HYDERABAD: People turning up at the doorstep of so-called 'diabetologists' need to be clear about who they are actually visiting, say experts, who warn that the 'diabetologist' tag is not a legally approved by the Medical Council of India (MCI). The term is loosely used to refer to either an endocrinologist (a super-specialist doctor in diabetes and other hormones with three years training after the PG medical course) or an MD general physician (three year PG course after MBBS) practising diabetes management. Experts say a third group also use this tag and they are the cause of worry. "This third group mostly comprise MBBS doctors who may have gone through unrecognised courses for a few months and started practising as diabetelogists. Since these courses are not recognised by MCI, their practice can be called illegal," said Dr Shyam Kalavalapalli , director, Institute of Diabetes, Endocrinology and Adiposity. Dr K Ramesh Reddy, MCI member said only endocrinologists or MD general physicians are entitled to practice diabetes management. This recognition is for joining a medical college for teaching purposes only. You do not need MCI recognition for practicing a specialty. There are many Medical colleges in India whose MBBS itself is not recognized by MCI but doctors are allowed to practice anywhere. No fellowship or a certificate course is recognized by MCI in India. Same is true abroad also. These courses train a doctor to take up a specialty subject which he/she can practice later on. 2. What is the duration of the course & which is the ideal course to enroll in? It is ideal to do the 2 year course if one is post MBBS even though the Rajiv Gandhi Heath Universitys ( RGUHS) Certificate course is for 1 year. Research Society for Study of Diabetes (RSSDI ) is the National body that governs Diabetes in India. RSSDIs Advanced course in Diabetology is now approved by Jaipur National University from January 2017 batch onwards. This course is for 1 year if the candidate has completed his/her MD in Internal Medicine & is for 2 years if the candidate has completed only his/her MBBS. Diabetes is currently reaching epidemic proportions in the United States with over 29 million persons affected in this country. The total costs of healthcare for people with diabetes in the U.S. exceeded $322 billion in 2012. Eastern North Carolina is one of the areas in the diabetes belt that is defined by having counties with over an 11% prevalence of diabetes. The incidence is higher in Eastern North Carolina given the high percentage of minorities, the high prevalence of obesity, and substandard health care in much of our rural area. This is certainly bad news for the health of our area but a great opportunity for learners. The good news is that thanks to medical research, we are undergoing an unprecedented explosion in the development of new treatments for patients with diabetes. What is ironic and most unfortunate is that at this very time we are facing a serious shortage of diabetes specialists in the U.S. and in areas Eastern N.C. in particular. Most diabetologists are endocrinologists, i.e., physicians who have been formally trained in internal medicine or pediatrics and have completed an additional two or three year fellowship in either adult or pediatric endocrinology to develop an expertise in diabetes. There is a huge shortage of endocrinologists in the US. Furthermore, many endocrinologists are not passionate about or even interested in caring for people with diabetes. Unfortunately, true diabetes specialists may be considered an endangered species with the dwindling ranks of endocrinologists who dedicate themselves to care of the patients with diabetes.
