Despite many proposals, only a few projects are dedicated to the development of complete recommender system infrastructures that incorporate BG prediction algorithms for diabetes patients. The development and implementation of such a system aided by mobile technology is of particular interest to patients with gestational diabetes mellitus (GDM), especially considering the significant importance of quickly achieving adequate BG control for these patients in a short period (ie, during pregnancy) and a typically higher acceptance rate for mobile health (mHealth) solutions for short- to midterm usage. Objective: This study was conducted with the objective of developing infrastructure comprising data processing algorithms, BG prediction models, and an appropriate mobile app for patients’ electronic record management to guide BG prediction-based personalized recommendations for patients with GDM. Objective: To summarize current data on the magnitude, prevalence, variability, pathogenesis, and management of the dawn phenomenon in patients with diabetes mellitus. Methods: On the basis of the pertinent available literature and clinical experience, we propose a quantitative definition of the dawn phenomenon, discuss potential pathogenic mechanisms, and suggest management options. Results: The "dawn phenomenon" is a term used to describe hyperglycemia or an increase in the amount of insulin needed to maintain normoglycemia, occurring in the absence of antecedent hypoglycemia or waning insulin levels, during the early morning hours. To be clinically relevant, the magnitude of the dawn increase in blood glucose level should be more than 10 mg/dL or the increase in insulin requirement should be at least 20% from the overnight nadir. Controversy exists regarding the frequency, reproducibility, and pathogenesis of the dawn phenomenon. Approximately 54% of patients with type 1 diabetes and 55% of patients with type 2 diabetes experience the dawn phenomenon when the foregoing quantitative definition is used. The most likely pathogenic mechanism underlying the dawn phenomenon is growth hormone-mediated impairment of insulin sensitivity at the liver and muscles. The exact biochemical pathways involved are unknown. Therapeutic decisions aimed at correcting fasting hyperglycemia should take into account the variability and magnitude of the dawn phenomenon within individual patients. Ancient literature from Egypt and India describe the disease; it was easily identifiable because a patients urine attracted ants ( 1 ). More than two millennia later, monitoring glycemic status continues to be central to clinical management. It is currently the only variable accepted for standardization of both diagnosis as well as treatment ( 2 ). Despite its strong association with diabetes, however, hyperglycemia is not the disease per se. It is important to ask what other variables besides glucose are relevant to the disorder. In particular, it would be very useful to discover covariates of glucose that can help predict patient recovery on anti-diabetic treatment. The recognition that medical care needs to be personalized since individual responses to anti-diabetic treatment vary with patient pathophysiology is relatively recent. The American Diabetes Association, for example, now recommends that targets of glycemic control be selected individually, not uniformly ( 3 ). Raz et al. point to the need for data that can help identify what characteristics of patients determine how well they respond to specific treatments ( 4 ). This article discusses the links between diabetes and cancer, the different agents available for treating diabetes, and the cancer risk associated with these therapies. Exogenous insulin, insulin secretagogues, and incretin-based therapies are under scrutiny because of their potential influences on cancer development in a population already at risk. At present, we lack adequate prospective data on the cancer risk from diabetes drugs. Patients with a personal history of bladder cancer should avoid pioglitazone, and those who have had pancreatic cancer should avoid incretin therapies until definitive clinical data become available. Patients with a personal or family history of medullary thyroid cancer or multiple endocrine neoplasia type 2 should not receive glucagon-like peptide-1 receptor agonists.
