His wife, my patient, had brought him in to see me to clarify his diagnosis. He had undergone blood tests in Detroit and was told by his doctor there that he had diabetes. Upset and disbelieving, he refused to take medications. Once in Canada, he went to see his own family doctor. After doing blood tests here, his Canadian physician told him he was fine - and that he did not have diabetes. "Well, yes and no," I had to reply. It was an unusual situation based on different diabetes-testing criteria in the United States and Canada. The same patient with the same results can be told that he either has or does not have the condition. "Michael, both your doctors are right. In Michigan, you have diabetes. In Toronto, you don't." That didn't seem to help his peace of mind. Doctors are supposed to be without borders, and the diagnosis of common conditions should not change between countries that share so much. But American physicians are diagnosing diabetes even more aggressively than their Canadian counterparts. In January, 2010, the American Diabetes Association essentially made it easier for patients to be diagnosed with diabetes by adding the important Hemoglobin A1C test to the diagnostic list. Now, in the United States, a HbA1C level greater than 6.5 per cent means you have diabetes. I know it's a common problem, and one I've struggled with personally, so I want to make sure you get to see it, too. From Markus Berndt: It’s one of the first recommendations you get after being diagnosed with diabetes. “Get active, do more exercise, it’s good for you!” And since we’ve been a child we’ve heard that exercise is healthy. If we do it consistently we’re rewarded, literally, with an awesome beach body. Adding exercise into our day is also good for our diabetes. We’re taught that exercise lowers blood sugar, right? But can the opposite also be true? Can you have high blood sugar after exercise? Up close We now know that physical activity usually lowers blood sugar because it reduces how much insulin is needed to move sugar into the cells. While, in the past, most experts advised frequent training intervals at moderate intensity, but recent studies have shown that even short, intense workouts are very effective. For example, a 15-minute intense weight training lowered blood sugar even more than what’s seen in some endurance training. So activity lowers blood sugar – but not always! Personally, I experienced this very early on and was extremely irritated! The results of the nationwide Diabetes Control and Complications Trial (DCCT) show that the closer you keep your blood glucose to normal, the more likely you are to prevent diabetes complications such as eye disease, nerve damage, and other problems. For some people, other medical conditions, age, or other issues may cause your physician to establish somewhat higher blood glucose targets for you. What should your blood sugar levels be? Once diagnosed with diabetes, your health care team will review your "target" blood sugar levels with you. You will likely be told to start checking your blood sugars at home using a meter. Normal blood sugar levels (i.e., people who have not been diagnosed with diabetes) are usually between 4.0 mmol/L and 8.0 mmol/L. If your blood sugars are at levels recommended by your physician or primary health care provider, then it is said that your blood sugars are "in control." For people with either type 1 or type 2 diabetes, the recommended target blood glucose levels are: 4.0 mmol/L to 7.0 mmol/L when measuring blood glucose fasting or before eating 5.0 mmol/L to 10.0 mmol/L when measuring blood glucose 2 hours after eating (your physician or primary health care provider may recommend a range of 5.0 mmol/L to 8.0 mmol/L if you are not at your A1C target - see below) These are general recommendations - your health care provider may suggest different targets for you. In addition, pregnant women, the elderly, and children 12 years old and younger may have different targets. What is urine testing? Before the advent of home blood glucose monitors, the only way to monitor or check for high sugar levels was by urine testing.
