Your blood glucose levels can determine whether you have or are at risk for developing diabetes, a condition in which your body no longer effectively processes and absorbs glucose from the bloodstream. Blood glucose levels fluctuate during the day, particularly after meals. Postprandial -- which means after eating -- glucose levels that rise beyond a certain level may mean you have diabetes or prediabetes. However, two-hour postprandial blood sugar testing is not recommended to screen for or diagnose diabetes. Video of the Day Two to 3 hours after eating a meal, blood glucose levels typically fall to normal fasting levels. For people without diabetes, this is typically 125 mg/dL or less, according to criteria established by the American Diabetes Association. If your 2-hour postprandial blood glucose level is higher than 125 mg/dL, your doctor will likely order one of the ADA-recommended blood tests for diagnosing diabetes. Blood Glucose Testing Importance
Author's Perspective: As engineers, we just love numbers -- we love everything about numbers -- counting, recording, adding, subtracting, multiplying, dividing, collecting and analyzing the numbers. We really love doing all of this to help solve a complex problem. As it turned out, blood glucose testing was a Godsend for me. It helped me to separate fact from fiction, and also how to better use the data to help me to improve and optimize my blood glucose control. During my recovery, my mother told me: "Son, God gave you the perfect disease for an engineer who loves mathematics." She was right -- being able to collect and analyze the data put me in a comfort zone that allowed me to safely wean off the insulin. During my research, I discovered that most books written on diabetes and most doctors and diabetes experts only used blood glucose testing to determine the drug dosage, the number of carbs that a diabetic could eat, and whether there was a problem with a specific food. I couldn't find a diabetes book or expert that explained how to leverage the data to actually reverse the diabetes! Death to Diabetes is the only book and the only program that I'm aware of that leverages the data (and the other blood testing data) to reverse the disease! In addition, so that you don't have to wait to get the test results from the medical lab, the data can also be used to extrapolate hemoglobin A1C readings. Exercise at least 30 minutes a day, 5 days a week. Do muscle strengthening exercises 2 or more days a week. Avoid sitting for more than 30 minutes at a time. Try speed walking, swimming, or dancing. Pick an activity you enjoy. Always check with your doctor before starting any new exercise plans. Follow your meal plan. Take your medicines the way your health care provider recommends. Checking your blood sugar levels often and writing down the results will tell you how well you are managing your diabetes. “Gary, I think I need more insulin at breakfast.” “Why do you say that, Julianne?” “Because I’m always having high readings right afterwards, and my obstetrician said I shouldn’t spike after I eat.” “And what happens after the spike?” “It usually comes down to normal before lunch. So do you think I should take more insulin?” After-meal blood sugar spikes can create quite a quandary for anyone with diabetes, particularly during pregnancy. Research has shown that fetal macrosomia (overgrowth of the baby) becomes more common when post-meal blood sugars exceed 120 mg/dl (6.7 mmol). With post-meal readings above 140 mg/dl (7.8 mmol), the risk more than doubles from baseline. Fetal macrosomia can cause many problems during pregnancy. When the baby grows and develops too rapidly, it can lead to a premature and more complicated birth. It may also cause injuries to occur to the baby during delivery. Why do after-meal blood sugars have such a major influence on the baby’s growth? Nobody knows for certain. Perhaps, when the mother’s blood sugar “spikes” suddenly after meals, the baby is fed more sugar than its pancreas can “cover” with insulin, and high fetal blood sugar results. And because the baby’s kidneys spill almost all excess sugar from the baby’s bloodstream back into the amniotic fluid, the baby then drinks in the extra glucose and winds up growing more than it should. Checking your blood sugar (glucose) levels at home is part of your type 2 diabetes management plan. You can use the results of these tests to help improve your blood sugar control. But if you’re only testing first thing in the morning, you might be missing the full picture. “I like to tell my patients that the first morning glucose level check is usually the lowest of the day, and checking only in the morning is akin to purposefully ‘blindfolding’ yourself to only see the best-case scenario,” says endocrinologist Ildiko Lingvay, MD, MPH, an associate professor in the department of internal medicine/endocrinology at the University of Texas Southwestern Medical Center in Dallas. “If you want to see a more complete picture, I recommend patients mix it up and check their glucose level at various times during the day — and, most importantly, when they know they might have not followed the best advice — to see the full effect of their choices.” The American Diabetes Association (ADA) recommends checking blood sugar levels before eating (fasting blood sugar) and then again one to two hours after the beginning of a meal — particularly if target A1C goals aren’t being met. The American Diabetes Association defines GDM as any degree of glucose intolerance with onset or first recognition during pregnancy. This includes women who may have had diabetes or abnormal glucose tolerance prior to the pregnancy but were undiagnosed. In the U.S., the prevalence of gestational diabetes varies from 1-14% depending on the population studied. Poorly controlled gestational diabetes increases the risk of fetal macrosomia with risk of shoulder dystocia or other birth injury, C-section, maternal hypertensive disorders, neonatal hypoglycemia, jaundice, hypocalcemia, and polycythemia. Fasting hyperglycemia with blood glucose > 105 mg/dl (5.8 mmol/l) may be associated with an increased risk of intrauterine fetal demise in the last 4-8 weeks of gestation. Since organogenesis is complete by the time that gestational diabetes appears, major congenital anomalies are uncommon. During Postprandial Glucose study, a meal acts as a glucose challenge to the body’s metabolism. Insulin is normally secreted immediately after a meal in response to the elevated blood glucose level, causing the level to return to the premeal range within 2 hours. In patients with diabetes the glucose level usually is still elevated 2 hours after the meal. The PPG is an easily performed screening test for DM. If the results are greater than 140 and less than 200 mg/dL, a glucose tolerance test may be performed to confirm the diagnosis. If the 2-hour PPG is greater than 200 mg/dL, the diagnosis of DM is confirmed. Also, a glucose tolerance or glycosylated hemoglobin test can be performed to corroborate and better evaluate the disease. The 2-hour PPG test is a measurement of the amount of glucose in the patient’s blood 2 hours after a meal is ingested (postprandial). It is used to diagnose diabetes mellitus (DM). When the 2 Hour Postprandial Glucose test is performed, a fasting blood glucose test is performed before the patient takes the meal. The patient should start a 12 hours of fasting before performing the test.
