The Touro Diabetes Center provides a comprehensive program of educational services and support to individuals and groups. The Diabetes Center works closely with physicians to identify each patients specific needs and helps them develop the self-management skills needed to control their disease. Overview of diabetes (Type 1 and 2, gestational) Prevention, detection and treatment of complications The educators in the Touro Diabetes Center are certified through the National Certification Board of Diabetes Educators, and the American Diabetes Association recognizes this program as meeting the national standards for diabetes self-management education. Our team also includes a licensed, registered dietitian to help coach patients through meal planning and weight control. To be eligible to become an IDF Diabetes Excellence in Diabetes Care, the institution/hospital/member organisation must provide evidence of the following criteria: Willingness to play a key role in the vision and contribute to the implementation of the IDF Global Strategic Plan. Institute must be providing holistic diabetes care and management (need to be a University Hospital, Diabetes Care Centre or equivalent) Written commitment of leadership to support all IDF initiatives Commitment to work closely with and act as a reference point for the IDF Global office. Willingness to accept and support review and assessment processes by IDF. Willingness to abide by IDF values and beliefs. As an integral part of the IDF School of Diabetes, IDF will appoint centres based on a competitive application process. Eachcentre will be independently evaluated before it is selected and must meet a high standard of care for the treatment of people with diabetes Each selected centre will be charged an annual fee of USD 3,000 to be granted the title of IDF Centre of Excellence in Diabetes Care. The Diabetes Ambulatory Care Centre at Royal Prince Alfred Hospital in Sydney provides a variety of ambulatory services, based on a team approach, for the person with diabetes. Diabetes Type 1
There are certain things that you should expect from your medical team. If you have just been diagnosed with Type 1 diabetes you should have: A full medical examination- this may also include a referral to a specialist eye clinic. A talk with a diabetes care nurse who will explain what diabetes is and about your treatment. Several sessions with your diabetes nurse for basic instruction on injection technique, looking after your insulin, blood glucose meter and pen, blood glucose testing and what the results of your own testing means. You should also expect to have a discussion about hypoglycaemia (hypos) and how to deal with it. After this initial help, you should have access to a diabetes care team where you will have the opportunity to talk to doctors, nurses and dieticians. They will assess your diabetes control and discuss any problems with you. You should also be given a contact number so that you are able to contact a member of the team for advice whenever you need it. Most diabetic clinics have a specialist nurse who will visit you at home between hospital appointments especially in the early days after your diagnosis. Each year you are entitled to an Annual Review assessment by the diabetes care team.This should include a blood pressure check, a measurement of height and weight, drawing blood to find out your cholesterol level and your HbA1c (average blood glucose over the last few weeks), plus a urine test to check whether your kidneys are working well. They should also examine your feet and reflexes to check that your nerves are okay, and they may ask you whether you smoke and offer help to give up if you would like to do so. The JCMH Diabetes Center of Excellence provides important education and training. A Registered Nurse and a Registered Dietitian host diabetes education classes for newly diagnosed diabetic patients and patients who have diabetes and need education and lifestyle information. The JCMH Diabetes Center of Excellence team works with healthcare professionals on current treatment options, clinical protocols and research data. They teach diabetic patients and family members how to manage the disease in order to live longer, healthier lives. On the Jackson County Memorial Hospital campus,East entry of the Hospital annex Individualized education and self-management training is offered in all aspects of diabetes self-care including: Preventing, detecting and treating complications What type of diabetes programs are available? This is a group program for those newly-diagnosed with type 2 diabetes or those who have had no training in managing diabetes. Based on the idea of creating a connection to nature, the Center weaves together the indoors and outdoors, in order to stimulate and nurture patients and visitors. The main entrance to the project faces south to ensure natural lighting, and features a rolling landscape that leads inside, with an in situ poured concrete pathway and landscaped staircases that connect to a public rooftop garden. On arrival, visitors are greeted by a luscious, rolling landscape leading inside. The area is designed with curiosity in mind from the outset patients and visitors must feel welcome and enticed to explore. Courtesy of Vilhelm Lauritzen Architects, Mikkelsen Architects and STED Landscape Common and treatment areas are located on the ground floor of the Center surrounding six courtyards and interspaced by a series of waiting areas, silent rooms, library spaces, and reading benches, with research and treatment areas located above on the first floor.
