Of these, over half live in developing nations, with India being home to an estimated 97,700 children with T1DM. This is more than the relative burden of type 2 diabetes India has to shoulder: every fifth TIDM child in the world is an Indian, while (only) every seventh type 2 diabetes mellitus (T2DM) adult across the globe claims Indian nationality.[ 1 ] In Bangladesh the estimated prevalence of diabetes for the year 2010 in age group 20-79 years was 6.1%[ 2 ] and for the year 2011 it has been raised to 9.58%.[ 1 ] The incidence of T1DM is rising by 3% per annum, especially in younger children and in so-called low-prevalence countries. Type 1 diabetes mellitus (T1DM) has a wide presence in children and has a high mortality rates. The disease, if left unmanaged, poses various challenges to the patient and health care providers, including development of diabetic complications and thus decreasing the life expectancy of the affected child. [1] A study in USA showed that when a 10-year-old boy or girl develops diabetes, he/she loses almost 18.7-19 years of life compared to their nondiabetic counterparts. A researcher has also stated "focused studies show that outcomes can be improved by better management, but there is no evidence of this actually happening on a worldwide basis." Newer treatment approaches have facilitated improved outcomes in terms of both glycemic control and reduced risks for development of complications. The major challenges remain in the development of approaches to the prevention and management of T1DM and its complications. The challenges of T1DM include awareness of the disease which is very poor amongst the general public and also in parents of T1DM children, and this needs to be improved to effectively counter the problem of T1DM. The general population is unaware of the existence of diabetes in children of 2-5 years of age and believe that it affects only the middle-aged group. Awareness among the public health care professionals is also very poor, and the syllabus curriculum of medicine students does not emphasize much on T1DM. The challenge of lack of awareness of T1DM can be met by increasing public awareness programs, conducting workshops for diabetes educators regarding T1DM in children (giving details of symptoms, diagnosis, diet, monitoring and management), newsletters, CMEs, online courses, and by structured teaching modules for diabetes educators. I saw the poverty and the need. But with the recent passing of Reghunath Varma , a young man from India who could afford insulin and died for lack of it, I realized it is not just access to medicine and supplies that needs to be fixed in impoverished countries. It is basic education -- and and at times demystifying cultural superstition. In India, where most people with Type 1 diabetes die because they can't afford insulin, a young man died because he chose not to take his. Reghunath Varma was 29. He was told by an acupuncturist to stop taking his insulin -- the medicine that keeps people with Type 1 diabetes alive -- because the acupuncturist's treatment would cure him. In India many revere doctors as gods. During my three weeks in Bangalore at the Jnana Sanjeevini Medical Center, one evening a 23-year-old woman arrived. She had Type 1 diabetes since the age of 5 and already had numerous diabetes complications, including the inability to straighten her fingers. AURANGABAD: A 26-year-old city woman struggled to find a suitable life partner, because every time she disclosed she had Type 1 diabetes , the marriage proposal would be rejected. Married last year, her partner valued that she had disclosed her medical condition. "Some youth with Type 1 diabetes get into a compromised marriage with some negotiation. Very few find a rational partner, who accepts them with diabetes," she said. According to the World Health Organisation , Type 1 diabetes results from the body's failure to produce insulin and was previously called to as 'insulin-dependent diabetes mellitus' (IDDM) or 'juvenile diabetes'. Interestingly, a matrimony agency has set up a centre in Delhi for Type 1 diabetics, who find it difficult to find partners. Physician Aniket Inamdar said that a survey was conducted in India in 2006 on marriage-related problems among diabetics. Of the 2,000 respondents in age group of 19 to 31 years, finding a suitable life partner was among the most common concern, he said. An investigation was carried out by expert Ahlfield JE, Soler NG, Marcus SD on the impact of insulin-dependent diabetes mellitus on marriage and having children a study on 50 young adults (aged 20-35 yr) with onset of the disease less than 20 year was done. None of the young adults in the study had made a conscious decision to remain unmarried, but 14 of 50 had decided not to have children.
