We’ve all been there before. You wake up. Lay in bed for a few before getting your booty up to go kill the workday and accomplish big things. Check your blood sugar. 115 (6.3 mmol/l) stares back at you. You smile to yourself: life is good. Forty minutes later, when you sit down to eat, your CGM gives you a “high” notification, and you’re 180. You have eaten NOTHING. All you’ve done is prepare for the day and prepare food. Now you face the grim potential of chasing your sugars all day long. What the… This isn’t Dawn Phenomenon Many people would blame this rise in blood sugar on dawn phenomenon (DP), which has a similar endpoint, but a different mechanism. Dawn phenomenon is the result of hormones releasing in the body in the early morning – predominantly growth hormone, cortisol, epinephrine, and glucagon – which in turn increase insulin resistance. The current basal insulin from the pump or long-acting injections is no longer enough, and blood sugars rise. That hormonal surge happens around 2am-6am, with most of it occurring in the middle of the night. Let’s say you woke up at 8:30am and aren’t in the “DP zone.” It’s not DP. Then what? Feet on the floor The moment your feet touch the floor as you roll out of bed, you signal to your body, “Hey, I need energy for all the stuff I’m about to do!” Your body recognizes you haven’t eaten in lord knows how many hours. Although it’s not a major problem when it occurs from time to time, consistently high morning levels need your doctor’s attention. Causes of Morning Hyperglycemia High blood glucose in the morning typically occurs due to one of three distinct causes: Dawn Phenomenon The “dawn phenomenon” describes high morning glucose that occurs due to a natural rise in hormone levels. During the early morning hours between about 4 and 8 a.m., your body releases hormones like cortisol and growth hormones to get ready for the day. For reasons experts don’t completely understand, your liver produces extra glucose in response to these hormones. People without diabetes secrete more insulin to handle the extra glucose. But for people with diabetes, blood glucose levels can rise too high. Increased blood sugar due to the dawn phenomenon is usually treated with diabetes medication. If you are on insulin, your insulin levels may need to be changed. The Somogyi Effect It’s possible to experience low blood sugar, or hypoglycemia, while sleeping and not even know it. The potentially dangerous problem can occur for a number of different reasons—from not eating enough or taking too much insulin, to drinking too much alcohol. It doesn't seem fair, does it? You haven't eaten anything all night and you still wake up with high blood glucose! What is going on and how can you lower it? For those of you with diabetes, this post is for you! Basics Your body strives to keep blood glucose (BG) within a safe range, but with diabetes the balance is disturbed. The insulin your pancreas produces might not be sufficient to cover your BG lowering needs. As well, the insulin it does produce might not be handled properly by target body cells. And to make matters worse, your liver might be on glucose production overdrive. All of this results in your body's failure to control BG overnight as well as after meals or snacks. Dawn Phenomenon Due to normal daily changes in our body's release of hormones during sleep and wake cycles, BG typically starts to rise in the very early morning – starting about 3 AM - and continues to rise as the morning progresses. For folks without diabetes, insulin production simply increases and takes care of the rising BG. However, for folks with diabetes, unless there is medication on board to cover this early morning rise, BG will eventually rise out of target range by the time they wake up. The classic pattern is to see BG within target range at both bedtime and during the middle of the night, and then see a high fasting BG. If you have noticed that your fasting BG is creeping up over time and is no longer within target range despite sticking to a carb controlled eating plan and taking your diabetes medication as prescribed, then it might be time to talk with your doctor about your diabetes medication. I know it's a common problem, and one I've struggled with personally, so I want to make sure you get to see it, too. From Markus Berndt: It’s one of the first recommendations you get after being diagnosed with diabetes. “Get active, do more exercise, it’s good for you!” And since we’ve been a child we’ve heard that exercise is healthy. If we do it consistently we’re rewarded, literally, with an awesome beach body. Adding exercise into our day is also good for our diabetes. We’re taught that exercise lowers blood sugar, right? But can the opposite also be true? Can you have high blood sugar after exercise? Up close We now know that physical activity usually lowers blood sugar because it reduces how much insulin is needed to move sugar into the cells. While, in the past, most experts advised frequent