The emergency condition most type 2s dread is hypoglycemia, where plummeting blood sugar levels can bring on a dangerous semi-conscious state, and even coma or death. However, hyperglycemia, high-blood sugar levels consistently above 240 mg/dL, can be just as dangerous. Left untreated, at its most extreme high-blood sugar, can induce ketoacidosis, the build-up of toxic-acid ketones in the blood and urine. It can also bring on nausea, weakness, fruity-smelling breath, shortness of breath, and, as with hypoglycemia, coma. However, once they’ve been diagnosed with diabetes, most type 2s have taken steps to prevent or lessen the most dangerous effects of high-blood sugar levels. Their concern shifts to dealing with unexpected, sometimes alarming spikes in blood sugar levels. Anyone older than age 45 is advised to receive an initial blood sugar screening, and then, if the results are normal, to be screened every three years thereafter. Blood sugar levels for diabetics are slightly different from blood sugar levels for people without diabetes. Because a diabetic's blood sugar can vary more than an average person's can, there is a larger range for the diabetic. Doctors consider this range safe for the diabetic; though, all diabetics should strive to keep glucose or blood sugar levels as stable as possible to prevent sugar highs and sugar lows. Keeping stable blood sugar levels requires monitoring blood sugar and watching the intake of carbohydrates. Measurement Blood sugar is measured in milligrams per deciliter and percentage of sugar in the blood. The average milligrams per deciliter for non-diabetics are 70 to 100 mg/dL. After fasting for 14 hours, the measurement is percent. The average is four to five percent. The range for a diabetic is 70 to 120 mg/dL or a percentage of seven or under, after fasting for 14 hours. After Eating Everyone's blood sugar rises after eating. Even a person who is not diabetic may have blood sugar levels as high as 175 mg/dL after a meal. The difference for a non-diabetic is the sugar levels drop to normal faster than the sugar rates of a diabetic. However, a diabetic should try to keep blood sugar levels under 180 mg/dL after a meal. In the U.S. it is measured in milligrams per deciliter, abbreviated as mg/dl. In most of the rest of the world it is measured in millimoles per liter, abbreviated as mmol/L. The concentration of glucose in our blood changes continually throughout the day. It can even vary significantly from minute to minute. When you eat, it can rise dramatically. When you exercise it will often drop. The blood sugar measures that doctors are most interested in is the A1c, discussed below. When you are given a routine blood test doctors usually order a fasting glucose test. The most informative blood sugar reading is the post-meal blood sugar measured one and two hours after eating. High Blood Sugar After Meals And What To Do About It
I will admit to having a bit of a diabetes crush on Gary Scheiner, MS, CDE, Type 1, and founder of Integrated Diabetes Services, LLC. And you know what? I’m not the only one. I recently saw Gary give a talk called “Strike the Spike” at the American Association of Diabetes Educators’ (AADE) 2013 conference to a room packed with diabetes educators. The point was to help CDEs understand why managing/avoiding post-meal blood glucose spikes is important – and to learn new techniques for how to do so. I was there because I am constantly struggling with post-meal spikes. I appear to digest food quickly and absorb insulin slowly — that’s why I’m on Symlin, which helps slow down the emptying of my stomach so I’ve got some chance of having my insulin start working by the time my food makes it to my blood. (I love my Symlin.) But I wanted to hear what other tips Gary might have, and what the responses might be. Gary started with a seemingly simple question: why should anyone care about post-prandial (i.e. post-meal) spikes? At first this question made me furrow my brow — I can’t imagine that anyone wouldn’t care about post-meal blood glucose spikes — but I guess that in some cases, health care providers stress the A1c average more than they do the swings in between. Normal sugar ranges or levels, mean the normal range of concentrations blood glucose levels (sugar) that occur in the blood measured by a test. Values that are below or above the range of values considered "normal" are abnormal. either too high or two low ( hypoglycemia and hypercyglycemia). Diabetes is defined as a disease in which the body has an impaired ability to either produce or respond to the hormone insulin . People with type 1 diabetes have a pancreas that does not make insulin. People with type 2 diabetes have cells in the body that are resistant to insulin or have a pancreas that slows or stops producing adequate insulin levels (blood glucose). Both types of diabetes can result in abnormal glucose levels. Normal blood levels may range slightly depending on what blood tests are used, and your doctor may have, but the variances are small. In addition, what are normal ranges for nondiabetics are not the same for diabetics; it is generally accepted that target blood sugar measurements for people with diabetes will be slightly higher than those without diabetes. A person who is does not have a normal glucose range of 72-99mg/dL while fasting and up to 140mg/dL about 2 hours after eating. People with diabetes who have well-controlled glucose levels with medications have a different target glucose range. These people may have a fasting range of about 100 mg/dL or less and 180mg/dL about 2 hours after eating. 7 Techniques To Reduce Post-meal Spikes During Pregnancy
