Diabetes Care: Starting Treatment With Humulin R U-500 Robert Hood, MD: Here we are with a patient on over 200 units of insulin a day, and their A1C [glycated hemoglobin] is not at goal. Typically, youve been working with them for many visits. The patients usually very happy to have an alternative, especially when the alternative involves taking fewer injections a day. But we need to do things properly so that we get good outcomes and minimize the risk of hypoglycemia. The first safety thing is that we have to assume these patients are probably not getting all of their injections, and thats why in the protocol we did a dose reduction when A1C was less than or equal to 8%: a 20% reduction in the total daily dose of insulin. Thats making the assumption that patients maybe arent getting all of their insulin. To be safe with the A1C not being outrageously high, lets cut back on the insulin and then we can go back upward subsequently. We encourage patients to take their insulin a half hour before meals, so we match insulin with food as much as we can. We encourage the patients to eat 3 meals a day and not skip them. But if theyre due for their dose of insulin and they miss their meal, they are told to cut their dose in halfnot withhold it altogether because it includes basal insulin, but reduce it in half to reduce the risk of hypoglycemia. Blood sugar monitoring is very important. In the titration protocol, there was an emphasis on dose reduction for hypoglycemia that would trump any dose increase for hyperglycemia, but blood sugars were to be checked 4 times a day and reviewed on a regular basis. Problems associated with the use of U-500 insulin syringes. We received two reports of accidental prescribing of the new BD U-500 insulin syringes for outpatients who were using a U-100 insulin product. In one case, an endocrinology clinic called the hospital pharmacy to ask how to order U-500 syringes. During the conversation, it became apparent that there was some confusion about the use of these syringes. The pharmacist stressed that the patient should not use a U-500 syringe to measure any insulin concentration other than U-500, or the measurement would be incorrect, leading to a 5-fold underdose. At another organization, some providers have been selecting U-500 insulin syringes in error via electronic prescribing systems. There have been 8 incidents where U-500 syringes were prescribed in error, although pharmacists intervened each time and prevented patients from receiving the wrong syringes. The computer system at the clinic where this happened listed the “U-500” designation far to the right of the entry, making it easy for both the prescriber and dispensing pharmacist to overlook it: “SYRINGE INSULIN SYRINGE 0.5ML 31G 6MM (U-500) XA854 100/Box….” As a result, pharmacy created a ‘quick order’ for U-500 syringes to minimize the risk of providers inadvertently selecting this item during routine order entry. Moving the “U-500” designation to the left of the insulin syringe entry also helps give it prominence. In addition, caution patients about the risk of a mix-up and dosing errors if multiple family members in the home use insulin and both U-500 and U-100 insulin syringes are available. Humulin R is the brand name for Eli Lilly's recombinant human regular insulin. The usual version of Humulin R is U-100 and contains 100 units of insulin activity per 1 milliliter of fluid. But there's also a rarely-used version called U-500. The U-500 insulin is 5 times more potent than U-100 insulin. U-500 insulin contains 500 units of insulin activity per 1 milliliter of fluid. People may need to use U-500 if their diabetes is not well-controlled with U-100. The U-500 version should be used only for patients requiring doses above 200 units a day. Important information about U-500 insulin Since the use of U-500 insulin is not as common as the use of U-100 insulin, some health professionals may not be aware of it. This by itself increases the chance of dispensing errors. The main problem with U-500 insulin is the risk of a mix-up between the insulin U-100 and insulin U-500 versions. If this occurs, it can result in very dangerous low blood sugar or very high blood sugar. A mix-up between the two insulin concentrations may occur if doctors or pharmacists select the wrong concentration (for example, selecting the U-500 regular insulin from computer screen instead of U-100). Sometimes the two concentrations appear one line apart on the computer screen, which sets up the possibility of selection errors.
