El Centros Diabetes Self-Management Program offers a structured setting with ongoing diabetes education classes and support groups to teach the skills needed in the day-to-day management of diabetes. The program offers continuous medical care, appropriate diagnostic testing and follow up, and constant education to provide information and motivation for patients to lead healthier lifestyles. Monthly group classes for patients who have diabetes or are at risk for diabetes are taught by a certified diabetes educator (CDE). In addition, El Centro offers one-to-one visits with the CDE to improve access to diabetes self-management education. Weekly educational workshops are part of El Centros outreach and are offered free of charge to the community and as well as to El Centro patients, which cover topics such as nutrition, exercise, mental health (as associated with living with diabetes), medication side effects, disease complications, foot care and self-monitoring activities. El Centros diabetic patients also receive vouchers for free eye examinations at the University of Houston Eye Institute, and receive smoking cessation counseling through M. D. Reed, a Certified Diabetes Educator (CDE) from TMF Health Quality Institute, Austin, Texas and one of the main facilitators of the Faith and Diabetes Initiatives Congregational Health Leadership training program. By integrating faith into diabetes awareness and support, the Faith and Diabetes Initiative in Houston adds a new twist to the traditional diabetes care team. Lay members of various religious communities are trained to provide an evidence-based diabetes education curriculum, peer support, and prevention programs in their houses of worship, whether churches, mosques, synagogues or temples. Our Diabetes Education Program provides information and education to people with all types of diabetes, as well as insulin resistance and pre-diabetes. Participants learn lifestyle behaviors to help manage their conditions and lead active lives. The Kingwood Medical Center Diabetes Education Center is accredited by The American Diabetes Association and follows The National Standards for Diabetes Self Management Education. Certified Diabetes Educators (CDEs) and Registered Dietitians (RDs) provide training, counseling, and follow up for each participant in our Diabetes Self-Management Program. Meal Planning, Healthy Food Choices, Portion and Carbohydrate Control The program is open to those diagnosed with: The program can help those newly diagnosed, who need an update or review of their diabetes management plan and Insulin Pump users. Families and support persons are welcomed to attend the visits and classes. Persons with or without diabetes who need additional nutritional counseling and support can also use our program/services. These patients would need a physician order for Medical Nutrition Therapy (MNT) to schedule an appointment. We offer both group classes and individual appointments. The program focuses on individualized treatment plans for people with diabetes to help them self-manage the disease and improve their health and vitality. All patients with pre-diabetes or newly diagnosed diabetes or those who have never attended a diabetes self-management class should attend these classes. Designed to help individuals understand diabetes and learn simple strategies to gain control over the disease, these classes introduce the four cornerstones of diabetes education: medication, monitoring, diet and exercise. Patients will attend two three-hour classes. Other topics include glucose pattern management, foot care, preventing long-term complications, weight loss, basic meal planning, introduction to carbohydrate counting and managing sick days and travel. Advanced diabetes classes are designed for patients who have had diabetes for some time and have a basic understanding of self-management skills but are not getting the control they need. These patients take insulin or multiple oral medications. These classes teach patients how to identify blood glucose trends and troubleshoot situations that lead to out-of-range blood-sugar readings. In addition, more emphasis is placed on using carbohydrate counting to help with meal planning.
