Causes Insulin is a hormone produced in the pancreas by special cells, called beta cells. The pancreas is below and behind the stomach. Insulin is needed to move blood sugar (glucose) into cells. Inside the cells, glucose is stored and later used for energy. When you have type 2 diabetes, your fat, liver, and muscle cells do not respond correctly to insulin. This is called insulin resistance. As a result, blood sugar does not get into these cells to be stored for energy. When sugar cannot enter cells, a high level of sugar builds up in the blood. This is called hyperglycemia. The body is unable to use the glucose for energy. This leads to the symptoms of type 2 diabetes. Type 2 diabetes usually develops slowly over time. Most people with the disease are overweight or obese when they are diagnosed. Increased fat makes it harder for your body to use insulin the correct way. Type 2 diabetes can also develop in people who are thin. This is more common in older adults. Family history and genes play a role in type 2 diabetes. Low activity level, poor diet, and excess body weight around the waist increase your chance of getting the disease. Regular blood glucose testing helps you determine how well your diabetes management program of meal planning, exercising and medication (if necessary) is doing to keep your blood glucose as close to normal as possible. The results of the nationwide Diabetes Control and Complications Trial (DCCT) show that the closer you keep your blood glucose to normal, the more likely you are to prevent diabetes complications such as eye disease, nerve damage, and other problems. For some people, other medical conditions, age, or other issues may cause your physician to establish somewhat higher blood glucose targets for you. The following chart outlines the usual blood glucose ranges for a person who does and does not have diabetes. Does this test have other names? Glucose, postprandial; glucose, two-hour postprandial; two-hour PPG; two-hour postprandial blood sugar What is this test? This is a blood test to check for diabetes. If you have diabetes, your body doesn't make enough insulin to keep your blood sugar in check. This means your blood sugar levels are too high, and over time this can lead to serious health problems including nerve and eye damage. A 10-minute Walk After A Meal 'good For Diabetes'

"Short stroll after meals better for blood sugar than walks at other times," says The Daily Telegraph. The story is based on a study which aimed to see whether taking a 10-minute walk after a main meal resulted in lower blood glucose levels than a single 30-minute walk each day for people with type 2 diabetes. The study, which involved 41 adults, found that taking shorter, more frequent walks immediately after meals reduced blood glucose by around 12% compared with a single 30-minute walk. The greatest benefit was seen after the evening meal when carbohydrate consumption was high and participants tended to be less active. The researchers did not explain why a post-meal walk was more effective at lowering glucose in the blood. However, they believe a short stroll after every meal could reduce the need for insulin injections which can help people better manage their weight. Researchers say current physical activity guidelines should be changed to specifically include post-meal activity, especially after meals with lots of carbohydrate, such as bread, rice, potatoes and pasta. There are 4 million people living with diabetes in the UK and 90% of those have type 2 diabetes, according to Diabetes UK. Type 2 diabetes occurs when the body doesn't produce enough insulin to function properly, or the body's cells don't react to insulin. Due to the short duration of this study we can't be sure that the effect seen in this research would last and lead to an overall improvement in blood glucose levels and diabetes control. Regina Castro, M.D. Reactive hypoglycemia (postprandial hypoglycemia) refers to low blood sugar that occurs after a meal — usually within four hours after eating. This is different from low blood sugar (hypoglycemia) that occurs while fasting. Signs and symptoms of reactive hypoglycemia may include hunger, weakness, shakiness, sleepiness, sweating, lightheadedness and anxiety. It's possible to have symptoms that are similar to reactive hypoglycemia without actually having low blood sugar. True reactive hypoglycemia symptoms that are caused by low blood sugar occurring after eating are uncommon. For the majority of people with postprandial symptoms, the actual cause of the symptoms is not clear but may relate to what food was eaten or variations in the timing of the food moving through the stomach and intestinal tract. Generally, a medical evaluation is done to determine whether symptoms are caused by low blood sugar — and whether symptoms resolve once blood sugar returns to normal. Further evaluation of reactive hypoglycemia depends on the severity of symptoms. For the majority of people, reactive hypoglycemia usually doesn't require medical treatment. It may help, however, to pay attention to the timing and composition of your meals: Eat a well-balanced diet, including lean and nonmeat sources of protein, and high-fiber foods, including whole grains, fruits and vegetables. Glucose is a simple sugar and approximately 4 grams of glucose are present in the blood of humans at all times.[2] The body tightly regulates blood glucose levels as a part of metabolic homeostasis.[2] Glucose is stored in skeletal muscle and liver cells in the form of glycogen;[2] in fasted individuals, blood glucose is maintained at a constant level at the expense of glycogen stores in the liver and skeletal muscle.[2] In humans, glucose is the primary source of energy, and is critical for normal function, in a number of tissues,[2] particularly the human brain which consumes approximately 60% of blood glucose in fasted, sedentary individuals.[2] Glucose can be transported from the intestines or liver to other tissues in the body via the bloodstream.[2] Cellular glucose uptake is primarily regulated by insulin, a hormone produced in the pancreas.[2] Glucose levels are usually lowest in the morning, before the first meal of the day, and rise after meals for an hour or two by a few millimoles. “Gary, I think I need more insulin at breakfast.” “Why do you say that, Julianne?” “Because I’m always having high readings right afterwards, and my obstetrician said I shouldn’t spike after I eat.” “And what happens after the spike?” “It usually comes down to normal before lunch. So do you think I should take more insulin?” After-meal blood sugar spikes can create quite a quandary for anyone with diabetes, particularly during pregnancy. Research has shown that fetal macrosomia (overgrowth of the baby) becomes more common when post-meal blood sugars exceed 120 mg/dl (6.7 mmol). With post-meal readings above 140 mg/dl (7.8 mmol), the risk more than doubles from baseline. Fetal macrosomia can cause many problems during pregnancy. When the baby grows and develops too rapidly, it can lead to a premature and more complicated birth. It may also cause injuries to occur to the baby during delivery. Why do after-meal blood sugars have such a major influence on the baby’s growth? Nobody knows for certain. Perhaps, when the mother’s blood sugar “spikes” suddenly after meals, the baby is fed more sugar than its pancreas can “cover” with insulin, and high fetal blood sugar results. And because the baby’s kidneys spill almost all excess sugar from the baby’s bloodstream back into the amniotic fluid, the baby then drinks in the extra glucose and winds up growing more than it should. Suffice to say that post-meal blood sugar spikes are something to avoid during pregnancy. But how do we do it? Pre-diabetes (previously called Impaired Glucose Tolerance IGT) was first named in 2003 and is designed to foster attention and action in people who receive this diagnosis. It is defined as having a blood glucose level that is higher than normal, but not high enough to be classified as diabetes. The cutoff for pre-diabetes is a fasting blood sugar of 100 mg/dl. Fasting levels between 100 and 126 mg/dl are diagnosed as pre-diabetes and a fasting level of 126 mg/dl and up is diabetes. The other determiner of pre-diabetes is a blood sugar level two hours after eating carbs of 140 to 199 mg/dl. A blood sugar under 140 mg/dl is considered normal and one 200mg/dl and over is considered diabetes. Early diagnosis is important. In the early years of pre-diabetes or diabetes, the beta cells are progressively damaged by high blood sugars.Usually by the time diabetes is diagnosed, half of the beta cells are nonfunctional. This can not be reversed so that the beta cells can go back to insulin production. When an early diagnosis of pre-diabetes is made, almost 100 percent of beta cells are functional.
