Source: Web exclusive: June 2011 When you have diabetes, perhaps the most important thing you need to know is the level of your blood glucose, also known as your blood sugar. Since many factors can raise or lower your blood glucose, you may have to check it several times a day. But once you obtain a blood glucose reading, what exactly does it mean? Crunch those numbers When you test a drop of your blood with a glucose meter, the big number that pops onto the screen refers to the number of millimoles (mmol) of glucose per litre (L) of your blood. A millimole (mmol) is one-thousandth of a mole, which is a standard unit for measuring the mass of molecules. And if that’s not already confusing enough, the United States uses a completely different system than Canadians for measuring blood glucose. South of the border, blood glucose is measured in milligrams per decilitre (mg/dL). This can sometimes be rather bewildering, especially if you’re brand new to diabetes and researching your disease on the Internet. “I tell people to go to a Canadian site first,” says Tabitha Palmer, a certified diabetes educator at the Centre for Clinical Research in Halifax. Know your targets So what numbers should you be looking for? Your target reading before meals should be between 4 and 7. Your blood sugar normally spikes two hours after a meal, so between 5 and 10 is a good range after you eat. Besides food, other factors that can cause your blood sugar to go up or down include exercise, illness, medications and stress. What Are Normal Blood Glucose Levels?
If you get a physical every year, chances are that your doctor orders a blood test that will tell you, among other things, if your blood sugar level is “normal.” If it’s not, you may have diabetes, or be at risk for it in coming years. But what’s this strange thing called “normal” anyway? There’s two main ways to measure blood glucose, depending on where you’re located. If you’re in Europe: In Europe, blood sugar is measured using millimoles per litre. A “normal” blood glucose level comes in at around 4 – 7 mmol/L or 4 – 8 mmol/L for a child with Type 1 diabetes before meals. Two hours after a meal, a normal blood sugar range should be under 9 mmol/L for people with T1D. For T2D, the upper range is slightly lower at 8.5 mmol/L. If you’re in the US: The clinical definition puts “normal” blood glucose at 70-120 mg/dL (milligrams per decilitre) if you’ve fasted eight to twelve hours, or 70-160 mg/dL if you did not fast. That probably makes perfect sense if you have “M.D.” after your name. If you don’t, here’s the translation: 70 to 120 milligrams per deciliter. Clear as … uh…. blood, right? Don’t worry, it’s just the mathematics of measuring density. High Glucose: What It Means And How To Treat It
What is high blood glucose? People who do not have diabetes typically have fasting plasma blood glucose levels that run under 126 mg/dl. Your physician will define for you what your target blood glucose should be — identifying a blood glucose target that is as close to normal as possible that you can safely achieve given your overall medical health. In general, high blood glucose, also called 'hyperglycemia', is considered "high" when it is 160 mg/dl or above your individual blood glucose target. Be sure to ask your healthcare provider what he or she thinks is a safe target for you for blood glucose before and after meals. If your blood glucose runs high for long periods of time, this can pose significant problems for you long-term — increased risk of complications, such as eye disease, kidney disease, heart attacks and strokes and more. High blood glucose can pose health problems in the short-term as well. Your treatment plan may need adjustment if the blood glucose stays over 180 mg/dl for 3 days in a row. It is important to aim to keep your blood glucose under control, and treat hyperglycemia when it occurs. What are the symptoms of high blood glucose? What Your Blood Glucose Numbers Mean
In order to get a complete picture of your blood glucose throughout the day, it is useful to test at different times. Your doctor will help you set your blood glucose targets. Here is what the numbers can mean. These examples are based upon the American Diabetes Association guidelines. Fasting blood glucose before breakfast. This reading on an empty stomach shows how well you use the long-acting insulin that you take. The number should be between 4 and 7 mmol/L. Pre-meal blood glucose before lunch and dinner. This reading shows the effectiveness of your breakfast and lunch insulin doses. The number should be between 4 and 7. Two hours after eating. Your blood glucose peaks a few hours after you eat. This reading shows if the insulin you took was enough to cover the carbs you ate. The reading should be less than 10 mmol/L. Just before bedtime. A target range for someone with diabetes is 6 to 8 mmol/L. You don't want to go to bed with blood glucose that is too low, because that puts you at risk of having a severe low blood sugar episode during the night. Blood Glucose Level Ask Your Doctor If your blood glucose before breakfast is lower than 4 mmol/L, that may be too low. Ask your doctor if you should eat a small snack before you go to bed or reduce your bedtime dose of long-acting insulin. You might want to do a 3:00 am blood glucose check. If the reading before breakfast is over 7 mmol/L, and several 3:00 am readings are in your target range, you may be experiencing "Dawn Phenomenon". Eag: Estimated Average Glucose Levels
The American Diabetes Association (ADA) is now recommending the use of a new term in the management of diabetes called an eAG or, estimated average glucose. This new term was introduced so that healthcare providers could give their patients their A1c data in the same units used when self-monitoring blood sugar, which is either mg/dL in the USA or mmol/L in most of the rest of the world. Your A1c test result is expressed in percentage values, such as a 7%. That 7% A1c test result correlates to a 154 mg/dL (or 8.6mmol/L), which is the measurement used when you test your blood sugar with your meter. To understand your A1c, be sure to read HbA1c: Everything You Need to Know.. So if you got an A1c test result of 7% then your eAG is 154 mg/dL (8.6mmol/L). How Blood Sugar Affects Your Body