training intervals at moderate intensity, but recent studies have shown that even short, intense workouts are very effective. For example, a 15-minute intense weight training lowered blood sugar even more than what’s seen in some endurance training. So activity lowers blood sugar – but not always! Personally, I experienced this very early on and was extremely irritated! I just learned that exercise lowers blood sugar, but an intense 45-minute run consistently resulted in higher blood sugars than when I started! What in the world? That early morning jump in your blood sugar? It's called the dawn phenomenon or the dawn effect. It usually happens between 2 and 8 a.m. But why? Generally, the normal hormonal changes your body makes in the morning will boost your blood sugar, whether you have diabetes or not. If you don't, your body just makes more insulin to balance everything out. You don't even notice that it's happening. But if you have diabetes, it's different. Since your body doesn't respond to insulin the same as most, your fasting blood sugar reading can go up, even if you follow a strict diet. The boost in sugar is your body's way of making sure you have enough energy to get up and start the day. If you have diabetes, your body may not have enough insulin to counteract these hormones. That disrupts the delicate balance that you work so hard to keep, and your sugar readings can be too high by morning. The effects of the dawn phenomenon can vary from person to person, even from day to day. Some researchers believe the natural overnight release of what are called counter-regulatory hormones -- like growth hormones, cortisol, glucagon and epinephrine -- makes your insulin resistance stronger. This will make your blood sugar go up. You may also have high blood sugar in the morning because: You didn't have enough insulin the night before. You took too much or too little medicine. Many people with diabetes find that their fasting blood sugar first thing in the morning is the hardest blood sugar to control. In addition, they find that if they eat the same food for breakfast as they do for lunch or dinner they will see a much higher blood sugar number when testing after breakfast than they see at the other meals. The reason for this is a normal alteration in hormones experienced by many people not just people with diabetes. It is called "Dawn Phenomenon." What Causes Dawn Phenomenon? The body prepares for waking up by secreting several different hormones. First, between 4:00 and 6:30 a.m. it secretes cortisol, epinephrine, and norepinephrine. You may recognize these as the hormones involved in the "fight or flight response." In this case, their job is more benign, to give you the energy to get up and moving so you can find the food your body needs for energy. To help you do this, these hormones also raise your blood sugar. After a long night's sleep, the fuel your body turns to to get you going is the glucose stored in the liver. So after these stress hormones are secreted, around 5:30 a.m., plasma glucose rises. In a person with normal blood sugar, insulin will also start to rise at this time but many people with diabetes won't experience the corresponding rise in insulin. So instead of giving their cells a dose of morning energy, all they get is a rise in blood sugar. Not Everyone Experiences Dawn Phenomenon Researchers who have infused different hormones into experimental subjects have found that the trigger for dawn phenomenon is a nocturnal surge in growth hormone. Blood Sugar: What Causes High Blood Sugar Levels In The Morning
There are two reasons why your blood sugar levels may be high in the morning – the dawn phenomenon and the Somogyi effect. The dawn phenomenon is the end result of a combination of natural body changes that occur during the sleep cycle and can be explained as follows: Your body has little need for insulin between about midnight and about 3:00 a.m. (a time when your body is sleeping most soundly). Any insulin taken in the evening causes blood sugar levels to drop sharply during this time. Then, between 3:00 a.m. and 8:00 a.m., your body starts churning out stored glucose (sugar) to prepare for the upcoming day as well as releases hormones that reduce the body's sensitivity to insulin. All of these events happen as your bedtime insulin dose is also wearing off. These events, taken together, cause your body's blood sugar levels to rise in the morning (at "dawn"). A second cause of high blood sugar levels in the morning might be due to the Somogyi effect (named after the doctor who first wrote about it). This condition is also called "rebound hyperglycemia." Although the cascade of events and end result – high blood sugar levels in the morning – is the same as in the dawn phenomenon, the cause is more "man-made" (a result of poor diabetes management) in the Somogyi effect. Keeping a good record of your diet, medication and blood glucose can often help you solve the mystery. Dawn phenomenon is a a sudden