed a very significant benefit in controlling the glucose triad levels (HbA1C, fasting plasma glucose and postprandial glucose level) when compared to dual therapy. Voglibose has an effect to decrease Total Cholesterol (TC), triglycerides (TGs) and Low Density Lipoproteins (LDL) level and increase HDL significantly. There was no effect was found on blood urea level. Distribution according to duration of diabetes Group A was having 12 patients of duration of diabetes between (0-1 years), Continue reading >>

Welcome To Journal Of The Association Of Physicians Of India

Received: 29.05.2017; Accepted: 04.06.2017 Background: Conventionally, diabetes management involved targeting the triad of FPG, PPG, and HbA1c. However, several studies have suggested the quintessential need for a paradigm shift to incorporate glycemic variability and quality of life in the holistic diabetes control regimen. Aim: To generate a consensus and ratify the position of Glycemic Variability (GV) and Quality of Life (QOL), along with the traditional triad, in diabetes management in India. To evaluate whether the triple fixed dose combination of metformin, glimepiride, and voglibose can accomplish the goals of glycemic pentad. Methodology: Glycemic pentad forum was instituted comprising of 55 experts from different regions of India in the field of diabetology who discussed various evidences related to the topic and shared their experiences and expressed their opinion on the relevance of glycemic pentad in the present diabetes management and whether triple fixed dose combination of metformin, glimepiride, and voglibose is able to achieve glycemic pentad targets. Results: Forum has come to a consensus that the conglomerate of quintuple elements FPG, PPG, HbA1c, glycemic variability and quality of life to be termed as glycemic pentad and these milestones to be considered for any antidiabetic therapy. Experts opined that combination therapy is required to achieve the Glycemic Pentad, as monotherapy might not address all the five arms of Glycemic Pentad. Group also agreed that the diabetes management in Indians require separate attention due to their distinct dietary habits (high carbohydrate content) and socio-economic status (economically weak and poorly educated). Therefore, mild adjustments to the standard practices in the western countries are suggested. After Continue reading >>

To work properly, the amount of glimepiride you take must be balanced against the amount and type of food you eat and the amount of exercise you do. If you change your diet, your exercise, or both, you will want to test your blood sugar to find out if it is too low. Your doctor will teach you what to do if this happens. glimepiride is available only with your doctor's prescription. Diabetic foot when untreated or when the sugar is uncontrolled, leads to sepsis, and eventually amputation (cutting off the infected toe or leg). I suggest you visit a physician who will advise appropriate medication, even if he does give you a scary lecture. Your situation is definitely scary. You will also require lifestyle modification, dietary changes and exercise. Drugs like glimepiride, gliclazide, glinides etc are given before food about 30 to 40 minute earlier. Because they stimulate insulin secretion. So when food reached the gut, adequate insulin should be available.Metformin is usually given after food as in some taking it before food may cause gastrointestinal symptoms. But in combination mostly it is given before food. Voglibose and acarbose are given with food as they inhibit enzymes which convert disaccharides to glucose and delay the absorption. This dosing time is not related anything else. Usually after food it takes about 2 to 3 hours for glucose levels to come down. After about 8 to 10 hours glucose level is lowest. Mr. Lybrate-user kumar, the present treatment looks quite good. Metformin belongs to class of medications called antidiabetics. It decreases the amount of glucose absorbed from the food and the amount ofglucose made by liver. Metformin also increases the body's response to insulin(a natural substance that controls the amount of glucose in the blood). Triple Fixed Drug Combinations In Type 2 Diabetes

Triple Fixed Drug Combinations In Type 2 Diabetes

Type 2 diabetes is characterized by progressive beta-cell failure that results in sequential adding of different oral and injectable medications to achieve optimal glycemic control.[1,2] This necessitates administration of multiple tablets or pills to achieve good glycemic control as the disease progresses.[2] In addition, subjects with type 2 diabetes may have other co-morbidities such as dyslipidemia, hypertension and cardiovascular disease, which further increase the burden of medications. Polypharmacy with increased pill burden and dosing frequency is identified as one of the factors responsible for poor adherence to oral hypoglycemic therapy.[3] Even in countries with high access to healthcare, only 39% of patients reported good medication adherence.[4] In a study of 2741 patients on oral antidiabetic drug (OAD), there was an inverse relationship between OAD adherence and HbA1c; controlling for baseline HbA1c and therapy regimen, each 10% increase in oral diabetes medication adherence was associated with a 0.1% HbA1c decrease (P = 0.0004), suggesting that adherent patients are more likely to achieve glycemic control than the nonadherent ones.[5] Go to: One method to improve drug adherence is to use fixed drug combinations (FDC).[6] United States Food and Drugs Administration (USFDA) defines FDC as “two or more drugs may be combined in a single dosage form when each component makes a contribution to the claimed effects and the dosage of each component (amount, frequency, duration) is such that the combination is safe and effective for a significant patient population requiring such con-current therapy as defined in the labeling for the drug.”[7] The rationality of FDCs should be based on certain aspects such as: The drugs in the combination should act by differe Continue reading >>