When you have diabetes, your blood sugar (glucose) levels may be consistently high. Over time, this can damage your body and lead to many other problems. How much sugar in the blood is too much? And why is high glucose so bad for you? Here’s a look at how your levels affect your health. They're less than 100 mg/dL after not eating (fasting) for at least 8 hours. And they're less than 140 mg/dL 2 hours after eating. During the day, levels tend to be at their lowest just before meals. For most people without diabetes, blood sugar levels before meals hover around 70 to 80 mg/dL. For some people, 60 is normal; for others, 90. What's a low sugar level? It varies widely, too. Many people's glucose won't ever fall below 60, even with prolonged fasting. When you diet or fast, the liver keeps your levels normal by turning fat and muscle into sugar. A few people's levels may fall somewhat lower. Doctors use these tests to find out if you have diabetes: Fasting plasma glucose test. The doctor tests your blood sugar levels after fasting for 8 hours and it’s higher than 126 mg/dL. Oral glucose tolerance test. After fasting for 8 hours, you get a special sugary drink. Hyperglycemia: When Your Blood Glucose Level Goes Too High
Hyperglycemia means high (hyper) glucose (gly) in the blood (emia). Your body needs glucose to properly function. Your cells rely on glucose for energy. Hyperglycemia is a defining characteristic of diabetes—when the blood glucose level is too high because the body isn't properly using or doesn't make the hormone insulin. You get glucose from the foods you eat. Carbohydrates, such as fruit, milk, potatoes, bread, and rice, are the biggest source of glucose in a typical diet. Your body breaks down carbohydrates into glucose, and then transports the glucose to the cells via the bloodstream. Body Needs Insulin However, in order to use the glucose, your body needs insulin. This is a hormone produced by the pancreas. Insulin helps transport glucose into the cells, particularly the muscle cells. People with type 1 diabetes no longer make insulin to help their bodies use glucose, so they have to take insulin, which is injected under the skin. People with type 2 diabetes may have enough insulin, but their body doesn't use it well; they're insulin resistant. Some people with type 2 diabetes may not produce enough insulin. Mean Blood Glucose And Biological Variation Have Greater Influence On Hba1c Levels Than Glucose Instability
OBJECTIVE—Mean blood glucose (MBG) over 2–3 months is a strong predictor of HbA1c (A1C) levels. Glucose instability, the variability of blood glucose levels comprising the MBG, and biological variation in A1C (BV) have also been suggested as predictors of A1C independent of MBG. To assess the relative importance of MBG, BV, and glucose instability on A1C, we analyzed patient data from the Diabetes Control and Complications Trial (DCCT). RESEARCH DESIGN AND METHODS—A glucose profile set and sample for A1C were collected quarterly over the course of the DCCT from each participant (n = 1,441). The glucose profile set consisted of seven samples, one each drawn before and 90 min after breakfast, lunch, and dinner and one before bedtime. MBG and glucose instability (SD of blood glucose [SDBG]) were calculated as the arithmetic mean and SD of glucose profile set samples for each visit, respectively. A statistical model was developed to predict A1C from MBG, SDBG, and BV, adjusted for diabetes duration, sex, treatment group, stratum, and race. RESULTS—Data from 32,977 visits were available. The overall model was highly statistically significant (log likelihood = −41,818.75, likelihood ratio χ2[7] = 7,218.71, P > χ2 = 0.0000). MBG and BV had large influences on A1C based on their standardized coefficients. SDBG had only 1/14 of the impact of MBG and 1/10 of the impact of BV. CONCLUSIONS—MBG and BV have a large influence on A1C, whereas SDBG is relatively unimportant. Consideration of BV as well as MBG in the interpretation of A1C may enhance our ability to monitor diabetes management and predict complications. Glucose Test During Pregnancy For Gestational Diabetes
Congratulations! A baby is on the way. Your nine months will be filled with preparations, from decorating the nursery to stocking up on bibs and booties to going for regular checkups to ensure that you and your baby are as healthy as possible. One of the tests that you’ll have during this time is to check for gestational diabetes. A few weeks ago, we looked at Type 1 and Type 2 diabetes. Gestational diabetes, which is growing more common among pregnant women, will be our focus this week. What is gestational diabetes? Gestational diabetes, or GDM for short, is diabetes that occurs during pregnancy. In fact, it only occurs during pregnancy. (Gestational diabetes is not the same as diabetes in women who have existing diabetes and become pregnant). Diabetes, as most of you know, is a condition in which blood glucose levels go too high. High blood glucose levels can be harmful to you and, in the case of pregnancy, to your unborn child. Fortunately, blood glucose, or sugar, levels can be controlled during pregnancy, and in most instances, high blood sugar levels return to normal after the baby is delivered. According to the National Institutes of Health, up to 10% of pregnant women in the United States have gestational diabetes. What causes gestational diabetes? A lot of changes occur in the body during pregnancy, many of them occurring due to widely fluctuating hormone levels. The placenta, which is what connects the baby to the mother’s uterine lining, makes various hormones, and while this is a good thing, these hormones can sometimes make it hard for the body’s insulin to work properly (a condition called insulin resistance). RATE★★★★★ A random blood glucose test is used to diagnose diabetes. The test measures the level of glucose (a type of sugar) in your blood. If your blood glucose level is 200 mg/dL or higher and you have the classic symptoms of high blood sugar (excessive thirst, urination at night, blurred vision and, in some cases, weight loss) your doctor may diagnose you with diabetes. If you do not have any symptoms of high blood sugar, your doctor will probably have you take another test for further evidence of diabetes.1 Usually, having high blood glucose can be a sign that your body is not functioning normally and that you may have diabetes. If you have high blood glucose and it is not treated, it can lead to serious health complications.
