They show that it’s possible for some people to put their Type 2 diabetes into remission using a low-calorie, diet-based, weight management programme, delivered by their GP. Almost half (45.6%) of those who took part in the programme were in remission after a year. The study found there was a close link between Type 2 diabetes remission and total weight loss. 86% of people who lost more than 15kg on the programme were in remission after a year, as were 57% of people who lost 10 to 15kg, and 34% who lost 5 to 10kg. In the comparison group, where people had the best diabetes care currently available, but didn't try the new weight management approach, only 4% went into remission. Links between type 2 diabetes and diet have been well established – it is triggered when the body can no longer manage its blood glucose levels – but this latest study adds to growing evidence that the condition could be reversed in some cases. The team of researchers gave a very low calorie diet (VLCD) to rats with type 2 diabetes and found the animals' blood glucose levels rapidly lowered. What's more, they were able to reveal extra details about why exactly it was happening. "Using this approach to comprehensively interrogate liver carbohydrate and fat metabolism, we showed that it is a combination of three mechanisms that is responsible for the rapid reversal of hyperglycemia following a very low calorie diet," says one of the researchers, Gerald I. Shulman from the Howard Hughes Medical Institute in Maryland. The three liver mechanisms affected by the diet were the conversion of lactate and amino acids into glucose, the conversion of liver glycogen into glucose, and a decrease in fat content, which then improves the liver's response to insulin. Big New Study Shows how readily Type 2 Diabetes can be Reversed The new study, called DiRECT (Diabetes Remission Clinical Trial), which involved people with type 2 diabetes who had been diabetic for less than 6 years, was published online on Tuesday in the leading medical journal, the Lancet. The 298 patients who agreed to take part in the study were randomly allocated to either getting standard diabetes care from their GP, or going on a rapid weight loss diet. On this diet they were asked to stick to 800 calories a day for up to 20 weeks, then switch to a programme of long term support to maintain weight loss. The results were remarkable. Many patients commented on how easy the diet had been to stick to and though not everyone succeeded, those who did saw dramatic weight loss. One in four people lost more than 15kgs, which was impressive, particularly when you consider that many of the patients came from areas of severe deprivation. What was even more impressive was that nearly half the patients (46%) who were allocated to the rapid weight loss regime managed to get their blood sugars back to normal, without medication, compared to only 4% of those getting standard diabetes care. This follows a trial of an intensive weight loss programme for overweight and obese people with type 2 diabetes, conducted at GP surgeries in Scotland and Tyneside. People were randomised to follow either the Counterweight Plus weight loss programme or standard care for 12 months. Counterweight Plus is a low-calorie diet plan that involves an initial phase of consuming around 850 calories a day for 3 to 5 months. This is followed by a 2- to 8-week period where calorie intake is slowly increased. Participants are then encouraged to attend monthly advice meetings, with the aim of maintaining their weight loss. People on the Counterweight Plan lost 10kg on average, with around a quarter achieving the target of losing 15kg or more. Half went into diabetes remission – defined as normal blood glucose control – compared with only a handful in the standard care group. The dietary approach definitely shows promise, but there are several reasons to be cautious at this stage. This sort of intensive calorie restriction wouldn't be suitable for everyone and should only be conducted under careful medical supervision.
