The association between asthma and type 1 diabetes, two chronic, immune-mediated diseases, has been of longstanding interest, but the evidence is still conflicting. We examined this association in a large, nationwide case-cohort study among Finnish children, using a novel statistical approach. Among the initial cohort of all children born between 1 January 1981 and 31 December 2008, those who were diagnosed with asthma (n = 81 473) or type 1 diabetes (n = 9541) up to age 16 years by the end of 2009 were identified from the Central Drug Register maintained by the Social Insurance Institution of Finland. A 10% random sample from each initial birth year cohort was selected as a reference cohort (n = 171 138). The association between asthma and type 1 diabetes was studied using a multistate modelling approach to estimate transition rates between healthy and disease states since birth. Hazard ratios (HRs) with 95% confidence intervals (CIs) were calculated to represent the change in the transition rate between the disease states. After adjusting for sex and birth decade, previous diagnosis of asthma increased the risk of subsequent type 1 diabetes by 41% (95% CI: 1.28, 1.54), whereas previous diagnosis of type 1 diabetes decreased the risk of subsequent asthma by 18% (95% CI: 0.69, 0.98). The findings of the present study imply that the association between the diseases is more complex than previously thought, and its direction depends on the sequential appearance of the diseases. Asthma May Raise Risk of Diabetes, Heart Disease Study Shows Link Between People With Asthma and Diabetes, Heart Risk March 21, 2011 -- Asthma may increase your risk of developing diabetes and heart disease , shows new research presented at the annual meeting of the American Academy of Allergy , Asthma & Immunology in San Francisco. The common denominator between these conditions appears to be inflammation, according to researchers led by Young J. Juhn, MD, MPH, of the Mayo Clinic in Rochester, Minn. Juhn and colleagues followed 2,392 people with asthma and 4,784 people without asthma from 1964 to 1983. People with asthma were at higher risk for developing diabetes and heart disease , but not inflammatory bowel disease or rheumatoid arthritis , the study showed. Specifically, the diabetes rate in people without asthma was 104 per 100,000 people compared to 138.4 per 100,000 people among those with asthma. For heart disease , the rate in people without asthma was 134 per 100,000 people vs. 188.6 per 100,000 among those participants with asthma. While its important for clinicians to be aware of the increased risks of coronary artery disease and diabetes in asthmatics, these findings should be interpreted cautiously given the preliminary nature, Juhn says in a news release. Aim: The aim of this study is to evaluate the prevalence of asthma in type 2 DM patients,presented in IKEDA Hospital, in the period of time January 2010December 2012. Patients and methods: We examined all patients diagnosed with type 2 DM. We excluded patients taking other medications than antidiabetics, with familial history of asthma or other illness, and patients with any allergic risk factors. Type 2 DM has been diagnosed according to WHO criteria of 1985, and Bronchial Asthma was diagnosed with the use of GINA criteria. Results: The total number of Type 2 diabetic patients was 240, of which 150 females and 90 males. The mean age was 58,5 (range 4374 years old). 32 cases (13.3%) of the total number resulted with Bronchial Asthma. Most cases of them were evidenced in females, in the group-age of 5060 years old, in patients with older history of diabetes and with/or higher BMI. The most important result is the fact that the prevalence of asthma is lower in diabetic patients taking antiinsulin resistance therapy, as biguanides and thiazolidinediones. According to the study, nearly 11 percent of almost 2,000 children and young adults under age 21 with diabetes also had asthma. According to the Centers for Disease Control and Prevention, 9 percent of children and young adults in the country have asthma. What the study also found was that children who suffered the double whammy of both diabetes and asthma had a tougher time controlling their blood sugar levels than the children who had only diabetes, 15.5 percent to 9 percent. Susan McQuade, respiratory therapist at Hendrick Medical Center stated that, children who have both diseases have more things to remember to do or not do and to monitor, no easy tasks for children or their parents. McQuade said, however, shes not sure exactly how the two conditions, along with obesity, are linked. Both diabetes and asthma are inflammatory, but for a long time we didnt think they were related, she said. Even though this study shows a link, there still needs to be further research. Response from Leon Lebowitz, RRT: The medical treatment of asthma focuses on a step-wise approach, as recommended by the National Heart, Lung, and Blood Institute. Medications can include rescue inhalers, inhaled corticosteroids (ICS), long acting beta agonists (LABAs), and oral corticosteroids, to name a few. The medical literature reports that ICS are the most effective medications for the reliable control of long-term asthma. Inhaled corticosteroids are the very backbone of asthma treatment for most adults and children with persistent asthma. Oral corticosteroids tend be used in the treatment of severe acute asthma and may be used at the beginning or end of an attack, usually on a short-term basis. They are only used on a long-term basis to treat patients with the most severe asthma. Corticosteroids carry a risk of side effects, some of which can cause serious health problems. Because oral corticosteroids affect the entire body (instead of just a particular area, as with aerosols), this route of administration is the most likely to cause serious side effects. When these are used over a long period of time, you may experience high blood sugar. High blood sugar can trigger or worsen diabetes, which is the main concern you expressed. In order to get the most benefit from corticosteroid medications with the least amount of risk, I would suggest trying lower doses or intermittent dosing, with your physician directing treatment.