rise of blood glucose levels 10 to 20 milligrams per deciliter in the early morning between 2:00 and 8:00 a.m. It is caused by an increased release of hormones such as cortisol, glucagon and epinephrine while your body is preparing to wake up. These hormones increase insulin resistance and stimulate your liver to release glucose, causing blood glucose levels to rise. Insufficient insulin or medication, excessive carbohydrate snacks at bedtime or high-fat meals at dinner may also cause blood glucose to be elevated in the morning. To rule out if your high blood glucose is due to dawn phenomenon, your endocrinologist may ask you to eat a low-fat, carbohydrate-controlled dinner, maintain your usual physical activities and check your blood glucose around 2 or 3 a.m. for several days. Today’s Dietitian Vol. 14 No. 11 P. 18 Jill is frustrated. Her type 1 diabetes seems out of control, and she comes to your office at her wits’ end. She says she’s doing everything right: counting carbs, taking her insulin as prescribed, monitoring her blood glucose levels four times per day. A look at Jill’s testing logs and most recent blood work confirms there’s a problem. She has a hemoglobin A1c of 9.2, and her blood glucose levels are all over the map. Her numbers generally are fine before she goes to bed but incredibly high in the morning. Recently, her physician increased her nighttime basal insulin dose to counteract the morning highs, but things seem worse now than ever. Her breakfast bolus doesn’t seem to be effective, and her high blood glucose levels persist into the afternoon. “Fluctuating blood sugars can be very frustrating,” says Eileen M. Sturner, RD, LDN, CDE, BC-ADM. “RDs can play an important role in helping patients get to the bottom of problems such as morning highs. Dawn phenomenon The dawn phenomenon is a normal rise in blood sugar as a person's body prepares to wake up. In the early morning hours, hormones (growth hormone, cortisol, and catecholamines) cause the liver to release large amounts of sugar into the bloodstream. For most people, the body produces insulin to control the rise in blood sugar. If the body doesn't produce enough insulin, blood sugar levels can rise. This may cause high blood sugar in the morning (before eating). Somogyi effect If the blood sugar level drops too low in the early morning hours, hormones (such as growth hormone, cortisol, and catecholamines) are released. These help reverse the low blood sugar level but may lead to blood sugar levels that are higher than normal in the morning. An example of the Somogyi effect is: A person who takes insulin doesn't eat a regular bedtime snack, and the person's blood sugar level drops during the night. The person's body responds to the low blood sugar by releasing hormones that raise the blood sugar level. This may cause a high blood sugar level in the early morning. Loud snoring sleepers (many of whom may have sleep apnea), compared to quiet sleepers, double (2x) their risks of developing certain types of metabolic syndrome(s); including diabetes, obesity, and high blood pressure. This likelihood also increased dramatically to 80% in those who found it difficult to fall asleep and to 70% for those who woke up feeling not as refreshed. Blood Sugar and Sleep Problems Sleep can affect your blood sugar levels, and your blood glucose control can also affect your sleep. It’s a vicious cycle. As the amount of sleep decreases, blood sugar increases, escalating the issue. Lack of sleep has been shown to increase blood sugar levels and the risk of diabetic issues. Why Do I Have Low Blood Sugar In The Morning?
Your body uses blood sugar, called glucose, as a source of energy for cells and organs. Low blood sugar, also called hypoglycemia, happens when your body doesn’t have enough glucose to use for energy. People with diabetes mellitus may have low blood sugar in the morning due to too much long-acting insulin, also called background insulin and basal insulin. Insulin helps to manage blood sugar by allowing glucose to enter your cells, where it can be turned into energy. Too much insulin of any kind can cause low blood sugar. Some noninsulin medications to treat type 2 diabetes mellitus can cause hypoglycemia also. People without diabetes can also have low blood sugar, known as non-diabetic hypoglycemia. This is usually caused by lifestyle factors, such as diet and exercise habits. Low blood sugar is usually defined as a glucose reading below 70 milligrams per deciliter (mg/dL). Readings below 54 mg/dL are more significant and signal that you may need immediate medical treatment. If you have low blood sugar in the morning, you may wake up with some of these symptoms: headache sweating dry mouth nausea lightheadedness dizziness shaking hunger anxiety blurred vision pounding heart beat If your blood sugar dips below 54 mg/dL, you might have more severe symptoms, including: fainting seizures coma If you have any of these severe symptoms, get medical help as soon as possible.
