Download PDF(368.6 KB) Download PDF(368.6 KB) One of the most important goals in the treatment of type 2 diabetes mellitus (T2DM) is to prevent complications at both the macrovascular and microvascular level. To do so, diabetes treatment aims to control glycaemia, blood pressure, and cholesterol levels, and if necessary to reduce body weight and achieve smoking cessation. More than 20% of all individuals with T2DM in primary care need insulin treatment ( Sharma 2016 ), and this group needs to be especially capable of adequate self-management, which means they should make healthy food choices, exercise frequently, monitor their blood glucose regularly, administer insulin properly, and adjust both dietary intake and insulin dose in relation to physical activity. However, optimal self-management of T2DM requires sufficient knowledge on relevant topics and often behavioural change. REVIEW ARTICLE Maria de Fátima Ferreira GrilloI,II,III; Cristina Rolin NeumannI,III; Suzana Fiore ScainI,II; Raquel Farias RozenoIII; Jorge Luiz GrossII,III; Cristiane Bauermann LeitãoII,III,* IPrimary Health Care Service, Porto Alegre, RS, Brazil IIEndocrinology Service, Hospital de Clínicas de Porto Alegre, Porto Alegre, RS, Brazil IIIUniversidade Federal do Rio Grande do Sul, Porto Alegre, RS, Brazil ABSTRACT Education plays an important role in diabetes mellitus (DM) treatment, as it enables patients to manage their disease. There is a wide range of tested educational interventions, and, to date, no universal model that can be standardized and recognized as effective for all individuals with the disease has been defined. This article aims to review the effect of different types of educational interventions for self-management of glycemic control in patients with DM type 2, in addition to define general recommendations for this treatment strategy. Keywords: Diabetes mellitus; Education; Health education; Self-management Introduction Diabetes mellitus (DM) has become a serious public health problem due to its high prevalence and to the chronic nature of the disease and its complications. The global prevalence of DM in adults is estimated to be 9%.1 In Brazil, the prevalence was 7.6% in the 80's,2 increasing in 2003 to 12% in men and 16% in women,3 and, recently, a population-based study carried out in the state of Rio Grande do Sul estimated that 12.4% of adults showed this condition.4 In Brazil, the direct costs of the disease vary between 2.5% and 15% of the annual budgets allocated for health care,5 which represents approximately US$ 3.9 billion.6 The approach currently adopted in Brazil to treat DM is proving to be inefficient. Recently, a multicenter study car Continue reading >>

My Aade Network : Blogs : The 2017 Revised National Standards For Diabetes Self-management Education And Support

The 2017 Revised National Standards for Diabetes Self-Management Education and Support Wondering what changes were made to the National Standards for DSMES and how they impact your DSMES service? This blog is intended to give a brief overview of the standards, highlight the changes that were made, and how to implement them into your DSMES services. Here is the article in full (if you are an AADE member you can access it free of chargejust sign in), published in The Diabetes Educator: It can also be found on the American Diabetes Association website: At the end of this blog is my contact information. I help DSMES services with meeting the National Standards and Recognition or Accreditation, so that you can bill Medicare. Im happy to help any way I can! The National Standards for Diabetes Self-Management Education and Support were developed and published in 1984, and programs were first recognized in 1987 using a review process based on the Standards. ADA became a National Accrediting Organization (NAO) in 1986, followed by AADE in 2009. CMS began reimbursing for education in 1997, and the National Standards have been the basis for determining quality and evidence-based diabetes education programs that could bill and be reimbursed for their services. I had the honor of serving on the current Revision Workgroup, and I have to say, it was an incredible learning opportunity! The Standards are typically revised every five years; the last revision happened in 2012, when Montanas Carla Cox was on that particular revision committee. It is possible that the next revision might be in three years because of how healthcare and DSMES are changing so quickly right now! One of the goals of this work was to not make things more complex for those offering DSMES services, but to continue Continue reading >>

But what type of education do you think is effective for patients? I think a more comprehensive plan would be beneficial to patients. I know that cost can be a factor. Providing patients with proactive coping skills - using mental simulations to help patients anticipate and overcome potential barriers to compliance. Patients who set realistic goals are more apt to be compliant. Peer groups can provide additional learning and support. Diabetes is a lifelong and progressive disease and needs ongoing self-management interventions that provide follow up and support. But what does that mean? What type of mental simulations? How do you get patients to actually find a treatment plan that works for them? I've had type 1 diabetes for 36 years. I have had access to exceptional doctors and diabetes educators (my first doctor was Donnel Etzweiler at Park Nicollett who is called the "father of diabetes education" and as an older child I was a patient of the Barbara Davis Center for Childhood Diabetes in Denver). Diabetes Self-management Education And Support In Type 2 Diabetes: A Joint Position Statement Of The American Diabetes Association, The American Association Of Diabetes Educators, And The Academy Of Nutrition And Dietetics

Diabetes is a chronic disease that requires a person with diabetes to make a multitude of daily self-management decisions and to perform complex care activities. Diabetes self-management education and support (DSME/S) provides the foundation to help people with diabetes to navigate these decisions and activities and has been shown to improve health outcomes (1–7). Diabetes self-management education (DSME) is the process of facilitating the knowledge, skill, and ability necessary for diabetes self-care. Diabetes self-management support (DSMS) refers to the support that is required for implementing and sustaining coping skills and behaviors needed to self-manage on an ongoing basis. (See further definitions in Table 1.) Although different members of the health care team and community can contribute to this process, it is important for health care providers and their practice settings to have the resources and a systematic referral process to ensure that patients with type 2 diabetes receive both DSME and DSMS in a consistent manner. The initial DSME is typically provided by a health professional, whereas ongoing support can be provided by personnel within a practice and a variety of community-based resources. DSME/S programs are designed to address the patient’s health beliefs, cultural needs, current knowledge, physical limitations, emotional concerns, family support, financial status, medical history, health literacy, numeracy, and other factors that influence each person’s ability to meet the challenges of self-management. It is the position of the American Diabetes Association (ADA) that all individuals with diabetes receive DSME/S at diagnosis and as needed thereafter (8). This position statement focuses on the particular needs of individuals with type 2 diabet Continue reading >>
