The hemoglobin A1c test, as we all know, is supposed to give a sense of your average blood glucose levels over the past three months. But here’s a question for you: have you ever tried to figure out what those average blood glucose levels actually are? Say you have an A1c of 6.5% — what, in mg/dl, does that translate to? Try searching Google — it’s hard to find an answer. To quote from a post I wrote a few years ago (see entry from 4:45), that’s partially because: “Not only is there no one standardized definition as to the correlation between A1c and mean glucose levels (JDRF says 1% = 24.4 mg/dl, ADA says 28.7), but different people have different correlations. For example, if you are a ‘high glycolator’ (more glucose sticks to your hemoglobin than the average) you can have a relatively high A1c but a low mean glucose. The speaker gave the example of a patient who had a 8.2% A1c, but a mean glucose of 159 mg/dl (he was speaking using the generally accepted idea that 7% roughly equals a mean of 154 mg/dl). Treat him more aggressively, and you’ll end up with hypos. K.P. is a 44-year-old woman who was diagnosed with type 1 diabetes 22 years ago. She has been diagnosed with background retinopathy, which has been stable, and microalbuminuria, which is well-controlled on an angiotensin-converting enzyme (ACE) inhibitor. She has hypercholesterolemia, which is controlled with simvastatin (Zocor), and neuropathy. She has no history of hypertension, tobacco abuse, or cardiovascular disease. She currently is treated with ultralente insulin and a rapid-acting insulin in boluses before each meal. She has had good control, as determined by HbA1c levels averaging 7% over the past 2 years. Recently, however her HbA1c increased to 9%. K.P. is 5′ 3′′ tall, and her weight has recently decreased unintentionally from 115 to 110 lb. She works as a special education teacher and drives daily between two schools. One week ago, she was found to be disoriented and hypoglycemic after her car ran into a curb while she was driving. Her blood glucose level was 26 mg/dl as measured by the paramedics who were called to the scene. After treatment, she stated that she had eaten lunch 2 h before the accident and that her blood glucose before lunch had been 130 mg/dl. Most blood glucose meters store a certain number of readings in their memory, along with the date and time of each reading. Most also report either a 14-day or 30-day average of readings. An average is calculated by adding up all the numbers in a set and dividing the sum by the number of numbers in the set. For example, if you checked your blood glucose level 25 times in 14 days, your meter would tally all of your readings and divide the sum by 25 to get your 14-day average. Blood glucose averages can be useful, but they can also be misleading. 1-hour Glucose Test 178 Score...
Finally got my 1-hour glucose results back (2 weeks later cuz the lab forgot to send them to my dr), and it was 178 so I have to do my 3-hour in the morning. Has anyone had a 1-hour result that high and passed the 3-hour? Seriously nervous I'm in the same boat as you. I failed mine with a 132 and have to do the 3 hour....praying I pass, I have so much going wrong with me as it is GD is the last thing I need! Thanks, prayers for you as well! And I hear you about this being the last thing you need. I've been on bedrest since 24 weeks (31 weeks now), and getting weekly progesterone shots. I might lose my mind if I had to do finger pricks and change my diet as well... I failed the one hour with a 178. Passed two out of three on the three hour test. I passed my one hour but I was nervous because I'm a little overweight and at times my blood pressure can be a few numbers off so I found out a protein heavy breakfast and water is good before drinking all of that sugar. One of the topics that comes up a lot in the email I get from visitors to my What They Don't Tell You About Diabetes web site is the question of when is the best time to test your blood sugar. A lot of doctors still tell people with Type 2 to test first thing in the morning and before meals. That was what I was told at diagnosis in 1998. People who test using this schedule may tell you their blood sugar is usually 120 mg/dl, which sounds pretty good, except that since this is a fasting number it usually hides the information that the person's blood sugar maybe going to 250 mg/dl or higher after every meal. Research has shown that for people with Type 2 diabetes--especially those who have been diagnosed recently and still retain some beta cell function--it is the high spikes after meals that contribute most heavily to raising the A1c and causing complications. If you only test your fasting blood sugar, you will not know anything about how high your blood sugar is spiking after meals, so you won't know which foods are toxic to you because they cause dangerous spikes. If you are like most people with Type 2 your access to the very expensive blood sugar testing strips is limited. You may have to pay for strips yourself or your insurance may pay for a single box each month. That means that you need to use each strip as efficiently as possible. Here are some strategies that you can use to get the information out of your blood tests that will let you drop your A1c back into the healthy zone. Instead, blood sugar rises slowly and gradually, causing complications that may damage your organs -- heart, eyes, kidneys, nerves, feet, and even skin are at risk. Sometimes you wonder, "Is my blood sugar too high? Too low?" because "normal" levels are so important. "Diabetes is not a 'one-size-fits-all' condition, and neither are blood sugar readings. Different targets are established for different populations," says Amber Taylor, M.D., director of the Diabetes Center at Mercy Medical Center in Baltimore, Maryland. Targets may vary depending on a person's age, whether they have type 1 or type 2 diabetes and for how long, what medications they're taking, whether they have complications, and, if the patient is a female, whether she is pregnant. It doesn't seem fair, does it? You haven't eaten anything all night and you still wake up with high blood glucose! What is going on and how can you lower it? For those of you with diabetes, this post is for you! Basics Your body strives to keep blood glucose (BG) within a safe range, but with diabetes the balance is disturbed. The insulin your pancreas produces might not be sufficient to cover your BG lowering needs. As well, the insulin it does produce might not be handled properly by target body cells. And to make matters worse, your liver might be on glucose production overdrive. All of this results in your body's failure to control BG overnight as well as after meals or snacks. Dawn Phenomenon Due to normal daily changes in our body's release of hormones during sleep and wake cycles, BG typically starts to rise in the very early morning – starting about 3 AM - and continues to rise as the morning progresses. For folks without diabetes, insulin production simply increases and takes care of the rising BG. However, for folks with diabetes, unless there is medication on board to cover this early morning rise, BG will eventually rise out of target range by the time they wake up. The classic pattern is to see BG within target range at both bedtime and during the middle of the night, and then see a high fasting BG. There are many different times you can test your blood sugar. While a fasting blood sugar test, one taken when you have not had anything to eat or drink for the previous eight hours, is typically used to diagnose diabetes, testing at other times throughout the day can help you keep your blood sugar under control. Depending on your needs, you may test your blood sugar as little as twice daily or as frequently as seven or more times daily. Your doctor will recommend when and how frequently you should test your blood sugar. At-home blood sugar testing is usually performed with a hand-held monitor. Video of the Day Before and After Meal Testing A blood glucose level measured before a meal other than breakfast is typically a non-fasting blood sugar. Checking your blood sugar before a meal can help you choose which foods you can eat, which foods you should avoid, and how much insulin you should take if you are on a sliding scale. Testing your blood sugar about two hours after your meal lets you know how your body is processing your meal and whether you have enough insulin in your system to handle the food you ate. Fasting and before meal blood sugars should be in the 80 to 120 mg/dL range and after meal blood sugars should be less than 180 mg/dL, or as directed by your doctor.