“Gary, I think I need more insulin at breakfast.” “Why do you say that, Julianne?” “Because I’m always having high readings right afterwards, and my obstetrician said I shouldn’t spike after I eat.” “And what happens after the spike?” “It usually comes down to normal before lunch. So do you think I should take more insulin?” After-meal blood sugar spikes can create quite a quandary for anyone with diabetes, particularly during pregnancy. Research has shown that fetal macrosomia (overgrowth of the baby) becomes more common when post-meal blood sugars exceed 120 mg/dl (6.7 mmol). With post-meal readings above 140 mg/dl (7.8 mmol), the risk more than doubles from baseline. Fetal macrosomia can cause many problems during pregnancy. When the baby grows and develops too rapidly, it can lead to a premature and more complicated birth. It may also cause injuries to occur to the baby during delivery. Why do after-meal blood sugars have such a major influence on the baby’s growth? Nobody knows for certain. Perhaps, when the mother’s blood sugar “spikes” suddenly after meals, the baby is fed more sugar than its pancreas can “cover” with insulin, and high fetal blood sugar results. And because the baby’s kidneys spill almost all excess sugar from the baby’s bloodstream back into the amniotic fluid, the baby then drinks in the extra glucose and winds up growing more than it should. It is important to check your blood glucose (sugar) levels, because it will: Provide a quick measurement of your blood glucose (sugar) level at a given time; Determine if you have a high or low blood glucose (sugar) level at a given time; Show you how your lifestyle and medication affect your blood glucose (sugar) levels; and Help you and your diabetes health-care team to make lifestyle and medication changes that will improve your blood glucose (sugar) levels. How often should you check your blood glucose (sugar) levels? How frequently you check your blood glucose (sugar) levels should be decided according to your own treatment plan. You and your health-care provider can discuss when and how often you should check your blood glucose (sugar) levels. Checking your blood glucose (sugar) levels is also called Self-Monitoring of Blood Glucose (SMBG). How do you test your blood glucose levels? Just How Low Can A Normal Blood Sugar Go In Pregnancy?
Recently in the clinic I was asked “Is 3.8 mmol/l (68 mgs/dl) two hours after my lunch too low? The blood sugar normally varies though out the day: lower before breakfast, peaking just over an hour after a meal and coming back down by two hours. Firstly, sugars at this level do not harm the baby but let us look at what normal sugars in people without GDM are and then we can look at the situation in gestational diabetes and for those with pre-existing diabetes, either Type 1 or Type 2 diabetes. During the night the baby is consuming glucose so the before breakfast or fasting sugar is lower. Based on studies in pregnant women who did not have diabetes (see GDM-Research 1st Sept. 2011) the usual sugar values found are shown in the Table with the range being the levels that more than 95% of women had sugar readings between: Fasting 1 Hour after meal 2 Hours after meal Units Average Range Average Range Average Range 4.5 3.7 – 5.2 6.1 4.6 – 7.5 5.5 4.4 – 6.7 mmol/l 81 67 – 94 109 83 – 135 99 79 – 120 mgs/dl These are the numbers are in pregnant women without diabetes. When dealing with women with GDM the important issue is what treatment are they on. If on diet alone then numbers falling below this typical range are likely not a concern. Their meter may be inaccurate and reading low (see GDM-Research 25th Mar 2011), this just may be “normal” for them, perhaps the release of insulin from their pancreas is a little sluggish and peaks later causing the lower numbers. If the low readings continue or the mom feels unwell with them then an earlier snack may help. Gestational diabetes does not increase the risk of birth defects or the risk that the baby will be diabetic at birth. Also called gestational diabetes mellitus (GDM), this type of diabetes affects between 3% and 20% of pregnant women. It presents with a rise in blood glucose (sugar) levels toward the end of the 2nd and 3rd trimester of pregnancy. In 90% if cases, it disappears after the birth, but the mother is at greater risk of developing type 2 diabetes in the future. Cause It occurs when cells become resistant to the action of insulin, which is naturally caused during pregnancy by the hormones of the placenta. In some women, the pancreas is not able to secrete enough insulin to counterbalance the effect of these hormones, causing hyperglycemia, then diabetes. Symptoms Pregnant women generally have no apparent diabetes symptoms.
