Prevalence of vitamin D deficiency in pregnant women and its association with gestational diabetes mellitus Al-Musharaf, Sara (2017)Prevalence of vitamin D deficiency in pregnant women and its association with gestational diabetes mellitus. PhD thesis, University of Warwick. Several reports have linked vitamin D deficiency with an increased risk of gestational diabetes mellitus (GDM). Both of these conditions are alarmingly common in Saudi Arabia, and pose additional risk of developing future metabolic disease. This study, therefore, investigates the vitamin D status amongst pregnant Saudi women, and the potential influence of vitamin D deficiency on metabolic dysfunction, such as GDM. A total of 578 pregnant women (28.8 5.4 years) were recruited for this study during their first trimester of pregnancy (8-12 weeks) and followed up in their second trimester (24-28 weeks), where data were collected from 297 [51.3% (297/578); 28.9 5.3 years] women. The study collected socio-economic, anthropometric and biochemical data, along with dietary intake, physical activity and sun indices. The findings of this study indicate that during the first trimester 81% of women being vitamin D deficient, dropping to 77% in the second trimester. It was also noted that being younger in age, multiparous, having a lower level of education, being a housewife, and living in West Riyadh were all associated with vitamin D deficiency during the first trimester (p < 0.05), and this further corresponded to reduced sun exposure (p < 0.001). In contrast, physically active pregnant women, women adequately exposed to sunlight at noon (p < 0.001), and residents of North Riyadh all had significantly higher circulating vitamin D levels (p < 0.05). When a crude aqueous extract of Artemisia was given in their drinking water, it had little effect on these symptoms after 10 days of treatment. Title: Clinical Application of the Food Insulin Index to Diabetes Mellitus Authors: Bell, Kirstine Issue Date: 14-May-2014 Publisher: University of Sydney. Abstract: The Food Insulin Index (FII) is a novel system of ranking foods based on the insulin response in healthy subjects relative to an isoenergetic reference food. The goal of this thesis was to explore the clinical application of the FII to mealtime insulin dosing in type 1 diabetes. Carbohydrate counting is the current gold standard method for determining prandial insulin dose in type 1 diabetes, however only 7 studies assessing the efficacy of carbohydrate counting for glycaemic control in people with type 1 diabetes could be identified. Meta-analysis revealed there was no significant improvement in HbA1c with carbohydrate counting over general dietary advice and/or usual care (-0.35%, p = 0.096). This study highlighted the need for research into alternative strategies to improve the accuracy of the mealtime insulin dose. The FII reveals a notable insulin demand for foods high in protein and fat, nutrients that would normally be disregarded for mealtime insulin dosing with traditional carbohydrate counting. Compared with carbohydrate counting, the FII algorithm was able to reduce mean blood glucose level for six foods high in protein (5.7 +/- 0.2 mmol/L vs 6.5 +/- 0.2 mmol/L, p = 0.003), without significantly increasing the risk of hypoglycaemia (p = 0.155). Diabetes is a chronic disease that influences almost all aspects of life of an individual, especially Quality of Life (QoL). This PhD thesis is the outcome of a research that aimed to assess the QoL of people with diabetes in Greece. The study is important because the area of diabetes-related QoL is generally understudied and is the first to have investigated this within the Greek population. This work involved the creation of the Greek version of the Audit of Diabetes Dependent Quality of Life (ADDQoL-Gr) through translating, culturally adapting and validating the instrument as a QoL measure. The version was accepted by the author and the MAPI institute as the Greek version of the ADDQoL. The QoL for people with diabetes in Greece was investigated using the ADDQoL-Gr. An assessment of their health status using the SF-36, as well as the relationship of QoL and health status with medical and socioeconomic factors was also investigated. 1027 diabetes patients participated in this cross-sectional analytical study from 8 different sites from central and northern Greece. This thesis contributes to knowledge because i) identifies some characteristics of QoL predictors; ii) explores and prioritises QoL determinants; iii) reveals the complexity of the relationship between medical-socioeconomic factors and QoL; and iv) offers an instrument for measuring diabetes-related QoL for Greek speaking populations.The main conclusion drawn from this research was that diabetes seriously affects QoL, with complications (microvascular disease followed by foot amputation) having the most adverse effect. The use of ensembles of classifiers, and specifically ensembles of feed forward neural networks, for the prediction of Diabetes Mellitus for Pima Indian women and Cardiovascular Diseases for patients with Type 2 Diabetes Mellitus has been examined. Several classifiers have been developed, others follow the Bagging paradigm, others are ensembles of Feed-forward Neural Networks with different numbers of hidden neurons or layers, others follow the Binary Logistic Regression paradigm, others follow the Bayesian approach, and others are variations of Decision Trees. It has been shown that the ensembles of classifiers that have been trained following the Bagging approach and the ensembles of feed forward neural networks with different numbers of hidden neurons or layers have achieved the highest levels of prediction accuracy and their predictions are closer to the real risk scores, as indicated by the results of the Hosmer-Lemeshow test. Background The prevalence of diabetes, chiefly type 2 (T2DM), is particularly high in the United Arab Emirates (UAE). Effective management of the disorder and its co-morbidities is needed; however quality of T2DM care is variable and suboptimal worldwide. In the UAE, few studies have been undertaken to systematically review the prevalence of T2DM and its risk factors and any changes in these trends overtime. Also, studies on the quality of T2DM care and factors influencing it are lacking. Aim To examine the quality of care provided to people with T2DM in Abu Dhabi, particularly Al-Ain, and identify factors influencing it. Methods This was a multi-method study involving systematic reviews, and quantitative and qualitative approaches. Quantitative data were collected from a random sample of medical records of people with T2DM to assess the quality of T2DM care and improvement overtime and investigate any differences in the care provided to different age groups and genders. The qualitative method includes semi-structured interviews with healthcare professionals to investigate factors affecting the quality of T2DM care. Results Findings from the quantitative study demonstrated that the care provided to people with T2DM is sub-optimal for glycaemic and blood pressure control. Better glycaemic control was more common among people aged 40 and above. However, encouraging progress with regard to intermediate outcomes of diabetes control including glycaemic and lipid between 2008 and 2010 was found among both genders.
